You, Misty Coffey, Lead Teacher, assisted me with today’s visit. Your last annual compliance visit was conducted on November 6, 2024. Prior to today’s visit your compliance history score was 99%. Today, I reviewed with you the facility information found in our system. You stated there have been no changes to your phone number, email address, mailing address, or corporate contact for the facility. I reviewed the facility’s permit with you today. Required postings were posted at the back of the classroom. Your program currently operates with a five-star license, issued on February 9, 2018, earning seven (7) points in staff education, seven (7) points in program standards and one (1) quality point for 75% of lead teachers have a BA/BS or higher in ECE/CD. Your facility operates with the following permit restrictions: daytime care only, meets enhanced ratios, meets enhanced space and reduced staff/child ratios by one per group. Your most recent fire inspection was completed on September 9, 2025. Your most recent sanitation inspection was completed on May 13, 2025, with a superior classification. Your written operational, administrative, personnel policies and your parent participation plan for the 2025 – 2026 school year were sent to me prior to the beginning of the school year for review. You stated that your policies were last updated August 2025 and there had been no changes to your written policies and procedures. A walkthrough of your indoor and outdoor spaces was conducted, and students were participating in teacher-directed whole group activities, free choice play, outdoor play, transitions, toileting/handwashing routines, eating lunch, and rest time. Teacher interactions with children were positive and nurturing. Daily sign in/out sheets and attendance was current/accurate. Incident reports were logged and stored in the child’s file. The Catawba Elementary School – School Risk Management plan was current, printed and has been reviewed with staff annually. A current menu was posted, documented appropriate nutritious snacks and meals, showed a variety of foods, and complied with Meal Patterns for Children in Child Care Programs. Hazardous product storage, general safety, and discipline practices were monitored. There were no medications, no medication permission to administer forms and no Medical Action Plans to monitor. A portion of the children’s files and staff record files were monitored. Your facility does not provide transportation. You stated that you do provide screen time and it was logged on the lesson plan as required. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff includes the lead teacher, Misty Coffey, and the teacher, Christie Warlick. A selection of files was monitored for having completed health assessments and developmental screenings. You use the Ages and Stages developmental screening tool and the Teaching Strategies Gold as the formative assessment to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The Creative Curriculum is the approved curriculum that your program uses. Parent/teacher communication is through the mobile app “Parent Square”, face to face and the daily planner. Parents encouraged to participate in classroom activities as well as other parent engagement. The NC Pre K program operates Monday – Friday during the hours of 8:00 am to 2:45 pm and follow the Catawba County Schools calendar and the NC Pre-K Calendar for the 2025 – 2026 school year. The “Annual Compliance Monitoring Checklist for Child Care Centers” was completed during today’s visit. Health and safety requirements were monitored. Rated License Information: On November 7, 2025, JR Sigmon, Administrator, sent me the following items that were reviewed in my office: * Application for Assessment for a Rated License for Centers * Facility Continuous Quality Improvement (CQI) Plan * Family and Community Engagement Standards Child Care Centers/Centers Located in a Residence Form * Staff Information and Education Worksheet During today’s visit I monitored each staff member’s Individual Continuous Quality Improvement (CQI) and Professional Development Plan (PD) on file. As of today’s date, there are no additional items needed. The Environmental Rating Scale Assessment will be requested by December 19, 2025. The following violations were observed and cited during this visit: Violation Number Comment Rule 620 All walls and ceilings including doors and windows were not kept clean, free of visible fungal growth, and in good repair. There are water marks on one classroom ceiling tile. 15A NCAC 18A .2825(a) 1792 Staff did not model appropriate eating behaviors by consuming food or beverages that meet the nutritional requirements specified in the Meal Patterns for Children in Child Care Programs in the presence of children in care. There was a can of Coca-Cola and a can of Sun-Drop on the counter. .0901(i) TECHNICAL ASSISTANCE: Child care programs are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score. • There are water marks on one classroom ceiling tile. As discussed, all classroom furnishing should be kept clean and in good repair. I suggest that the classroom be monitored daily to ensure that the classroom materials/furnishings are always clean and in good repair and a maintenance work order be completed when it is noticed that something requires fixed or replaced. Also, I suggest that a work order be completed to have the ceiling tile replaced. • There was a can of Coca-Cola and a can of Sun-Drop on the counter. As discussed, staff drinks should be in a container that models proper eating habits. I suggest that food/drinks that do not model proper eating habits be placed in an unmarked covered container prior to children arriving. This violation was corrected during the visit by the staff member placing the drinks in the locked storage closet. CONSULTATION: QRIS Pathways to the Stars Information: Prior to today’s visit on October 22, 2025, I met with your program administrator to discuss the QRIS pathway for this facility. I have attached a QRIS Documentation form to this visit that outlines what was discussed during that meeting. These forms should be updated annually with staff files, children’s records and program record documents to ensure that all information stored in your facilities files are current and accurate prior to DCDEE monitoring visits. All violations cited and not corrected today must be corrected immediately. The name of your center 2. The center’s ID number 3. The date you write the letter 4. Address each violation stating how you corrected the violation and are now in compliance. 5. During today’s visit I monitored a sampling of staff files and a random selection of children’s records. I observed allergies listing to be posted in the classroom. I monitored a total of 1 emergency medication and Medical Action Plan during today’s visit. I observed the menu to be current and posted. Today’s breakfast menu was whole grain blueberry muffins, a cereal bowl and 1% milk. I observed breakfast being served as listed on the menu. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today, this program uses the DIAL 4 screening tool, Teaching Strategies Gold instrument to document evidence of children's ongoing progress and checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The classroom operates from 8:15 am to 3:00 pm. Parent and teacher conferences are held three times per year. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified and in compliance. I observed your indoor and outdoor learning environment to be free of hazards. You stated your program does use screen time that is documented on your lesson plan. The following violations were cited during today’s visit. Violation Number Comment Rule 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. The discipline policy should contain the child’s name, date of enrollment, the date the policy was given and explained and a parent’s signature as acknowledgement of the policy being explained and given to the parent/guardian. .1804(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Staff health and medical information should be stored in a separate file/location and not with the staff member’s personnel file. During today’s visit substitute teachers’ health questionnaire forms and emergency information forms were stored in the binder that contains all substitute staff records and were not stored in a separate location. I recommend that all staff members’ health and medical information be stored in a separate location in individual files for each staff member. .0701(d) Technical Assistance: • As discussed, staff health and medical information should be stored in a separate file/location and not with the staff member’s personnel file.
