You, Dawn Bradley, Lead Teacher, assisted me with today’s visit. Your last annual compliance visit was conducted on January 9, 2024. Prior to today’s visit your compliance history score was 96%. I reviewed with you today the facility information found in our system. You stated there have been no changes to your phone number, email address, mailing address, or corporate contact for the facility. I reviewed the facility’s permit with you today. Required postings were posted as required at the entrance to the classroom. Your program currently operates with a five-star license, issued on May 11, 2023, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point for all lead teachers have an AAS or higher in ECE/CD. Your facility operates with the following permit restrictions: daytime care only, meets enhanced ratios, and meets enhanced space. Your most recent fire inspection was completed on September 24, 2025. Your most recent sanitation inspection was completed on May 16, 2025, with a superior classification. Your most recent fire drill was conducted on October 30, 2025, and your most recent shelter-in-place emergency drill was conducted on October 16, 2025, and has been completed quarterly. Your written operational, administrative, personnel policies and your parent participation plan for the 2025 – 2026 school year were given to me at the beginning of the school year to review. You stated that your policies were last updated at the beginning of the school year and there had been no changes to your written policies and procedures. A walkthrough of your indoor and outdoor spaces was conducted, and students were participating in teacher-directed whole group activities, free choice play, outdoor play, transitions, toileting/handwashing routines, eating lunch, and rest time. Staff/child interactions with children were positive and nurturing. Children were observed to be engaged in free choice play, transitions, toileting routines, outdoors play and eating lunch. Developmentally appropriate materials were provided that were of sufficient quantities and in good repair and there were no potentially hazardous items accessible to the children. I observed the mulch to be sufficient depth for stationary structures. I observed the fence to of adequate height and in good repair. Daily sign in/out sheets and attendance was current/accurate. Incident reports were logged and stored in the child’s file. The School Risk Management (SRM) plan was current, printed and reviewed with staff annually. A current menu was posted, documented appropriate nutritious snacks and meals, showed a variety of foods, and complied with Meal Patterns for Children in Child Care Programs. Space capacity, adequate/approved space, supervision, staff/child ratios, group size, and appropriate discipline were observed today. There were two (2) medications, two (2) medication permission to administer forms and two (2) Medical Action Plans to monitor. A portion of the children’s files and staff record files were monitored. Your facility does not provide transportation and does screen time that is logged on the lesson plan and individual screentime logs. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. The current NC PreK staff includes the lead teacher, Dawn Bradley, and the teacher, Britany Morrison, teacher assistant. A selection of files was monitored for having completed health assessments and developmental screenings. You stated you were in the process of completing the Ages and Stages (ASQ) as the developmental screening tool. The center uses the Teaching Strategies Gold as the formative assessment to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. The NC Pre K program operates Monday – Friday during the hours of 7:30 am to 2:45 pm and follow the Catawba County Schools calendar for the 2025 – 2026 school year. The “Annual Compliance Monitoring Checklist for Child Care Centers” was completed during today’s visit. Health and safety requirements were monitored. • Rated License Information: o On November 7, 2025, JR Sigmon, Administrator, sent me the following items that were reviewed in my office: o Application for Assessment for a Rated License for Centers o Facility Continuous Quality Improvement (CQI) Plan o Family and Community Engagement Standards Child Care Centers/Centers Located in a Residence Form o Staff Information and Education Worksheet During today’s visit I monitored each staff member’s Individual Continuous Quality Improvement (CQI) and Professional Development Plan (PD) on file. As of today’s date, there are no additional items needed. The Environmental Rating Scale Assessment will be requested by December 19, 2025. The following violation was observed and cited during this visit: Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. There were four (4) tricycles on the playground that had visible rust on them. G.S. 110-91(6); .0601(b) TECHNICAL ASSISTANCE: Child care programs are required to maintain compliance with all applicable child care rules and regulations at all times. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past eighteen (18) months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Any violations cited during a visit may affect your score. • There were four (4) tricycles on the playground that had visible rust on them. As discussed, all materials and equipment must be kept clean and in good repair free of rust and debris. I suggest that when completing the monthly playground inspections all outside toys and materials be checked to ensure that they are all in good repair and removed from the playground until they are fixed or replaced. This violation was corrected during the visit by the teacher removing the tricycles from the playground. CONSULTATION: • QRIS Information: Prior to today’s visit on October 22, 2025, I met with your program administrator to discuss the QRIS pathway for this facility. I have attached a QRIS Documentation form to this visit that outlines what was discussed during that meeting. Your administrator states the plan is for this facility to choose Pathway 2 and plans to begin the application process by December of 2026. During today’s visit, we discussed the three QRIS Pathways to the Stars effective July 1, 2025. We covered staff education (including ensuring all staff have WORKS accounts, understanding the 50% requirement for lead teachers and for other educators, and how work experience may count), enhanced ratios and space expectations, the Family and Community Engagement Foundational Practices, and Continuous Quality Improvement (CQI) Plans. You, Melanie Bradley, assisted me with today’s visit. I observed the required posted in the foyer area including the following: five-star rated license, NC Summary of Child Care Law dated February 2025, sanitation placard, Emergency Medical Care Plan, Emergency Phone Numbers, First Aid poster, tobacco free signage, daily schedule, current activity plan, and safe arrival and departure procedures. Your program currently operates with a five-star license, issued on May 11, 2023, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point for having all lead teachers have an AAS or higher in EC/CD, for a total of 14 total points earned. Your facility operates with the following permit restrictions: daytime care only, meets enhanced ratios and meets enhanced space. Your center's compliance history was 97% prior to today’s visit. Your last annual compliance visit was conducted on January 9, 2025. I monitored the indoor and outdoor areas. I observed children throughout the classrooms participating in free play in activity areas, transitions, and personal care routines. I monitored for general safety, storage of hazardous products, appropriate discipline, storage of medication and medication administration. Program records including emergency medical care plan, safe arrival and departure procedures, incident log, outdoor safety inspections, fire drills, emergency drills and your Emergency Preparedness Response (EPR) Plan were monitored. Today, I reviewed staff record files for three (3) returning staff members to verify current and valid Criminal Background Check qualification letters, valid CPR and First Aid training, completion of the Recognizing and Responding to Suspicions of Child Maltreatment and special trainings. There were no new staff members to monitor today. Your program Administrator sent me your current handbook by email and it included: written operational, administrative, and personnel policies and your parent participation plan. You stated your written policies and procedures had not changed. Children were adequately supervised, minimum staff-child ratios were maintained, adequate approved space was used, enhanced space capacities were maintained, and permit restrictions including daytime care only, meets enhanced ratios and meets enhanced space, were maintained during today’s visit. Today’s lunch listed on the menu was hamburger, crinkle French fries, cole slaw and 1% unflavored white milk. I monitored two (2) medications, two (2) medication permission to administer forms and two (2) Medical Action Plans. I monitored screen time logs, the incident report log and the 2024-2025 Bandy’s High School -School Risk Management plan. You stated that your program does not provide transportation. Your most recent fire drill was conducted on April 29, 2025, and has been completed monthly for the past twelve (12) months and your most recent shelter in place emergency drill was conducted on March 8, 2025, and has been completed quarterly. Your most recent fire inspection was completed on September 5, 2024, and was sent to me by email. Your most recent sanitation inspection was completed on December 6, 2024, with a superior classification. During today’s visit I monitored for all health and safety requirements. The following violation was cited during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. One (1) child was not signed in on the daily sign in/out sheet. 10A NCAC 09 .0302(d)(4) TECHNICAL ASSISTANCE: • One (1) child was not signed in on the daily sign in/out sheet. As discussed, all children must be signed in at arrival and departure. I suggest that once all children are present and as children depart a staff person checks the daily sign-in sheet to ensure that all children have been signed in/out by a parent or staff member. This violation was corrected during the visit by the teacher signing the child in. I observed the required posted in the foyer area including the following: five-star rated license, NC Summary of Child Care Law dated February 2025, sanitation placard, Emergency Medical Care Plan, Emergency Phone Numbers, First Aid poster, tobacco free signage, daily schedule, current activity plan, and safe arrival and departure procedures. Your program currently operates with a five-star license, issued on May 11, 2023, earning seven (7) points in staff education, six (6) points in program standards and one (1) quality point for having all lead teachers have an AAS or higher in EC/CD, for a total of 14 total points earned. Your facility operates with the following permit restrictions: daytime care only, meets enhanced ratios and meets enhanced space. Your center's compliance history was 97% prior to today’s visit. Your last annual compliance visit was conducted on January 9, 2025. I monitored the indoor and outdoor areas. I observed children throughout the classrooms participating in free play in activity areas, transitions, and personal care routines. I monitored for general safety, storage of hazardous products, appropriate discipline, storage of medication and medication administration. Program records including emergency medical care plan, safe arrival and departure procedures, incident log, outdoor safety inspections, fire drills, emergency drills and your Emergency Preparedness Response (EPR) Plan were monitored. Today, I reviewed staff record files for three (3) returning staff members to verify current and valid Criminal Background Check qualification letters, valid CPR and First Aid training, completion of the Recognizing and Responding to Suspicions of Child Maltreatment and special trainings. There were no new staff members to monitor today. Your program Administrator sent me your current handbook by email and it included: written operational, administrative, and personnel policies and your parent participation plan. You stated your written policies and procedures had not changed. Children were adequately supervised, minimum staff-child ratios were maintained, adequate approved space was used, enhanced space capacities were maintained, and permit restrictions including daytime care only, meets enhanced ratios and meets enhanced space, were maintained during today’s visit. Today’s lunch listed on the menu was hamburger, crinkle French fries, cole slaw and 1% unflavored white milk. I monitored two (2) medications, two (2) medication permission to administer forms and two (2) Medical Action Plans. I monitored screen time logs, the incident report log and the 2024-2025 Bandy’s High School -School Risk Management plan. You stated that your program does not provide transportation. Your most recent fire drill was conducted on April 29, 2025, and has been completed monthly for the past twelve (12) months and your most recent shelter in place emergency drill was conducted on March 8, 2025, and has been completed quarterly. Your most recent fire inspection was completed on September 5, 2024, and was sent to me by email. Your most recent sanitation inspection was completed on December 6, 2024, with a superior classification. During today’s visit I monitored for all health and safety requirements. The following violation was cited during today’s visit. I suggest that once all children are present and as children depart a staff person checks the daily sign-in sheet to ensure that all children have been signed in/out by a parent or staff member. A selection of files was monitored for completed health assessments and developmental screenings. You stated you were in the process of completing the ASQ and the Dial 4 formative assessments. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. You stated that there was 1 medication, 1 medication permission to administer form and no Medical Action plans to monitor. Your facility does not provide transportation. A random sampling of 3 children’s records were monitored. I monitored 1 existing staff file was monitored. All health and safety requirements were monitored. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used during today’s visit. The following violation was cited. As discussed, when completing documents all information must be identified accurately on the form. Medication permission to administer forms providing standing orders must list the specific dates of the standing orders. I suggest that when receiving documents that have been completed by a parent/guardian that you review the forms for accuracy prior to filing placement. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. (DCDEE does not expect it to be ready before at least the fall of 2025.) This means that NO licensed child care providers have to undergo a Rated-License assessment after June 30, not even those in “Cohort 1.” Effectively, there is no longer a plan to resume assessments until after we have a new QRIS system, which the NC Child Care Commission is currently working to implement through rulemaking. * Senate Bill 425 still has to be signed by the governor to take effect, but Governor Cooper is expected to sign it before the June 30 deadline that would have eliminated the “hold-harmless” period and pause on assessments. This is a huge relief for licensed child care providers statewide and a testament to the power of advocacy when work together! The checkpoint assessments are conducted three times per year: at the beginning, middle and end of the school year. You stated that there was 1 medication, 1 medication permission to administer form and no Medical Action plans to monitor. Your facility does not provide transportation. A random sampling of 3 children’s records were monitored. I monitored 1 existing staff file was monitored. All health and safety requirements were monitored. (DCDEE does not expect it to be ready before at least the fall of 2025.) You, Dawn Bradley, Lead Teacher, assisted me, Meria Wilder, Child Care Consultant, with today’s visit. The last annual compliance visit was conducted on February 1, 2023. Your compliance history score was 100% prior to today’s visit. Your facility operates with a Five-Star license that was issued on March 22, 2019. Your program earned 6 points in program standards, 7 points in staff education and 1 quality point for a total of 14 points earned. Your program operates with the following restrictions: 1st shift (daytime care), Meets enhanced space and meets enhanced ratios. The following required postings was posted in outside of the classroom: Five-Star License, NC Summary of Child Care Law, Sanitation placard, Emergency Medical Care Plan, Emergency Phone Numbers, safe arrival and departure procedures, First Aid poster, menu, daily schedule, and activity plan. I observed the following inspections, sanitation inspection conducted on 12/1/23, fire inspection conducted on 12/14/23, playground inspection conducted on 1/4/23, and the last quarterly shelter-in-place emergency drill conducted on 9/19/23. I observed the menu to be current and chicken wrap, siracha chicken wings, peanut butter and strawberry jelly sandwiches, sweet potato fries, steamed broccoli and 1% unflavored white milk was listed for lunch. NC PreK requirements were monitored and observed to be in compliance during today’s visit. I monitored your incident log and found incident reports to be logged with the child’s file. I monitored your screen time log. I monitored your daily sign in/out records and daily attendance and found it to be current. I monitored your indoor and outdoor environment for safety hazards. You stated you do not have any medications. I observed adequate and approved space, supervision, group size and staff child ratios. I observed children being adequately supervised and cared for in a nurturing way. I observed children working independently during free choice play time and transitioning to outside time. I observed toys and materials to be child size and developmentally appropriate. I observed furnishings to be clean and in good repair. I monitored 1 existing staff file and there were no new staff files to monitor. I monitored a random sampling of 2 children’s records and 1 of them being for a child enrolled through NC PreK. The following violations were cited during today’s visit. Violation Number Comment Rule 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child with an enrollment date of 8/28/23 did not have a medical exam or health assessment on file within 30 days of enrollment. GS110-91(1) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A quarterly emergency drill was not completed during the month of December. .0604(u);.0302(d)(8) Technical Assistance: • One child with an enrollment date of 8/28/23 did not have a medical exam on file within 30 days of enrollment. As discussed, I recommend when receiving documents that you review the document prior to adding it to the child’s record to ensure all required information is completed and there are no blank places left on the form. • During the month of December 2023 an emergency drill was due to be completed and has not been completed. As discussed, I recommend an emergency drill be completed as soon as possible and quarterly thereafter. You may set a reminder to ensure that you remember to complete this drill to maintain compliance. All violations cited must be corrected immediately and a compliance letter for violations cited during today’s visit must be sent to me by February 2, 2024. Comments: Child care facilities must comply with all state laws, federal laws and local ordinances that pertain to child health, safety, and welfare of children (GS110-91 – Mandatory standards for a license). Failure to comply may lead to an administrative action (General statue 110-102-2- Administrative penalties). Any observed violations cited during visits will negatively affect the center’s Compliance History Score. Your program must maintain a compliance score of at least 75% as required by G.S. 110-90 (4)(c) in order to be eligible for a star rated license. You must maintain at least 75% Compliance History for each 18-month period as required by GS110-90(4)(c). Prior to today’s visit, your program maintained a compliance history of 97%. Please note any violations cited during future visits may negatively impact your compliance history. Repeated violations or violations left unresolved may lead to an administrative action.
