    Name of Operation: KIDDIE ACADEMY OF WEST CARY Facility ID: 92003267 Consultant: KIMBERLY GIROUARD Operation Type: Center Case Number: Visit Date: 4/7/2025 Number Present: 144 Completed Date: 4/7/2025 Age: From 0 To 5 Total Minutes: 315 Time In: 10:30 AM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during a Routine Unannounced visit. Kiddie Academy of West Cary operates with a Five (5) Star Child Care License issued on August 22, 2024. Director, Ms. Tracey Wilson, was present at the facility today and accompanied me throughout my visit. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, fire and sanitation inspections and playground inspections. All reports were found to be in compliance. CORPORATION STATUS Your current corporation status is listed as “Current-Active” on the North Carolina Secretary of State Website. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. Staff qualifications were discussed and reviewed. Seven new staff files were reviewed and a spot check of the remaining staff files was conducted. All spaces were monitored for supervision, staff/child ratios, staff/child interactions, materials, equipment and required postings. OBSERVATIONS I completed a walk-through of the facility. At the time of the visit, there were a total of 110 children present between the ages of 0yrs-5yrs. The children were observed engaged in indoor activities, having lunch, and going down for nap. The staff were observed engaged in the children’s activities while providing appropriate supervision. One violation were cited during today’s visit. Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. Sleep check documentation was not completed correctly. Consultant observed three different times that a staff member did not include the time the sleep checks were conducted or identify the initial position then infant was placed in the crib when going down for nap. 10A NCAC 09 .0606(a) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. Prior to today’s visit, the compliance history score for the center was 100% REMINDERS *Medical Action Plans – This facility has a lot of emergency medication. All documentation was found to be in compliance today. As a reminder, the maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. I suggested to the director that she review the names of all enrolled children who have emergency medications at every staff meeting to provide a constant reminder to everyone in the building. TECHNICAL ASSISTANCE *During today’s visit we discussed the importance of completing the safe sleeps checks correctly at all times. The caregiver placing the infant down for nap must initially record on the safe sleep chart their initials, the time the infant is being placed in the crib and the position the infant is placed in the crib. The caregiver must also continue to complete sleep checks throughout the infant’s nap. The caregiver conducting the sleep checks must do so at the intervals identified in the facility’s Safe Sleep Policy. We discussed the importance of always documenting that infants are placed on their back when being placed in their crib for nap time, unless there is a Written Waiver on file, signed by the child’s parent/legal guardian. This includes if the infant rolls onto their tummy or side when being placed down for nap. By documenting tummy or side when the infant is being placed in the crib for the first time, the care giver is stating they purposefully placed the infant on their tummy or side when placing them in the crib for nap time. Sleep checks should tell a clear story of how the infant was initially placed in the crib when going down for nap and the time the infant went down for nap, each position the infant was observed in during each sleep check conducted by the infant room staff, and when the infant woke up. I suggested that if an infant does not nap on any given day, that the infant room staff document on their safe sleep chart, NO NAP, on that specific day or if an infant is absent on any given day that the staff record absent on the day. Specialists are available with early childhood education backgrounds who are passionate about empowering teachers to develop learning environments and teaching practices that promote prosocial skills in young children. A flier with the challenging behaviors hotline information was provided today. *Background Checks – I sent Ms. Wilson information on the new Automated Background Check Management System (ABCMS) so she can begin to link her staff to the facility. *Please continue to visit DCDEE’s website for child care updates at https://ncchildcare.ncdhhs.gov/. Upon arrival, I was greeted by the director, Ms. Tracy Wilson. Ms. Wilson accompanied me on my walk-through of the facility today. At the time of my visit there were a total of 147 children present. The children were observed engaged in circle time and participating in art projects, playing outside, eating lunch, and napping. The classrooms were nicely equipped with age-appropriate materials. I observed two infant rooms today. Space 1 had 8 infants present today. Three infants were observed napping and five infants were observed engaged in floor activities. While monitoring the room today, I observed the teachers conducting safe sleep checks. Space two (2) had five (5) infants present today. All five infants were awake and engaged in floor activities during my visit. During my walk-through of the facility, I observed the staff engaged in the children’s activities, engaged in conversations with the children, and providing appropriate supervision. LICENSE STATUS Currently this center operates with a Five Star license issued September 20, 2019. The restrictions on the license state Daytime Care only, Meets enhanced ratios, Meets enhanced space. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire, and outdoor inspections. All were found to be in compliance today. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children. Staff files were reviewed. A selection of children’s files was reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today and given to the director at the conclusion of today’s visit. The following violations were observed during today’s visit. Violation Number Comment Rule 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Three baby food containers were observed in an infant room without the required date on each container of food. 15A NCAC 18A .2804(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. One child did not have the required medical exam on file within 30 days of their admission to the program. GS110-91(1) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff member stopped working at the facility and stared working at the facility again 11 months later but did not return to work with an updated medical retort or updated TB test. .0701(d) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 99%. Any violations cited during today's visit may affect this percentage. TECHNICAL ASSISTANCE • I reminded the infant teachers to confirm all bottles and food being dropped off by the children’s parents be labeled with the child’s name and dated accordingly. • I suggested a reminder message be sent to all parents reviewing the rules and requirements to maintain compliance with documentation of medication forms. And to make sure a staff member reviews all medication documentation when medication and forms are being left in the classroom. • In the event staff decide to no longer work at the facility and later come back to work at the facility, a new medical form, signed off by a medical professional, and a new TB test will need to be completed prior to returning to work at the facility. CONSULTATION/NOTES To improve the quality of your program, we discussed the following items: • During my walk-through of the facility the director and I observed the children eating in some of the classrooms. We discussed the importance of the staff always being within arm’s reach of the children during snack time and mealtime. • All rules were being met in the infant rooms regarding the infant safe sleep policy. I suggested the staff mark “No Nap” when an infant does not nap on any given day. We also discussed that unless there is a waiver in place, signed by the infant’s parent and doctor, all infants are to always be placed on their back when being placed in their crib for a nap. This rule stands even if the infant rolls to their side or tummy when being placed in the crib for nap time. • I reminded the infant teachers to confirm all bottles and food being dropped off by the children’s parents be labeled with the child’s name and dated accordingly. • I reminded the director to also check the expiration date on all medication. Sometimes the medication itself will expire before the required medication forms expire. • As discussed, when updating the EPR plan annually, you may choose to only print updated pages and the first page which lists the date the plan was last reviewed. At a minimum I would advise printing the first page and updating the binder annually to avoid confusion. Rated License/COHORT 1 Information Kiddie Academy of West Cary is in COHORT 1. The facility recently participated in a Mock Assessment offered by NCRLAP and are waiting for their results. We discussed how the facility can use the results of the mock assessment. Prior to today’s visit I emailed resources you could think about using as you begin preparing for your Rated License and Environmental Rating Scale visit. As discussed today, it is very important that all your staff create a WORKS account as soon as possible, if they have not already done so, and begin submitting any education that is not already documented in their WORKS account and continue to do so as they complete additional semester credit hours or degrees. DCDEE in collaboration with other agencies have started a challenging behavior hotline. To Speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in your classroom please call 1-888-600-1685 and choose Option 1. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected.
