A temporary license was issued January 14, 2026, through July 14, 2026. The facility was approved for 181 children aged 0-12 years old on first shift. Today’s visit was completed with the center director, Christina Power. I monitored seven classrooms for staff/child ratios, group sizes, capacity, supervision, discipline, interactions, materials, postings, activities, nutrition, and health and safety. Children were observed playing outside, chasing bubbles, riding tricycles, and hugging their friends. Teachers were observed moving about the indoor and outdoor environments, interacting with the children in a positive manner. The children appeared to be happy in the routine of the classroom. Fire drills: Completed as required. Outdoor inspections: Completed as required. Incident reports: Nine incidents that did not require professional medical treatment were on file for March 2026. Outdoor play area: No changes. Children were outside playing during the walk through. ABCMS: In progress. The director plans to submit the power form and update the staff roster. I reviewed fourteen children’s records today. A children’s records checklist was completed to show files reviewed and any information that was missing. All required information was on file for the files reviewed. I reviewed fifteen staff files during today’s visit. A staff and training worksheet was provided to show files reviewed. Two teachers had TB tests on file that were more than a year old when hired. Three teachers did not have a TB test on file at all. Three teachers did not have a medical on file. One other teacher will start around 4/6/26 and must have a TB test on file that is current or less than a year old as well as a medical completed by a licensed physician. QRIS: I confirmed education status for four staff members. Education is the most important piece to have in place first. I will pull education status in WORKS and will email you to let you know the outcome or if information is pending. We will set up a virtual meeting to review the Classroom and Instructional quality pathway 2 requirements. Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. Two teachers had a TB test that was older than 12 months. Three teachers did not have a TB test on file at all. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Three teachers did not have a medical on file. .0701(a) Reminders: Continue to work on setting up the ABCMS portal. Technical assistance: Maintaining staff records can be time-consuming. You currently use the staff file checklist to ensure information is on file and have procedures in place to double-check files. Slow down and review information that is handed to you during a busy time so you won’t miss dates on TB tests and medicals for staff. The violations cited today must be corrected immediately. Email me a letter stating when the violations were corrected, how the violations were corrected and how you plan to prevent repeat violations in the future.
