Name of Operation: CASAR ELEMENTARY SCHOOL Facility ID: 23000416 Consultant: TIFFANI SIMS Operation Type: Center Case Number: Visit Date: 4/15/2026 Number Present: 15 Completed Date: 4/15/2026 Age: From 4 To 5 Total Minutes: 215 Time In: 09:15 AM Time Out: 12:50 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for an annual compliance visit, including compliance with requirements located in Child Care Rule Section .3000 in space #1, where children participating in the NC Pre-K program are cared for. You, Lindsay Chaney, Lead Teacher, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 14, 2024, earning seven (7) points in staff education, even (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space and enhanced ratios reduced by one. Your program was also monitored for compliance with implementing an approved curriculum, Creative Curriculum Teaching Strategies Gold, as required for all four- and five-star licensed facilities where four-year-old children are enrolled. The last annual compliance visit was conducted on May 5, 2025. The sanitation inspection was completed March 23, 2026, with a “Superior” classification and zero (0) demerits. The last fire inspection was completed on August 21, 2025, and your facility was approved for daytime care only. The last shelter in place emergency drill was completed on March 6, 2026, at 9:00am and recorded. The last playground inspection was completed on March 26, 2026, by Lindsey Chaney. The last fire drill was completed on March 2, 2026, at 9:00am and recorded. Each indoor and outdoor space was monitored. Children enrolled in space #1 were observed during indoor free play, outdoor gross motor play, group time with songs and finger plays, teacher directed activities, and lunch. Proper hand washing techniques procedures were observed in both classrooms. Lunch was observed during today’s visit and included meatball subs/pepperoni and cheese pizza crunchers/chef salad with ranch dressing and crackers, sweet potato wedges, steamed broccoli, pears/apples, and milk. The NC Pre-K requirements in section .3000 of the child care rules were monitored for compliance. The NC Pre-K Program Site Monitoring Tool was reviewed today. Staff-child ratios and maximum group sizes required in Child Care Rule 10A NCAC 09 .3009 were verified in compliance. You stated that you had completed the Brigance screenings for the children in your classrooms by September 15, 2025, if not completed during preschool screening with the Cleveland County School’s Office of School Readiness. The center uses the Teaching Strategies Gold instrument to document evidence of children's ongoing progress. Checkpoint assessments are conducted three times per year: at the beginning in October 2025, middle in February 2026, and end of the school year in late April or May 2026. The first day of school was August 25, 2025. You also stated that additional resources are used in the classroom, such as Second Steps, Heggerty, and Finch. Parent conferences are held two times per year. You also stated that you conduct two home visits are completed each year. You stated that you use the Class Dojo (app) to communicate with parents, as well as in person and through telephone calls. Documentation was reviewed of parent newsletters, monthly calendar with notes, parent contacts in Class Dojo, and observational notes of children. Files Reviewed: The current NC Pre-K staff are lead teachers and teachers. A selection of files was monitored for completed health assessments and developmental screenings. Three (3) children’s files were reviewed during today’s visit. Three (3) staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during today’s visit. The classroom operating hours are from 7:35am to 2:45pm. The environment rating scale score received February 29, 2024, was 6.19. A checklist was used to note the requirements I monitored today. The following violation was cited during today’s visit. Violation Number Comment Rule 853 Incident logs were not completed and maintained as required. An incident log was not completed and was not available for review during today’s visit for an incident report dated March 25, 2026, for a child enrolled on August 25, 2026. .0802(g)(1-6) Technical Assistance regarding the violations observed during today’s visit: I suggested that the incident log record to be placed either on the side of the filing cabinet and/or on the wall next to the filing cabinet where children's files are kept for easy access. I also suggested that once an incident report is completed that you transfer that information to the incident log immediately. This violation was corrected during the visit by the staff adding the incident report information dated March 25, 2026, to the incident log. I discussed with you that no compliance letter was needed. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-eight percent (98%). Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Consultation: -Provider Access to ABCMS I reviewed the ABCMS Portal Roster for your child care program on April 14, 2026, and you have linked staff to the facility’s ABCMS Portal page. -Child Care Rule Effective January 1, 2024 Lead Testing: Child Care Rule .0601(f)- Safe Environments We discussed the test results that were available for review on the Clean Classrooms for Carolina Kids website for lead testing water testing, lead-based paint testing, and asbestos testing for your facility. The lead water testing results were completed on April 17, 2024. On April 14, 2026, I checked the Clean Classrooms for Carolina Kids website, and it stated that your facility had no results available for review for lead-based paint and/or asbestos testing. -QRIS Moderation Your facility had the Environment Rating Scales conducted on February 29, 2024, and your new license was issued May 14, 2024. You must apply for the new QRIS Pathways to the Stars prior to January 1, 2027. Additional training and resources can be accessed from the NC Rated License Assessment Project for the Environment Rating Scales Third Edition for ECERS-3. -Updating DCDEE WORKS Account: We discussed updating and/or creating an account on the DCDEE WORKS portal for staff’s education to be evaluated for the upcoming Pathways to the Stars process. An updated DCDEE WORKS account is required for all staff to process the education standard for your program. Education will be evaluated for administrators and staff who provide direct care to children. Safe Environments/Supervision During Naptime: I discussed with you the child care requirements about providing a safe environment and supervision during naptime. I observed in Space #1 and the naptime environment and that a safe environment and supervision met current child care requirements. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. You, Lindsay Chaney, Lead Teacher, assisted me with the visit today. The facility currently operates with a 5-star rated license issued on May 14, 2024, earning seven (7) points in staff education, even (7) points in the program standards, and one (1) quality point. The program's license restrictions include the following: daytime care, meets enhanced space and enhanced ratios minus one. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent (99%) as of December18, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today; all indoor and outdoor areas were monitored. I observed children in both the indoor and outdoor learning environments and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in indoor free play. The caregivers were interacting and meeting the developmental needs for each of the children. Inspections/Drills: The most recent sanitation inspection for your facility was conducted on November 5, 2025. A superior sanitation classification was issued with zero (0) demerits noted on the grade card. The last fire inspection was completed on August 21, 2025. The most recent fire drill was conducted on December 10, 2025, at 1:40pm and recorded. The most recent lockdown emergency drill was conducted on September 29, 2025, at 8:30am and recorded. The most recent playground safety inspection was conducted on November 24, 2025 by Lindsay Chaney. You stated one (1) new staff have been hired since your most recent annual compliance visit, which was conducted on May 5, 2025. The staff file was reviewed during and the staff and training worksheet was updated. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. The following violations of child care requirements were observed and documented during today’s visit. Violation Number Comment Rule 125 Daily records of arrival and departure times for children at the center were not maintained as children arrive and depart and/or were not made available for review. In Space 1, on December 2, 2025, one child five years of age enrolled on August 25, 2025, arrival time was not documented. 10A NCAC 09 .0302(d)(4) Technical Assistance regarding the violations observed during today’s visit: -Documenting Arrival and Departure Times: This was a repeated violation. All children must be signed in and out each day for before and after school care as well as full day care. If the parent fails to sign the child in or out, it is the staff member’s responsibility to document the child’s arrival and departure times. I suggested that you designate one staff member to monitor the arrival and departure records at designated times each day to ensure all children have been signed in and signed out. -Storage of Non-Prescription Items: Medications including prescription and non-prescription items shall be stored in a locked cabinet or other locked container and shall not be stored above food. Designated emergency medications shall be kept inaccessible to children but are not required to be in locked storage. Non-prescription diaper creams and sunscreen shall be kept inaccessible to children when not in use, but are not required to be in locked storage. “Inaccessible to children” is defined as a vertical distance of at least five feet from the finished floor. I suggested that you and staff conduct safety checks of each classroom to ensure all medications are stored appropriately. The violation was corrected during the visit. Compliance History: Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter Documentation: The violations documented above must be corrected immediately. Send me a letter of compliance describing in detail specifically how each violation was corrected and how compliance will be maintained in the future. Include the violation item number, statement of compliance, date of the visit and license ID number in the letter. I must receive your compliance letter by January 1, 2026. A second annual compliance monitoring visit was conducted this year due to the date of when your next annual compliance monitoring visit should have been scheduled in late August 2025. The facility currently operates with a 5-star rated license issued on April 6, 2020, earning seven (7) points in staff education, even (7) points in the program standards, and one (1) quality point. A checklist was used to note the requirements I monitored today. Files Reviewed: Children’s files, program files, and staff files were monitored today. Two (2) children's files were reviewed during today’s visit. Two (2) staff files were reviewed and no new staff files were reviewed today. I discussed the Staff and Training Worksheet and the Children’s Record form with you today. Both forms were completed and verified during the visit. The following violation was observed during the visit today. On April 11, 2025, four children’s departure times were not documented. 10A NCAC 09 .0302(d)(4) The following Technical Assistance regarding the violations observed during today’s visit was provided: Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart the child care facility to ensure accurate records are kept. I suggested as a closing activity for the classroom that you and/or the teacher assistant check the daily sign-in and sign-out sheets to ensure that arrival and departure times have be documented. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was one-hundred percent (100%). Please note any violations cited during future visits will negatively impact your compliance history. On September 8, 2023, one child’s arrival time was not documented; on September 11, 2023, one child’s arrival time was not documented; on September 15, 2023, one child’s arrival time and one child’s departure time were not documented; On September 22, one child’s arrival times were not documented. 10A NCAC 09 .0302(d)(4) The following Technical Assistance regarding the violation observed during today’s visit was provided: -Arrival/Departure Times As a reminder, daily sign-in and sign-out times must be recorded when children arrive and depart from the child care facility each day to ensure accurate records are kept. I suggested creating a system to check during the day when arrival and departure times have been recorded. For example, set a specific time during the mid-morning to check children's arrival times have been recorded and set a specific time at the end of the day to check children's departure times are recorded. Compliance History: You must maintain at least 75% Compliance History for each 18-month period as required. Prior to today’s visit your compliance history was ninety-eight (98) %. Please note any violations cited during future visits will negatively impact your compliance history. Repeated violations or violations left unresolved may lead to a recommended administrative action. Compliance Letter: You must correct the violations found during today's visit immediately. Please send me a letter verifying compliance by Monday, October 9, 2023. Please include in that letter each item number and explain in detail how you corrected each violation and what plan will be implemented to prevent these violations from occurring again. The period for background check qualification renewals was revised from three years to five years. 2. The provisional qualification period was reduced to forty-five days.
