Today's visit was conducted with Stephanie Bechtel, Administrator. Children were observed participating in routine activities in their rooms and playing outside. A partial assessment of applicable childcare requirements was conducted. The last annual compliance visit was conducted on 8/26/24. The last fire inspection was conducted 11/14/24. The last sanitation inspection was conducted 5/6/24. Staff and training worksheets and files for new staff hired since the last annual compliance inspection were reviewed. A fire drill was last conducted 1/14/25. The last lockdown drill was conducted 12/6/24. Outdoor inspections were observed to be completed monthly. Medications are stored in each room in the child's cubby which is above 5 feet in height. 3 Emergency medications were observed. One medication was observed expired. All 3 did not have the medication administration permission form. Medication forms were kept in the children's files. All required postings were observed. This operation has not signed up for the ABCMS background check system. Today I observed 2 violations regarding childcare requirements listed below and these violations were discussed with you in detail during the visit. Violation Number Comment Rule 847 Parent's medication authorization did not include required information. 3 emergency medications were observed to not have parent medication authorizations on file. 10A NCAC 09 .0803(4)(6-9) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. One emergency medication was on hand and was observed to have expired in November 2024. .0803(12) The violations documented above must be corrected immediately. A signed and dated letter of compliance must be mailed by March 12, 2025 stating that all violations have been corrected. TECHNICAL ASSISTANCE We discussed the requirement for medication authorization forms and that they can only be valid for 6 months. I recommend using some form of tracking to ensure they are reviewed regularly and renewed before they expire. Tracking of medication expiration dates would also be beneficial. As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Applicable childcare requirements and the inspection was discussed with Administrator, Stephanie Bechtel. Currently this facility operates with a 5 star license effective April 27, 2020. Secretary of state information for the governing body was observed active and current. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities. I observed whole group activities and free play activities. Age appropriate materials and equipment was accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored, permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. 9 children's records were reviewed and staff records were reviewed along with the staff and training worksheet. The last fire inspection was completed 11/16/23 and the last sanitation inspection was completed 5/6/24. The last fire drill was completed on 8/23/24. The last lockdown drill was completed 6/7/24. The operation's EPR plan was last updated 4/12/24. Outdoor inspections were observed to be completed monthly. Diaper creams and suntan lotions were observed to have current permissions in all rooms. Sleep checks were observed to be completed. 3 emergency medications were observed throughout the operation. All medications were stored securely and not expired. Permissions and allergy plans are maintained in the children's files. Transportation is not provided by this facility. I observed 1 violation during this inspection. Violation Number Comment Rule 1867 The depth of the loose surfacing was not based on critical height of the equipment. Surfacing at the ends of both slides on the older children's playground measured an inch and a half or less. .0605(k)(1-4) The violations documented above must be corrected immediately. Providers in Cohort 1 are not required to go any further with a rated license assessment unless they want to, and providers in Cohort 2 do not need to start their preparation year unless they want to. This is optional and up to your discretion at this time. I would advise to still work on preparing and ensuring staff education is current in WORKS. We discussed accreditation and how that works with the new QRIS process and processing star ratings. Currently the operation is not accredited. If you've had a lead water test in the past it is likely due (every 3 years). Visit cleanwaterforcarolinakids.org and apply for a water test timely before you receive a sanitation inspection. Please sign up for lead paint testing and asbestos testing on the same website. During signup after you complete the questionnaire, you may receive a letter(s) of exemption. Please keep these letters on file. On the same website for lead water tests you need to sign up for asbestos and lead paint testing. You will have to complete a questionnaire. At the conclusion of the questionnaire you may receive a letter of exemption. If you receive a letter please maintain it on file. The last annual compliance visit was conducted on 9/1/23. Today’s visit was conducted with you, Stephanie Betchel, Center Director. Your program currently operates with a Five-Star license. You accompanied me as I walked through the facility today. I observed in classrooms for an extended amount of time. Children were playing outside, participating in group time, listening to books being read, completing group art activities, and completing routines while preparing to transition outside. Infants were resting , playing on the floor, manipulating materials, and interacting with staff other infants. One group of infants were transitioning to rest time and were crying at once. The staff member attempted to calm the children while helping them transition to rest time. You assisted the staff member with the crying infants. The staff member then begin warming bottles to prepare to attempt to feed the infants to assist them to calm down. I observed staff complete name-to-face recognition while transitioning children outside in a classroom with three-year-olds. Staff assisted children with activities. I observed a staff member comfort a child that was upset in the room for older children. Staff also redirected younger children from different behavior both indoors and outdoors. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Three new staff’s files were monitored. Staff’s qualifying letter and Special Trainings still complied from the previous visit. Staff’s CPR and first aid certifications were current. Fire and sanitation inspections were current. Fire drills and playground inspections were current. One violation related to emergency drills was documented. The emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements complied. Medication requirements were monitored The facility continues to meet requirements for adequate and approved space. The NC Secretary of State website was reviewed the Owner was listed as current- active. Violation Number Comment Rule 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last drill, shelter-in-place, was practiced on 9/16/23 and not every three months since. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before 3/13/2024, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Updates/Resources: Today I discussed information regarding Clean Classrooms for Carolina Kids program to identify and eliminate exposure to lead and asbestos hazards in child care. Be sure to visit DCDEE website and access the What’s New section for the newest child care rules that were effective January 1, 2024.
