The facility currently operates with a Five Star Child Care License that was issued on August 28, 2020. The last annual compliance visit was conducted on May 27, 2025. Upon arrival, I was greeted by the program administration team. The Director, Ms. Garner, assisted me on a walk-through of the facility where attendance was taken and observations made. Infants were observed engaging in routine care and floor play. Preschoolers were observed engaging in indoor group time and free play activities as well as outdoor play. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations was observed. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. Medication permission forms for two sunscreens for children in space 2 were expired or not onsite avaliable for review. 10A NCAC 09 .0803(4)(6-9) In order to comply with the NC Laws and Rules any violations cited today must be corrected immediately. A compliance letter must be sent to me by January 9, 2025. The letter must address each violation and explain how it has been corrected. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. TECHNICAL ASSISTANCE: A violation was cited regarding medications not having adequate information or missing medication administration permission forms. We discussed the importance of insuring that medications, including diaper creams and sunscreens, have forms that include all information and are not expired. It is a good idea to send sunscreens home once the season of using them has ended. If they are to remain onsite, they must have a medication permission form accompanying them. We discussed the QRIS. The program is leaning towards the Classroom and Instructional Quality Pathway. The director plans to continue researching the pathway to ensure that they do or can meet all required components. We looked at the QRIS Modernization webpage and I guided the director on how to use it to access the various components of the pathways. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed. Limited monitoring was conducted today. I informed the administrator, Ms. Garner, of the purpose of my visit. The allegation is as follows: There are concerns regarding discipline and nurture and care. Findings: I spoke with 3 staff members, 2 administrators, and 1 support staff. All staff members reported that during outside play, the door to the classroom was propped open. A teacher was inside of the classroom when a young toddler began to enter the classroom. Another toddler who was on the playground began to close the door. One teacher ran to the young toddler and removed them from the space before the door closed on them, and then removed the child who had caused the door to be closed from the area and sat the child down. The administrators stated that the teacher who removed the children was suspended for the remainder of the day and the following day so that they could conduct an investigation. The teacher returned to work at the conclusion of the investigation. Based on my observations and the information gathered: I find the allegation regarding the violation of discipline unsubstantiated. I find the allegation regarding the violation of nurture and care substantiated. I observed children engaging in outdoor play. The following violation was cited. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A teacher removed a child from the classroom outside door in a manner that was not nurturing or developmentally appropriate. G.S. 110-91(10) The above violations must be corrected immediately. Limited monitoring was conducted today. I informed the administrator, Ms. Garner, of the purpose of my visit. Based on my observations and the information gathered: I find the allegation regarding the violation of discipline unsubstantiated. I find the allegation regarding the violation of nurture and care substantiated. I observed children engaging in outdoor play. The following violation was cited. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A teacher removed a child from the classroom outside door in a manner that was not nurturing or developmentally appropriate. G.S. 110-91(10) The above violations must be corrected immediately. The annual compliance checklist was completed during today's visit. The last sanitation inspection was completed May 6, 2024 with a “Superior” classification with 7 demerits. The last fire drill was completed on May 2, 2024. The last Shelter-in-place was completed on May 3, 2024. The last outdoor play inspection was completed on May 23, 2024. In regards to staff records the staff and training worksheet was discussed but due to the size of the facility and the staff and training worksheet not being available during today's visit a return visit will be made with-in two weeks to review these records in detail. The percentage of children's records were reviewed based on the number of children present during today's visit. The children's records were in order and no violations were cited. In addition, when checking the playground monthly make sure to look for hazards that may appear and place them on the form and update when hazard has been corrected. RATED LICENSE INFORMATION Rated License assessments have resumed, and your facility is in Cohort 1. Your preparation year will begin July 1, 2023 and will end on June 30, 2024.
