The facility currently operates with a One Star Child Care License that was issued on September 2, 2020. The last annual compliance visit was conducted on September 26, 2025. Upon arrival, I was greeted by the program director, who accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed engaging in indoor free play and art activities. The last sanitation inspection was completed on March 6, 2026, with a superior classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violation was cited. Violation Number Comment Rule 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. No monthly playground inspections were on site or available for review. .0605(q) The above violation must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by May 7, 2026. You can email the letter to me at caroline.sykes@dhhs.nc.gov. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. The staff were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. TECHNICAL ASSISTANCE: A violation was cited regarding the monthly playground inspections not being completed or available for review. We discussed that the inspections should be completed each month and filed and maintained onsite. It is important to complete the inspections to monitor the playground for safety hazards on a regular basis. 10A NCAC 09 .0605 OUTDOOR LEARNING ENVIRONMENT IN CHILD CARE CENTERS (q) Following completion of playground safety training as required by Rule .1102(e) of this Chapter, a monthly playground inspection shall be conducted by an individual trained in playground safety requirements. A trained administrator or staff person shall make a record of each inspection using a playground inspection checklist provided by the Division. The checklist shall be signed by the person who conducts the inspection and shall be maintained for 12 months in the center's files for review by a representative of the Division. The playground inspection checklist may be found online at https://ncchildcare.ncdhhs.gov/providers/credent.asp. The playground inspection includes a checklist of items related to safety, surfacing, and equipment quality. We discussed the QRIS Pathway to the Stars. The program is currently a one star, but has some interest in increasing stars with the new options, especially the Classroom and Instructional Quality Pathway. We reviewed the components of both the Program Assessment and Classroom and Instructional Quality Pathways. The provider will research the information and decide if she would like to proceed. The last Annual Compliance Visit was conducted on September 26, 2025. The center currently operates with a One Star Child Care License that was issued on September 2, 2020. The owner/director was present and assisted me during the visit. Children were observed engaging in outdoor play. The last sanitation inspection was completed August 4, 2025, with a “Superior” classification. The last fire inspection was conducted on October 4, 2024, and the facility was approved for daytime care only. The operator has contacted the fire inspector to schedule the fire inspection for this year. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. The following violations were observed: Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A. Carroll did not have a valid qualification letter on file. Her qualification had expired and the steps have not been taken to complete the criminal background check process. Be sure that the online portion of the application process is completed and that fingerprints are taken and submitted electronically so that the application can be processed and a qualification determined. It is best to begin the process 1-2 months prior to the expiration of a current qualification letter. We discussed ensuring that all documents in the children’s file requiring the administrator’s signature have that signature. It is a good idea to go through all of the children’s files and add signatures where needed. REMINDERS: -A. Carroll needs 6 training hours before 10/9/25 -A. Carroll will need to retake the Health and Safety trainings by 12/30/2025 -Be sure to make copies of all file documents that are shared between sibling files. The owner/director was present and assisted me during the visit. I completed a walkthrough of the program where attendance was documented, and observations made. The older children were observed eating breakfast and transitioning outside. Younger children were observed eating breakfast and engaging in indoor play. The last sanitation inspection was completed August 4, 2025, with a “Superior” classification. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. A return visit will be conducted to review staff and children’s files. The following violations were observed: Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The monthly fire drill record was not available for review. .0604(t); .0302(d)(5) 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Two children (siblings) had one bottle of shared sunscreen with no authorization form available. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. TECHNICAL ASSISTANCE: We discussed implementation of a new system to store and distribute items that need to be sent home, such as dirty clothes, cups, etc. One suggestion was to keep items in a baggie with the child’s name on it, stored in a bin out of reach of children. Teachers should provide reminders to parents at pick up to check the bin for items that need to go home. Violations were cited regarding medication forms being expired or not present with the medication. It is best to routinely check medications, including diaper creams, sunscreens and bug sprays, to make sure that they have not expired and/or the permission forms are not expired. When expiration occurs, items should be sent home and replaced with a new one if needed. When authorization forms expire, a new form must be completed in order to keep the mediation onsite and continue to administer it. Violations were cited regarding the fire, shelter-in-place and lockdown drills not having been practiced and/or the record of drills not being available for review. It is imperative that the drills are practiced on the required schedule-monthly fire drills and quarterly shelter-in-place/lockdown drills-so that staff and children know what to do in the event of one of these emergencies. The record for these drills should be kept onsite, and drills should be recorded on the form when practiced. We discussed that it may be beneficial to hang the drill log in the center and update it when the drills take place. We discussed the new QRIS and the director’s potential interest in increasing stars. I encouraged the owner/director to review the QRIS Modernization page on the DCDEE website to look closer at the various pathways and see if there is one that may best fit the program.
