Your program currently operates with a Five Star Center License issued August 25, 2022. Restrictions on the license include a first shift capacity of sixty-five (65) children ages 3 to 6, meets enhanced ratios, meets enhanced ratios minus one from the seven point level, and meets enhanced space. The last annual compliance visit was completed on January 9, 2025. The sanitation inspection was completed on November 7, 2025 with a “Superior” classification. The fire inspection was completed on October14, 2025 and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. Prior to the visit the program’s eighteen-month compliance history was one hundred percent (100%). P. Hester, principal/ administrator and C. Frink, teacher, were present for the visit. Thirty-seven (37) children were enrolled and thirty-three (33) children were present. There are three (3) indoor spaces and one (1) outdoor space currently approved for use. Space 19 is currently not in use and is not set up for use by preschool children. Today we discussed that you will contact me before you plan to re-open this room to preschool children. In spaces 20 and 21, age-appropriate materials were accessible to all children and spaces were arranged into activity areas. Children were observed moving between various activities such as free play that included language and fine motor activities, group time that included music and movement, and outdoor play that included ride-on toys, sliding, and dramatic play in a play house. During all observed activities, the teachers moved about the space and interacted with the children. Supervision and staff-child ratios were observed and found to be in compliance. Lunch consisted of roasted turkey, chicken drumstick, cornbread, rolls, yams, green beans, chilled pears, and milk. There are six (6) staff members employed at this facility. A percentage of veteran staff’s files were reviewed. New staff have not been hired since the last visit. A percentage of children’s files were reviewed. The program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled; this program uses the Creative Curriculum. NC Pre-K monitoring: The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes were verified in compliance. Children attend the program from (7:30am-3:00pm), meeting the instructional day requirement of6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Screening, and TS Gold for their ongoing assessments. Developmental Screenings completed and Health Assessments were on file. NC Pre-K staff information was verified in the NC Pre-K Plan and all staff members meet requirements for qualifications or have been approved by the contracting agency as substitutes. A Site Monitoring visit has been conducted by Columbus County Schools, the contracting agency, during a site visit on October 9, 2025. It was noted that all requirements were in compliance. Families are invited to a beginning of the year meet and greet and ongoing communication occurs through weekly phone calls and regular newsletters, as well as a daily folder. Parent conferences are offered at least twice a year. The following violation(s) were documented: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. A staff member's certification expired in September 2025. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. A staff member's certification expired September 2025. .1102(d) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen (18)-month compliance history score of at least seventy-five (75) percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before December 25, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Renewing this certification is crucial for staff to be refreshed in first aid and other lifesaving techniques, as well as receive the most up to date information. I suggest you set a reminder on your calendar to keep track of expiration dates and/or seek out training from additional approved agencies when staff is not able to attend courses offered locally or at the facility. Having organized staff files helps to ensure that you are meeting safety and training requirements and have necessary documentation on file, including ongoing requirements such as annual training hours. You can use the “File Checklist for Staff” and “File Checklist for Substitutes”, found on the DCDEE website in Provider Documents and Forms to assist you with this. Strategies to ensure requirements are being met include keeping the Staff and Training Worksheet updated as needed. Another strategy could include setting a monthly or quarterly goal to help keep on track and ensure annual requirements are met. Places where you can access training for annual training hours include Moodle at the DCDEE website, the NCRLAP website, the NC Child Care Resource and Referral, and your local Partnership for Children. Provider Portal for ABCMS (the criminal records check database): As stated in G.S. 110-90.2 & .2703(r), child care operators are to notify the Division of any new child care providers working who were hired within five business days. You have completed the provider portal training. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. This facility currently operates with a Five Star license issued August 25, 2022, earning 7 points in Education, 7 points in Program Standards, and 1 Quality Point. Restrictions on the permit include: a first shift capacity of 65 children 3-6 years of age, meets enhanced space, and meets enhanced ratios minus one from the seven-point level. Restrictions were in compliance today. The facility was also monitored for implementing an approved curriculum with children four years of age as required by the star rating; this program uses Creative Curriculum, Guided Edition. The last annual compliance visit was conducted January 26, 2023. A Superior sanitation rating was earned June 6, 2023, and a fire inspection was conducted January 27, 2023; the facility was approved for day time care only. Prior to the visit, the facility had an eighteen-month compliance history score of 93%. Administrator, K. Beck, and Lead Teachers, C. Frink, K. Horton and B. Bass were present and available for consultation today. Three indoor spaces and one outdoor space are currently approved for use by children. Upon arrival, children enrolled in Space #19 were observed in free choice activities including stringing beads, building towers with blocks and pretending to serve a meal in the dramatic play kitchen. One staff member assisted individual children with using paint to make ladybugs. In Space #20, children were observed playing in the sensory table, using lacing beads and pretending to be at the doctor’s office. Children in Space #21 were observed doing indoor gross motor activities including playing hopscotch and hot potato and doing yoga poses. Lunch served in the cafeteria consisted of a choice of tortilla chips with beef and cheese or peanut butter and jelly sandwich, pinto beans, corn, fresh apples and milk. After lunch, children enrolled in Space #20 were observed on the fenced in outdoor playground, riding tricycles and using the climbing equipment. Children in Space #19 were also observed eating a snack of Goldfish crackers and milk. Health and Safety training requirements as outlined in Child Care Rule 10A NCAC 09 .1102 were monitored today. NC Pre-K Site Monitoring All three spaces are operated as NC Pre-K classrooms. The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 7:30am to 3:00pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Developmental screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed in reports filed in children’s files. The NC Pre-K Site Monitoring Tool for 2023-2024 was signed by Terri Ard, representative of contracting agency Columbus County Schools, during a site visit November 13, 2023. It was noted that all requirements were in compliance. NC Pre-K staff information was verified in the NC Pre-K Plan and all staff meet education requirements. The Early Childhood Environment Rating Scale-Revised (ECERS-R) score received on March 10, 2022 was 6.05. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The following violations were observed today and must be corrected immediately: Violation Number Comment Rule 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The facility was also monitored for implementing an approved curriculum with children four years of age as required by the star rating; this program uses Creative Curriculum, Guided Edition. The last annual compliance visit was conducted January 26, 2023. A Superior sanitation rating was earned June 6, 2023, and a fire inspection was conducted January 27, 2023; the facility was approved for day time care only. Prior to the visit, the facility had an eighteen-month compliance history score of 93%. Administrator, K. Beck, and Lead Teachers, C. Frink, K. Horton and B. In Space #20, children were observed playing in the sensory table, using lacing beads and pretending to be at the doctor’s office. Children in Space #21 were observed doing indoor gross motor activities including playing hopscotch and hot potato and doing yoga poses. Lunch served in the cafeteria consisted of a choice of tortilla chips with beef and cheese or peanut butter and jelly sandwich, pinto beans, corn, fresh apples and milk. After lunch, children enrolled in Space #20 were observed on the fenced in outdoor playground, riding tricycles and using the climbing equipment. Children in Space #19 were also observed eating a snack of Goldfish crackers and milk. Health and Safety training requirements as outlined in Child Care Rule 10A NCAC 09 .1102 were monitored today. The definition of adequate supervision includes that staff should be moving around the indoor and outdoor space with children. The field of vision for supervision during outdoor play should not exceed 180 degrees. Closely supervise near high use or potentially dangerous areas such as the climbing structure. Ensure one piece of playground equipment does not block the view of others. If there is more than one teacher on the playground, swap places at least once during the play period. Due to the fact that the youngest child enrolled in this group is four years of age, this ratio may be utilized while still maintaining compliance with the restriction of reduced staff/child ratios and group sizes required at the seven-point level in Program Standards. Please be aware, however, when the youngest child in the group is three years of age, a ratio of one staff member to nine children with a maximum group size of eighteen children must be followed due to this restriction. This facility currently operates with a Five Star license issued August 25, 2022, earning 7 points in Education, 7 points in Program Standards, and 1 Quality Point. Restrictions on the permit include: a first shift capacity of 65 children 3-6 years of age, meets enhanced space, and meets enhanced ratios minus one from the seven-point level. Restrictions were in compliance today. The facility was also monitored for implementing an approved curriculum with children four years of age as required by the star rating; this program uses Creative Curriculum, Guided Edition. The last annual compliance visit was conducted January 26, 2023. A Superior sanitation rating was earned June 6, 2023, and a fire inspection was conducted January 27, 2023; the facility was approved for day time care only. Prior to the visit, the facility had an eighteen-month compliance history score of 93%. Administrator, K. Beck, and Lead Teachers, C. Frink, K. Horton and B. Bass were present and available for consultation today. Three indoor spaces and one outdoor space are currently approved for use by children. Upon arrival, children enrolled in Space #19 were observed in free choice activities including stringing beads, building towers with blocks and pretending to serve a meal in the dramatic play kitchen. One staff member assisted individual children with using paint to make ladybugs. In Space #20, children were observed playing in the sensory table, using lacing beads and pretending to be at the doctor’s office. Children in Space #21 were observed doing indoor gross motor activities including playing hopscotch and hot potato and doing yoga poses. Lunch served in the cafeteria consisted of a choice of tortilla chips with beef and cheese or peanut butter and jelly sandwich, pinto beans, corn, fresh apples and milk. After lunch, children enrolled in Space #20 were observed on the fenced in outdoor playground, riding tricycles and using the climbing equipment. Children in Space #19 were also observed eating a snack of Goldfish crackers and milk. Health and Safety training requirements as outlined in Child Care Rule 10A NCAC 09 .1102 were monitored today. NC Pre-K Site Monitoring All three spaces are operated as NC Pre-K classrooms. The NC Pre-K requirements in Section .3000 of the Child Care Rules were monitored for compliance today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. Children in this program attend from 7:30am to 3:00pm, meeting the instructional day requirement of 6.5 hours. This NC Pre-K program uses Creative Curriculum, Brigance Developmental screening, and Teaching Strategies Gold for their ongoing assessments. Developmental screenings and health assessments including vision, hearing, and dental screenings were on file for all NC Pre-K children’s files reviewed. Evidence of on-going assessments was observed in reports filed in children’s files. The NC Pre-K Site Monitoring Tool for 2023-2024 was signed by Terri Ard, representative of contracting agency Columbus County Schools, during a site visit November 13, 2023. It was noted that all requirements were in compliance. NC Pre-K staff information was verified in the NC Pre-K Plan and all staff meet education requirements. The Early Childhood Environment Rating Scale-Revised (ECERS-R) score received on March 10, 2022 was 6.05. Child Care programs are expected to always achieve and maintain compliance with child care rules, and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent.
