A full assessment of child care requirements was conducted. Your program currently operates with a Five-star license and is approved to care for children 0-12 years of age. Today’s visit was conducted with you, Ana Maria De La Cotera, Center Director. Ms. De La Cotera accompanied me throughout my walkthrough of the facility today. I monitored four classrooms in use for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. Two playgrounds were also monitored today. The facility was monitored for health and safety requirements both indoors and outdoors. Toddler and preschool children were observed engaged in outdoor play, participating in circle and center time, preparing for lunch and going down for nap. Four infants were observed present today and two staff were observed caring for the infants today. The staff were actively engaged in floor activities with the infants. Staff were observed assisting children with activities and meeting their developmental needs. Program requirements were monitored today, and two (2) violations were cited related to medication documentation. All inspections and drills were current. Children’s and staff records were monitored, and the information was documented on the children’s records form and staff and training worksheets. A checklist was also used to monitor compliance. The facility was licensed on July 3, 2017. The owner/operator is listed as current/active with the NC Secretary of State. Two violations were cited today and must be corrected immediately. Violation Number Comment Rule 844 Prescribed medicine was not in original labeled container or accompanied by signed and dated written instructions from prescribing physician or health care professional. One prescribed diaper cream medication was not accompanied with the required prescription pharmaceutical label. .0803(2)(a) 851 When medication was administered, documentation was not completed or maintained for 6 months and/or the documentation did not include the required information. Parents signature was found to be expired for one A&D diaper cream ointment. .0803(13)(a-e); .2318(3) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 6/18/2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. An example is: Jane Doe, Administrator AAA Child Care ID # 12345678 TECHNICAL ASSISTANCE: *To assist with preventing additional violations with maintaining the required documentation we discussed sending notifications to parents throughout the year regarding your school medication policy and the required procedures that need to be followed to maintain compliance with DCDEE. We also discussed coming up with a policy that involves a timeline for medication forms to be updated and returned PRIOR to the expiration date. *Classroom materials – It is important to have enough materials for all children in the room to be to participate in centers. During today’s visit the classrooms were observed to have enough materials for the children to use the materials were observed in good repair. We discussed the importance of making sure each classroom continues to have enough materials for the children to use at all times throughout the year. *Playground Safety – It is the responsibility of the facility to provide a safe inside and outside environment for the children at all times. As playground inspections are completed and areas of concern are noted, when items are needing immediate attention, I recommend submitting the required work orders right away so a safe environment is maintained. To maintain a safe outdoor environment during summer months, we discussed looking into installing an extra sun sail/shade to provide appropriate shaded coverage on both playgrounds throughout the day. I reminded Ms. De La Carter of the importance of checking the temperature of the playground equipment before allowing any children on it. Ms. De La Cotera showed me the heat guns they purchased and are using to confirm the play structures are safe for the children to play on BEFORE the children use them. I also left with Ms. De La Cotera two handouts, CPSC Fact Sheet – Burn Safety Awareness on Playgrounds and Protecting Children From Thermal Burns from Playground Equipment, to refer to when conducting temperature checks of the playground surfacing and equipment. *Weather Watch Charts were observed throughout the facility. AUTOMATED BACKGROUND CHECK MANAGEMENT SYSTEM *Ms. De La Cotera is currently using the Automated Background Check Management System. Ms. De La Cotera knows how to maneuver through the program and knows she can reach out to Ms. Serena Luke for additional guidance if needed (Serena Luke at serena.luke@dhhs.nc.gov or 919-814-0175 Office). *RATED LICENSE The facility had their Environmental Rating Scales conducted on June 6, 2024, and June 7, 2024. I spoke to the owners of the facility, Dan and Martha Rockaway, on June 1, 2025, and they confirmed at this time they do not plan to use the ERS scores received on June 6 & 7, 2024 and will be pursuing their star rated license by obtaining NAEYC Accreditation in the 2025/2026 fiscal year. *LEAD WATER TESTING -Centers are required to have their water tested every 3 years by the Clean Water for Carolina Kids™ program. Your last water testing was completed on March 1, 2024, and the results came back in normal range. You are due to have your water tested before March 1, 2027. -Lead Paint and Asbestos testing – Sounds and Colors At Carpenter Village, LLC was built after 1978 but has completed the required documentation and has sent it off to Clean Water for Carolina Kids. Sign up for their quarterly newsletter and find updated resources at www.healthychildcare.unc.edu or 800-367-2229, choose option 1, then 2. *The Healthy Social Behaviors (HSB) Project supports teachers to promote healthy social-emotional development and reduce the expulsion rate among young children in licensed child care centers across North Carolina. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children.
