Name of Operation: BLOSSOMS DAY CARE Facility ID: 68000564 Consultant: CAROLINE SYKES Operation Type: Center Case Number: Visit Date: 10/24/2025 Number Present: 19 Completed Date: 10/24/2025 Age: From 0 To 5 Total Minutes: 240 Time In: 10:00 AM Time Out: 02:00 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor for all applicable Child Care Requirements. The last Annual Compliance Visit was conducted on October 29, 2024. The center currently operates with a Four Star Child Care License that was issued on January 18, 2022. The director was present and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed eating breakfast and participating in indoor free play activities. The last sanitation inspection was completed October 7, 2025, with a “Superior” classification. The last fire inspection was conducted on September 17, 2025, and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. There were no medication permission forms for diaper creams in Space 1 and 1 prescription medication in Space 2. 10A NCAC 09 .0803(1)(a & b) 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. There was a prescription medication in Space 2 that was not stored in the original labeled container. .0803(2)(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. 2 staff members did not complete First Aid within 90 days of employment. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. Two staff members had not completed the required CPR/FA course within 90 days of employment. .1102(d) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. One staff member had not completed the required number of hours of on-going training. .1103(a) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. One teacher in the infant classroom had not completed the ITS-SIDS training within two months of employment. .1102(f) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. One staff member did not complete the required Health and Safety trainings within one year of employment. .1102(a) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by November 7, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM November 7, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. TECHNICAL ASSISTANCE: Violations were cited regarding staff members having not taken the required training courses such as CPR and First Aid courses, Recognizing and Responding to Suspicions of Child Maltreatment, SIDS, Health and Safety and on-going trainings. We discussed the importance of ensuing all staff have taken the required trainings. This ensures continued knowledge and understanding of best practices regarding health, safety, and child growth and development. Violations were cited regarding prescription medication not being stored in its original container with the label, and medications not having required paperwork completed for administration. We discussed the importance of having the prescription label to ensure dosage information is accurately followed, as well as medication permission forms, providing permission to administer medications. We discussed the new QRIS. The director is currently researching and leaning toward the Classroom and Instructional Quality pathway. We discussed instructional quality and the curriculums, including the one that she is looking into purchasing. She will continue to work on getting all staff entered into the NC DCDEE WORKS system for the education component of the license. The last Annual Compliance Visit was conducted on November 8, 2023. The center currently operates with a Four Star Child Care License that was issued on January 18, 2022. The director was not present upon my arrival to the center. She came in a short time later and assisted me during the visit. She accompanied me on a walk-through of the facility where attendance was documented and observations made. Children were observed participating in outdoor play. The last sanitation inspection was completed April 16, 2024 with a “Superior” classification. The last fire inspection was conducted February 8, 2024 and the facility was approved for daytime care only. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were cited. Violation Number Comment Rule 842 A drug or medication was administered without written authorization and/or instructions from a child's parent or authorized health professional. Diaper cream and sunscreen in space 1 for two children did not have permission forms for administering. 10A NCAC 09 .0803(1)(a & b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. In space 3, an expired inhaler had not been returned to the parents or replaced with one that is in date. .0803(12) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete Recognizing and responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) 1898 Staff did not complete the health and safety training within one year of employment. Program staff members have not completed the required Health and Safety trainings within one year of employment. .1102(a) The above violations must be corrected immediately and a letter must be sent to me no later than November 12, 2024 stating how the items were corrected. We discussed the importance of ensuring all staff have taken the required trainings, including Health and Safety and Recognizing and Responding to Suspicions of Child Maltreatment. These trainings are important to prepare teachers for their work with children and families.
