Today's visit was conducted with the owner/operator of Little China Home Daycare, Mrs. Wenmin Chen. Mrs. Chen’s husband works from home and was present during today’s visit to translate when necessary. Mrs. Chen has a part-time assistant caregiver, Ms. Xiuzhu Hu. She was not present today but all required documentation was on file for her today. Today’s visit was conducted a little early due to Ms. Chen going out of the country until mid- May 2026. The FCCH operates with a 1 STAR license issued on April 16, 2020. This FCCH operates first shift from 8:30am- 5:30pm M-F. You are licensed to care for 5 children ages 1-3 and one school age child on the first shift. Currently you have five (5) children ages 1-3 and one school age child enrolled in your family child care home. Today, I completed a walk-through of the family child care home, five (5) 1yr-3yr. year old children were present today along with one school age child. When I arrived, the children were having lunch and preparing for nap time. The provider, Mrs. Chen, and her husband, Mr. Brian Lin were sitting with the children assisting them with their meal. After lunch Mrs. Chen assisted the children with hand washing and going down for nap. During today’s visit, a full assessment of childcare requirements was conducted. A checklist was used to note the items monitored and provided to the owner/operator at the conclusion of the visit. Due to Ms. Chens upcoming vacation plans, today's visit was conducted early and due to the annual visit being conducted early Ms. Chen still has time to complete her annual training hours for the 25/26 year. Ms. Chen stated she will email me training certificates when her trainings are completed. Ms. Chen completed all of the required Health and Safety training on 8/13/2024 and is required to renew her Health and Safety training by 8/13/2029. Emergency telephone numbers were readily accessible, and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Child Care Space: The child care space is located on the entire first floor of the home, minus the kitchen area which is to remain blocked off from the children at all times. For outside play, the children use the neighborhood park which is walking distance from the family childcare home. Ms. Chen is using the DCDEE playground check list to complete her required checks of the neighborhood playground. Safety/Inspections: Fire drills, Outdoor playground inspections, and EPR drills were current and on file. Transportation: Mrs. Chen is not approved for transportation. Six violations were observed during today’s visit. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for this family child care home was 98%. Any violations cited during today's visit may affect this percentage. To avoid violations in the future, I recommend creating a tickler system that will help you remember when trainings and or documents are going to expire. You could put reminder on your phone or use your calendar to note expiration dates such as CPR/FA. CONSULTATION AND TECHNICAL ASSISTANCE *HOT SURFACES PLAYGROUND- With summer approaching and temperatures rising be mindful that playgrounds are often a hub of joy and activity for children, offering a space where they can run, jump, and play freely. However, as temperatures rise, so do the risks associated with hot surfaces on playgrounds. Be mindful of this and make sure you are checking the temperature of the playground equipment with your hand to confirm it is safe for the children to play on. *To avoid violations in the future, I recommend creating a tickler system that will help you remember when trainings and or documents are going to expire. You could put reminder on your phone or use your calendar to note expiration dates such as CPR/FA. Please visit the DCDEE website regularly and click on the "What's New" tab to stay on top of all updates occurring with the agency such as new rule rollouts, etc. The violations cited during today's visit must be corrected immediately, and a letter must be sent to me no later than April 15, 2026, stating how the violations have been corrected and a plan to maintain compliance with the requirement moving forward. Today's visit was conducted with the owner/operator of Little China Home Daycare, Mrs. Wenmin Chen. Mrs. Chen’s husband works from home and was present during today’s visit to translate when necessary. Mrs. Chen has also hired a part-time assistant caregiver, Ms. Xiuzhu Hu, who was present today. Ms. Hu had all the required provider documentation available for review today. The FCCH operates with a 1 STAR license issued on January 18, 2023. Mrs. Wenmin is approved to operate on first and second shift but currently has children enrolled on first shift only. This FCCH operates first shift from 8:30am- 5:30pm M-F. Little China Home Daycare is licensed to care for 5 preschool age children and one school age child. Currently you have four (4) children ages 1-3 years old enrolled on first shift. I completed a walkthrough of the children's space, kitchen, bathroom, and outdoor play area. I observed four childcare children between the ages of 1-3 years old present in the home today. I observed the childcare children engaged in indoor activities, taking a walk to the neighborhood playground, which is where they go daily for their outdoor playtime, having their diapers changed and preparing for lunch. I observed Mrs. Wenmin and Ms. Hu engaged in the children’s activities, changing diapers and preparing lunch while maintaining appropriate supervision of the four children present today. INSPECTION/SAFETY DRILL STATUS: The last documented lockdown drill was conducted on 7/17/2025. The last outdoor play area inspection was completed on 10/17/2025. The last fire drill was conducted on 9/8/2025. The last annual compliance visit was conducted on 6/10/2025. ANNUAL TRAINING HOURS: Mrs. Wenmin has not completed any annual training hours since her annual compliance visit was conducted on June 10, 2025. She stated she plans to begin them soon. SECRETARY OF STATE: Owner/Operator, Mrs. Wenmin, operates under Little China Home Daycare, LLC, which was found to be current and active on the Secretary of State website. AREAS MONITORED During this visit, a partial monitoring of applicable childcare requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Staff Records, Children’s Records, Program Records, License Posted, and Permit Restrictions. INDOOR ENVIRONMENT: The childcare space is located in the front of the home as you enter the house. The kitchen is blocked off from the childcare children. For outside play, the children use the fenced in neighborhood playground located down the street which is walking distance from the provider’s home. There are five highchairs available for the children to use during meal and snack times. There is also a child size table in the childcare space for the children to use as well. The provider, Ms. Wenmin, informed me of the following: •She does not provide transportation. •She has had no incidents to warrant an incident/accident report. •She currently has no children that require any medication, over the counter or prescription. Two violations were observed and cited during today’s visit. The provider did not conduct the required monthly fire drills. No fire drill was conducted in October 2025. .1721 (e )(2) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. RATED LICENSE Mrs. Wenmin currently operates with a One Star license. Today, Mrs. Wenmin stated she is researching the new Quality Rating Improvement System, Pathway to the Stars, to decide if she wants to move forward with this option to increase her stars. I told Mrs. Wenmin after reviewing the Pathway to the Stars, if she has any questions, we can schedule a call or another visit to address any and all questions. Mrs. Wenmin and her husband shared that they are thinking about moving. They are not sure if they will stay in Cary or move to another adjacent city. They shared once they decide what their plans are to move or stay in their current home, and have a solid timeline for that, they will then reach out to further discuss moving up in stars. REMINDERS: The following information was discussed with you today: *I encouraged Mrs. Wenmin to continue to review all the required documents that must be maintained to keep her FCCH safe and in compliance. I suggest she review all required documentation quarterly to confirm all forms are in place, up to date, and available for review. Fire drills are required monthly. Document dates that you will conduct fire drills each month ahead of time. Fire drills teach children what to do in case of a fire. If fire drills are not practiced monthly, it will affect the children’s safety if there is a fire. Emergency drills are conducted every 3 months. I suggested Mrs. Wenmin ask her husband to set off the fire alarm so she, Mrs. Wenmin. can be with the children throughout the entire drill Document the dates you plan to conduct the emergency drills as well. I also suggested that Ms. Wenmin schedule the required monthly Fire Drills on the same day each month, maybe the first or last day of month, which could help with not missing a month in the future. *Maintaining and filing daily attendance records is extremely important. You never know when a situation could come up with a child in your program that would require you to know the exact dates and times the child was in your care. Today's visit was conducted with the owner/operator of Little China Home Daycare, Mrs. Wenmin Chen. Mrs. Chen’s husband works from home and was present during today’s visit to translate when necessary. Mrs. Chen has also hired a part-time assistant caregiver, Ms. Xiuzhu Hu, who was present today. Ms. Hu had all the required provider documentation available for review today. The FCCH operates with a 1 STAR license issued on January 18, 2023. Mrs. Wenmin is approved to operate on first and second shift but currently has children enrolled on first shift only. This FCCH operates first shift from 8:30am- 5:30pm M-F. Little China Home Daycare is licensed to care for 5 preschool age children and one school age child. Currently you have four (4) children ages 1-3 years old enrolled on first shift. I completed a walkthrough of the children's space, kitchen, bathroom, and outdoor play area. I observed four childcare children between the ages of 1-3 years old present in the home today. I observed the childcare children engaged in indoor activities, taking a walk to the neighborhood playground, which is where they go daily for their outdoor playtime, having their diapers changed and preparing for lunch. I observed Mrs. Wenmin and Ms. Hu engaged in the children’s activities, changing diapers and preparing lunch while maintaining appropriate supervision of the four children present today. INSPECTION/SAFETY DRILL STATUS: The last documented lockdown drill was conducted on 7/17/2025. The last outdoor play area inspection was completed on 10/17/2025. The last fire drill was conducted on 9/8/2025. The last annual compliance visit was conducted on 6/10/2025. ANNUAL TRAINING HOURS: Mrs. Wenmin has not completed any annual training hours since her annual compliance visit was conducted on June 10, 2025. She stated she plans to begin them soon. SECRETARY OF STATE: Owner/Operator, Mrs. Wenmin, operates under Little China Home Daycare, LLC, which was found to be current and active on the Secretary of State website. AREAS MONITORED During this visit, a partial monitoring of applicable childcare requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Staff Records, Children’s Records, Program Records, License Posted, and Permit Restrictions. INDOOR ENVIRONMENT: The childcare space is located in the front of the home as you enter the house. The kitchen is blocked off from the childcare children. For outside play, the children use the fenced in neighborhood playground located down the street which is walking distance from the provider’s home. There are five highchairs available for the children to use during meal and snack times. There is also a child size table in the childcare space for the children to use as well. The provider, Ms. Wenmin, informed me of the following: •She does not provide transportation. •She has had no incidents to warrant an incident/accident report. •She currently has no children that require any medication, over the counter or prescription. Two violations were observed and cited during today’s visit. Violation Number Comment Rule 905 Record of monthly fire drills, giving the date, time of day and length of time to evacuate, and operator's signature, were not available. The provider did not conduct the required monthly fire drills. No fire drill was conducted in October 2025. .1721 (e )(2) 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. During today's visit, the provider did not have attendance records available for review, for any of the children, from October 1, 2025 - November 12, 2025. .1721(e)(6) COMPLIANCE HISTORY According to NC General Statute all day care centers must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. RATED LICENSE Mrs. Wenmin currently operates with a One Star license. Today, Mrs. Wenmin stated she is researching the new Quality Rating Improvement System, Pathway to the Stars, to decide if she wants to move forward with this option to increase her stars. I told Mrs. Wenmin after reviewing the Pathway to the Stars, if she has any questions, we can schedule a call or another visit to address any and all questions. Mrs. Wenmin and her husband shared that they are thinking about moving. They are not sure if they will stay in Cary or move to another adjacent city. They shared once they decide what their plans are to move or stay in their current home, and have a solid timeline for that, they will then reach out to further discuss moving up in stars. Today's visit was conducted with the owner/operator of Little China Home Daycare, Mrs. Wenmin Chen. Mrs. Chen’s husband works from home and was present during today’s visit to translate when necessary. Mrs. Chen has also brought a part-time assistant caregiver, Ms. Xiuzhu Hu, who was present today. The FCCH operates with a 1 STAR license issued on April 16, 2020. This FCCH operates first shift from 8:30am- 5:30pm M-F. You are licensed to care for 5 children ages 1-3 and one school age child on the first shift. Currently you have five (5) children ages 1-3 and one school age child enrolled in your family child care home. Today, I completed a walk-through of the family child care home, five (5) 1yr-3yr. year old children were present today along with one school age child. When I arrived the children were having lunch and preparing for nap time. The provider, Mrs. Chen, and her assistant, Ms. Hu, were sitting with the children assisting them with their meal. After lunch Mrs. Chen assisted the children with hand washing and going down for nap and Ms. Hu cleaned up the lunch table. Ms. During today’s visit, a full assessment of childcare requirements was conducted. A checklist was used to note the items monitored and provided to the owner/operator at the conclusion of the visit. Mrs. Chen completed all of the required Health and Safety training on 8/13/2024 and is required to renew her Health and Safety training by 8/13/2029. Emergency telephone numbers were readily accessible and emergency procedures were posted. A written daily schedule was posted. The daily schedule reflected outside time. Child Care Space: The child care space is located on the entire first floor of the home, minus the kitchen area which is to remain blocked off from the children at all times. For outside play, the children use the neighborhood park which is walking distance from the family childcare home. Ms. Chen is using the DCDEE playground check list to complete her required checks of the neighborhood playground. Safety/Inspections: Fire drills, Outdoor playground inspections, and EPR drills were current and on file. The EPR plan was also observed to be current and on file. Transportation: Mrs. Chen is not approved for transportation. *****Violations****** Two violations were observed during today’s visit. Violation Number Comment Rule 1409 Operator did not provide a physically safe and healthy indoor and outdoor environment that meets the developmental needs of the children in care. During today's visit the kitchen area was accessible to the children and the stove top burner was on with a large pot of boiling liquid was observed. 10A NCAC 09 .1719 (a) 1922 Plastic bags, toys, and toy parts small enough to be swallowed, including but not limited to materials that can be easily torn apart such as foam rubber and styrofoam, were accessible to children under the age of three. Three plastic bags of diapers were observed accessible to the children during today's visit. .1719(a)(18) COMPLIANCE HISTORY According to NC General Statute all day care centers must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for this family child care home was 98%. Any violations cited during today's visit may affect this percentage. AGENCY UPDATES DCDEE UPDATES AND NEWS WATER TESTING • Mrs. Chen sent off her water test and received notice on 4/1/24 that her test results came back indicating normal range. REMINDERS: The following requirements expire in the future; remember to complete these prior to their expiration. -CRC - every 5-years -CPR/FA – every two years (update before 8-9-2025) *20 training hours received this year. *Please note, first aid and CPR training is no longer counted towards training hour requirements. CONSULTATION AND TECHNICAL ASSISTANCE • HOT SURFACES PLAYGROUND- Playgrounds are often a hub of joy and activity for children, offering a space where they can run, jump, and play freely. However, as temperatures rise, so do the risks associated with hot surfaces on playgrounds. The scorching heat of the sun can turn slides, swings, and other equipment into potential burn hazards, causing burns and discomfort for children of all ages. To lessen these risks and ensure a safe environment for play, it's essential to inspect hot surface areas and implement cooling measures. It is important to check all equipment that children could touch with their hands, including handles, rails, platforms, slides, asphalt or concrete (if sitting during chalk play), rubber surfacing in the fall zones, steering wheels, monkey bars, geo dome climbers, swing seats, chains on swings, tire swings, tunnels that children can sit on or climb in, and anything metal or dark colored. One of the primary concerns that comes to mind during hot weather is the temperature of playground equipment. Metal slides and swings, in particular, can absorb and retain heat, becoming dangerously hot to the touch. Black rubber swing seats get hot. Darker colors heat faster than lighter colors. Children, unaware of the potential danger, may unintentionally come into contact with these heated surfaces, resulting in burns or severe discomfort. Additionally, rubberized surfaces such as those found in some playgrounds can also become excessively hot, posing a risk to bare skin. Regular inspections of playground equipment are crucial, especially during hot weather conditions. Maintenance personnel should routinely check for signs of excessive heat buildup on surfaces, focusing on areas that are exposed to direct sunlight. Using specialized tools like infrared thermometers can help accurately measure surface temperatures and identify potential hot spots. • Plastic Bags- Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. Any plastic bags must be stored out of reach of the children, which is five feet or higher from the finished floor. Three plastic bags of diapers were accessible to the children today. Mr. Chen immediately moved the plastic bags out of the reach of the children. • Safe Environment – Today I observed the kitchen area accessible to the children. There was a pot with something boiling on the stove top and all the cabinets and the pantry were accessible to the children. It is imperative that you keep the kitchen area blocked off from the children at all times. Especially when cooking appliances are in use. Injuries such as burns can occur in a second and as the child care provider it is your responsibility to confirm a safe indoor and outdoor environment is being offered to the children at all times. The provider did block off the area when I brought this concern to their attention. • Ms. Chen inquired about increasing her capacity. I suggested she start this inquiry by calling the Town of Cary. And forwarded her additional information once I returned to my office. • Mr. and Mrs. Chen are currently working on updating the Automated Background Check Management System (ABCMS). This access will allow providers to: (This is a requirement for ALL FCCH providers and Child Care Centers and must be competed immediately. *Access to the ABCMS requires the successful completion of a Moodle course consisting of a short video followed by a test. The course is titled ABCMS Child Care Provider Portal Training - under the Early Childhood Professional Development Tab, then under the Criminal Background (CBC) tab. This Family Child Care Home is currently licensed for first and second shift and operating with a One Star License. You had your routine unannounced visit conducted on December 8, 2022, and your last annual compliance visit was conducted on August 2, 2022. Wenmin Chen, Owner/Operator was present and assisted me during the visit. The operator’s husband was present during today’s visit. Your current corporation status is listed as Current-Active on the NC Department of the Secretary of State Website. There were five children present today between the ages of 2-5 yrs. I observed the children engaged in indoor activities, eating lunch, and going down for nap. I used the Annual Compliance Monitoring Checklist for Family Child Care Homes to monitor your program today. The monitoring checklist was reviewed with you at the end of your annual compliance visit today. The provider does not provide transportation to her childcare children and stated she understands she may not provide transportation to the children in her care without an additional visit from a DCDEE Licensing Consultant for prior approval. The transportation requirements were not monitored during today’s visit. The provider does not have any pets. One violation was cited and corrected during today’s visit. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Provider conducted the required outdoor play area check during today's visit. 10A NCAC .1721(e)(5)(A-F) CONSULTATION In order to improve the quality of your program, we discussed the following topic: 1) I did suggest to the provider that she use a milk crate or file box to house all of the required paperwork. This allows all paperwork to be housed in one location and when your consultant conducts visits everything is in one place, so he/she does not have to interrupt you while you are caring for the children. a. I reminded the provider to have the parents of her childcare children update their child’s emergency information annually. b. I suggested the provider attach the Children's File Check List to the front of each child’s file and arrange the paperwork in the order documented on the checklist. c. I suggested the provider do the same as above with her file and her husband’s file using the Provider File Check List. d. I suggested when the provider is enrolling a new child in her FCCH, to request the parent submit ALL of the required paperwork at least 1 week prior to the agreed start date. This will allow the provider time to review the submitted paperwork and request any additional paperwork or corrections be made before the child’s first day. 2) I left the Challenging Behaviors Helpline flier with Ms. Chen if she needs to utilize the assistance available to her. 3) I suggested the provider prepare meals the night before or early morning before the children arrive to allow for a more seamless mealtime. ***SUBSTITUTE INFORMATION*** The provider, Ms. Chen, informed me today she would like her husband to complete all requirements to be able to substitute for her when necessary and/or during emergency situations and that she is thinking of hiring an assistant to use as needed. Please review rule 10A NCAC 09 .1729 regarding ADDITIONAL CAREGIVER’S AND SUBSTITUTE PROVIDER QUALIFICATIONS so all required trainings and documentation are completed BEFORE you have anyone work with you and/or during your absence. I left the FCCH Substitute & Additional Caregiver Checklist with Ms. Chen to refer to. Moving forward, I will be your assigned childcare consultant.
