Gizem Imece, director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Imece and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. On October 28, 2025, a one-year-old child ingested a medication not prescribed to the child. 10A NCAC 09 .0601(a) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. On October 28, 2025, a staff member in the classroom for one-year-old children had a bottle of diet pills in their bag in an unlocked closet in the classroom with children. 15A NCAC 18A .2820(d) 1810 There was a substantiation of child maltreatment. On January 13, 2026, The Division of Child Development and Early Education substantiated child maltreatment. GS 110-105.6(a) 1883 When medication was administered in error, the child care center did not either call 911 in accordance with CPR or First Aid training recommendations, notify the center director, contact the child's parent, observe the child, or include all the information when documenting the error in writing. Today's visit was conducted with Gizem Imece, Administrator. When I arrived the Assistant, Ms. Yuret, greeted me and accompanied me on the walkthrough. Children were observed participating in routine activities in their rooms. A partial assessment of applicable childcare requirements was conducted. The last annual compliance visit was conducted on 8/30/24. The last fire inspection was conducted 9/12/23. The last sanitation inspection was conducted 4/22/24. Staff and training worksheets and files for new staff hired since the last annual compliance inspection were reviewed. A fire drill was last conducted 1/9/25. The last shelter-in-place drill was conducted 1/22/25. Outdoor inspections were observed to be completed monthly. Medications are stored in the front office in a locked cabinet. 9 Medications were observed during inspection to meet all rule requirements. All required postings were observed. Today I observed 1 violation regarding childcare requirements. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last observable fire inspection was completed 9/12/23. 10A NCAC 09 .0304(a) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by February 26, 2025 stating that all violations have been corrected. The letter can be emailed to scott.moore@dhhs.nc.gov or mailed to: DHHS Division of Child Development 2201 Mail Service Center Raleigh, NC 27699-2201 ATTN: Scott Moore If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. TECHNICAL ASSISTANCE When you reach out to the Fire Marshall to conduct an inspection inquire on if they have you scheduled for annual visits and what their process is (some operations have been told they were on 2 year visits). I.e. if they will automatically come out or if you are required to schedule the inspection. Hold harmless has been extended until the new QRIS is implemented (SB 425). Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. This is optional and up to your discretion at this time. I would advise to still work on preparing and ensuring staff education is current in WORKS. If you've had a lead water test in the past it is likely due (every 3 years). Visit cleanwaterforcarolinakids.org and apply for a water test timely before you receive a sanitation inspection. Effective January 2024 you must complete a lead water test before January 2025. Please visit cleanwaterforuskids.org/carolina/ to register for the pre-enrollment webinar. After completion of the webinar you will be able to request the test. A printout of the information sheet previously emailed to providers was given during inspection. On the same website for lead water tests you need to sign up for asbestos and lead paint testing. You will have to complete a questionnaire. At the conclusion of the questionnaire you may receive a letter of exemption. If you receive a letter please maintain it on file. Applicable childcare requirements and the inspection was discussed with Administrator, Gizem Imece. Currently this facility operates with a 5 star license effective February 11, 2020. This operation is accredited. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities. I observed whole group activities and free play activities. Children were observed playing outside during outdoor play with adequate supervision. Age appropriate materials and equipment was accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored, permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. 9 children's records were reviewed and staff records were reviewed along with the staff and training worksheet. The last fire inspection was completed 9/12/23. The last sanitation inspection was completed 4/22/24. The last fire drill was completed on 7/12/24. The last shelter-in-place drill was completed 8/8/24. The operation's EPR plan was last updated 5/24/24. Outdoor inspections were observed to be completed monthly. Diaper creams and suntan lotions were observed to have current permissions in all rooms. Sleep checks were observed to be completed. 6 emergency medications were observed stored securely in the Administrator's office. One was observed to have an expired permission form that exceeded the 6 month time frame. We discussed updating expiring medications and returning medications to parents. Transportation is not provided. I observed 2 violations during this inspection. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One emergency medication was observed to have an expired authorization form that exceeded 6 months. 10A NCAC 09 .0803(4)(6-9) 1867 The depth of the loose surfacing was not based on critical height of the equipment. No measurable depth of surfacing was observed at the ends of 2 slides on the large climbing structure. .0605(k)(1-4) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by September 3, 2024 stating that all violations have been corrected.
