Name of Operation: LUCY DANIELS CENTER FOR EARLY CHILDHOOD Facility ID: 92000206 Consultant: SCOTT MOORE Operation Type: Center Case Number: Visit Date: 10/7/2025 Number Present: 4 Completed Date: 10/7/2025 Age: From 4 To 6 Total Minutes: 50 Time In: 10:50 AM Time Out: 11:40 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s unannounced visit was to monitor compliance with applicable child care requirements. Today's visit was conducted with Maryellen P., Person in Charge. Children were observed participating in routine activities in their room. A partial assessment of applicable childcare requirements was conducted. The last annual compliance visit was conducted on 4/10/25. The last fire inspection was conducted 10/9/24. The last sanitation inspection was conducted 2/12/25. The Administrator was stated to be out today. I was also informed she has been filling in some classrooms due to the operation losing 2 teachers recently. A fire drill was last conducted 8/27/25. The last shelter-in-place drill was conducted 8/27/25. Outdoor inspections were observed to be completed monthly but the records stopped 8/2025. Medications are stored in each room in a cabinet above 5 feet in height. One emergency medication was observed and found to meet all requirements. All required postings were observed. The operation was observed to have a current roster in the ABCMS system. Today I observed 2 violations regarding childcare requirements listed below and these violations were discussed during the visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The last documented fire drill was observed to be completed 8/27/25. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. The last documented playground inspection was recorded as 8/2025. .0605(q) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by October 21, 2025 stating that all violations have been corrected. The letter can be emailed to scott.moore@dhhs.nc.gov or mailed to: DHHS Division of Child Development 2201 Mail Service Center Raleigh, NC 27699-2201 ATTN: Scott Moore If you state in the compliance letter that corrections have been made when they have not, this will be considered falsification of information. If you cannot meet the requirements by October 21, 2025, you must contact me with a proposed timeline of the corrections. In some cases the timeline may be extended. You are required to always maintain compliance with all applicable child care rules and regulations. NC GS 110-90(4)(d) requires all child care facilities to maintain a compliance history score of at least 75% for the past eighteen months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during today's visit may negatively impact your compliance score. TECHNICAL ASSISTANCE Fire drills and outdoor inspections should be completed monthly. I am sorry to hear about staffing issues in the operation. It may be helpful to train the primary caregiver of the room or another staff as a backup to complete these tasks should you be indisposed. While no required, it might be helpful to post the fire drill log and playground inspections in the room just to have another set of eyes on it so the task is completed on time. Your fire inspection expires in a few days. Please reach out to the Fire Marshall to request a new inspection. Please send me a copy of the inspection report once it is completed. On the website for lead water tests (cleanwaterforuskids.org/carolina/) you need to finish sign up for asbestos and lead paint testing. You will have to complete a questionnaire. At the conclusion of the questionnaire you may receive a letter of exemption. If you receive a letter please maintain it on file. If you are subject to testing they will reach out to you at a later date with further direction. The sanitation department is requiring completion of enrollment by January 2025 or it will be a major violation through them during their next inspection. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. If the operator fails to correct any documented violations within the established time period, the Division of Child Development and Early Education may deny, suspend, terminate, or revoke any permit to operate (10A NCAC 09 .2000).
