Name of Operation: SAS INSTITUTE MONTESSORI PRESCHOOL Facility ID: 92003709 Consultant: SCOTT MOORE Operation Type: Center Case Number: Visit Date: 10/23/2025 Number Present: 58 Completed Date: 10/23/2025 Age: From 3 To 5 Total Minutes: 65 Time In: 09:10 AM Time Out: 10:15 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable child care requirements. Today's visit was conducted with Ally Dober, Administrator. Children were observed participating in routine activities in their rooms and playing outside. A partial assessment of applicable childcare requirements was conducted. The last annual compliance visit was conducted on 4/3/25. The last fire inspection was conducted 9/11/25. The last sanitation inspection was conducted 5/5/25. Staff and training worksheets and files for new staff hired since the last annual compliance inspection were reviewed. A fire drill was last conducted 10/9/25. The last lockdown drill was conducted 8/28/25. Outdoor inspections were observed to be completed monthly. Medications are stored in each room in a backpack stored above 5 feet in height. 8 emergency medications were evaluated during inspection. All required postings were observed. The operation has completed enrollment in asbestos and lead paint testing. The operation has a current roster in the ABCMS background check system. Today I observed 1 violations regarding childcare requirements listed below and this violation was discussed with you in detail during the visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. One emergency medication evaluated had a permission that expired in August 2025. 10A NCAC 09 .0803(4)(6-9) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by November 6, 2025 stating that all violations have been corrected. The letter can be emailed to scott.moore@dhhs.nc.gov or mailed to: DHHS Division of Child Development 2201 Mail Service Center Raleigh, NC 27699-2201 ATTN: Scott Moore If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by November 6, 2025, you must contact me with a proposed timeline of the corrections. In some cases the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. TECHNICAL ASSISTANCE We discussed the operation's pursuit of Accreditation. Since the operation does not have a higher than 1 star rated license, there is no time frame for completion. Whenever the process is completed I will send you the application forms. After a higher star rated license is issued the license will need to be renewed every 3 years.
