Applicable childcare requirements and the inspection was discussed with Administrator, Virginia Macon. Currently this facility operates with a 5 star license effective 11/15/24. During the visit I monitored the indoor and outdoor spaces used for child care. I observed children during routine activities. I observed whole group activities and free play activities. Age appropriate materials and equipment was accessible to all children. The facility was monitored for compliance using the annual compliance monitoring checklist. I also monitored, permit restrictions, posting requirements, training requirements, use of approved space, program records, and nutrition. 4 children's records were reviewed and staff records were reviewed along with the staff and training worksheet. The last fire inspection was completed 9/3/25. The last fire drill was completed on 8/20/25. The last shelter-in-place drill was completed 6/5/25. The operation's EPR plan was last updated 10/4/24. Outdoor inspections were observed to be completed monthly. Diaper creams and suntan lotions were observed to have current permissions in all rooms. Sleep checks were observed to be completed. Menus were posted in each room. 3 emergency medications were evaluated during inspection. Allergies were posted in the rooms. Screen time is not practiced at this facility. The operation has completed enrollment in lead paint and asbestos testing. The operation was confirmed to have a current roster in the ABCMS system during inspection. Transportation is not provided by this facility. I observed 1 violation during this inspection. Violation Number Comment Rule 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One staff was observed to have CPR that expired in April 2025. .1102(d) The violations documented above must be corrected immediately. The last sanitation inspection was conducted 2/13/25. Staff and training worksheets and files for new staff hired since the last annual compliance inspection were reviewed. The last shelter-in-place drill was completed 9/18/24. The operation's EPR plan was last updated 3/23/23 which is past due. Outdoor inspections were observed to be completed monthly. Diaper creams and suntan lotions were observed to have current permissions in all rooms. Sleep checks were observed to be completed. There were no emergency medications present in the facility during inspection. Transportation is not provided by the facility. I observed 5 violations during this inspection. Documentation of a completed TB test and medical report was not able to be observed for one employee hired in 2023. .0701(d) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Virginia Macon, principal, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Macon. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On August 13, 2024, a staff member assigned to the classroom for one-year-old children held a child while on the changing table, leaving fingernail markings under the child’s arms. Eboni Jones, Regional Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Jones and Susan Moore, Assistant Principal. Limited monitoring of child care requirements occurred during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. On November 21, 2023, a staff member in the classroom for four-year-children excessively used personal and facility provided electronic devices and thus provided little engagement and supervision of the children. .1801(a)(1-5) 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. On November 21, 2023, a staff member in the classroom for four-year-old children provided screen time to the children for more than thirty minutes. .0510(d)(2)(A-C) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. On November 21, 2023, a staff member disciplined a four-year-old child by placing masking tape on the child's mouth and excessively placing the child in timeout for extended periods of time. On this same date, the staff member repeatedly spoke to the child in a harsh and intimidating manner. G.S. 110-91(10) 904 Child was handled roughly. On November 21, 2023, a staff member pulled and grabbed a four-year-child by their arm, slapped the child on their face and hand, and placed masking tape on the child's mouth. .1803(a)(1) 907 Discipline was related to food, rest or toileting. On November 21, 2023, a staff member placed a four-year-child in time out and prevented the child from eating with the rest of the class. .1803(a)(4-6) 1810 There was a substantiation of child maltreatment.
