This facility currently operates with a One (1) Star License issued on 8-22-18. The last annual compliance visit was conducted on 10-9-24. Today’s visit was conducted with you, Udayapangika Yapa, owner/operator. Upon arrival to the facility, I was greeted by you. A walk-through of the downstairs area of the residence was conducted visiting the one (1) room used for childcare. The required postings, along with appropriate equipment and furnishings were observed. The emergency drills, outdoor inspections, the activity plan, daily attendance, and five (5) children’s files were also observed and in compliance. Documentation of program records, as well as the operator verification information was observed and has been documented on the attached form. You stated that you do not transport children. The only medications observed for children on the premises were three (3) topical ointments. The required permission to administer the medications was observed. The children were observed on the floor moving about with age appropriate toys and equipment. You completed diapering and handwashing routines and feed the children as required. A caring and nurturing environment was observed as you interacted with the children and attended to their needs. During today’s visit, an assessment of applicable child care requirements was conducted. The following violation of child care requirements was cited and must be corrected immediately. Technical assistance on how to maintain compliance was provided. Violation Number Comment Rule 1875 The EPR Plan did not include the location of the Ready to Go File and/or required information. A Ready to Go File with the required information was not available. .1714(d)(10) The above violation must be corrected immediately and a letter must be sent to me no later than 10-17-25 stating how the item was corrected and a plan to maintain compliance with the requirements. The following were discussed with you during today’s visit: • Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. • Technical Assistance was provided with the following suggestions that may be beneficial: Ready to Go File: The Ready to Go File requirement is crucial because it acts as a readily accessible collection of critical information needed to quickly respond to an emergency, allowing you to efficiently evacuate children, contact parents, and address specific needs during a crisis, ensuring the safety and well-being of all individuals in the residence. You need to create a Ready to Go File notebook/binder/accordion folder with all of the required paperwork. A Ready to Go File checklist was provided today for you to ensure required paperwork is added to your notebook/binder/accordion folder. At least monthly, your Ready to Go File should be monitored to ensure that it is up to date. The file should be updated when new children enroll or disenroll, and/or when new medication is provided for a child or when the medication is no longer needed, and/or when a child has an allergy, the allergy list needs to be modified. It is suggested to mark your calendar or set an alarm to remind you to update your EPR Plan/Ready to Go File. • You were reminded about child care rules in .1721(f)(4) (A-B) regarding written record retention of sleep charts, attendance, fire drills, and outdoor play area inspections since you have many years of paperwork. • Your lead in water results were completed on 11-14-2024. Asbestos and lead in paint testing needs to be completed asap from Clean Classrooms for Carolina Kids. • You have linked you and your household members in the ABCMS portal. As a reminder, your twenty-four (24) year old’s qualifying letter will expire on 12-2-25. • The Division of Child Development and Early Education’s QRIS modernization requirements went into effect July 1, 2025. These requirements replace the old point-based system with three flexible "Pathways to the Stars" for earning star ratings in program assessment, classroom and instructional quality, and accreditation/Head Start. This new framework offers clear requirements for each star level within a pathway, expands education options for early childhood professionals, and focuses on continuous quality improvement and family/community engagement. Upon arrival, I was greeted by Udayangika Yapa, owner/operator, she accompanied me throughout the visit. I completed a general walk through of the children's space, kitchen and bathroom. During my visit today, the provider was caring for one infant and two toddlers. The infant was awake throughout my visit today and the toddlers were napping. I observed the provider feeding the infant a bottle, changing the infants’ diaper, and engaging the infant in tummy time activities. The provider also conducted sleep checks on the toddlers. This family childcare home currently operates with a One-Star License that was issued on April 4, 2018. All lunches are prepared by the parents and the children eat in highchairs. INSPECTION/SAFETY DRILL STATUS: The last documented shelter-in-place or lockdown was conducted on 1/10/2025 (SIP) The last outdoor play area inspection was completed on 3/20/2025. The last fire drill was conducted on 3/20/2025. The last annual compliance visit was conducted on 10/9/2024. LEAD WATER TESTING Ms. Yapa provided documentation that on 11-14-24 her lead water test results were returned within normal range. ANNUAL TRAINING HOURS: Ms. Yapa has completed 8.5 of her 10 annual training hours. She still has 6 months to finish her last 1.5 of annual training hours for the 24/25 fiscal year. SECRETARY OF STATE: Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Carolina Babies & Tots, LLC, is listed as current/active. During this visit, a partial monitoring of applicable child care requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Provider Records, Program Records, License Posted, and Permit Restrictions. Child Care Consultant also monitored children's records and found one child to be missing the required medical examination report. INDOOR ENVIRONMENT: The home is a two-level dwelling, which consists of a first and second floor. The provider cares for children on the main floor. In the children’s spaces there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to the children that are potty trained, a changing table is available for those in diapers. During today’s visit there were three (3) children present between the ages of 0 yrs. – 2 yrs. old. The outdoor play area is in the backyard of the home and is fenced in. The provider informed me of the following: •She does not provide transportation. •She has had no incidents to warrant an incident/accident report. •She will apply diaper cream when requested. One diaper cream was monitored today and all required documentation was in compliance. One violation was cited during today's visit. One child file was found to be missing the required medical report. Upon arrival, I was greeted by Udayangika Yapa, owner/operator, she accompanied me throughout the visit. I completed a general walk through of the children's space, kitchen and bathroom. During my visit today, the provider was caring for one infant and two toddlers. The infant was awake throughout my visit today and the toddlers were napping. I observed the provider feeding the infant a bottle, changing the infants’ diaper, and engaging the infant in tummy time activities. The provider also conducted sleep checks on the toddlers. This family childcare home currently operates with a One-Star License that was issued on April 4, 2018. All lunches are prepared by the parents and the children eat in highchairs. INSPECTION/SAFETY DRILL STATUS: The last documented shelter-in-place or lockdown was conducted on 1/10/2025 (SIP) The last outdoor play area inspection was completed on 3/20/2025. The last fire drill was conducted on 3/20/2025. The last annual compliance visit was conducted on 10/9/2024. LEAD WATER TESTING Ms. Yapa provided documentation that on 11-14-24 her lead water test results were returned within normal range. ANNUAL TRAINING HOURS: Ms. Yapa has completed 8.5 of her 10 annual training hours. She still has 6 months to finish her last 1.5 of annual training hours for the 24/25 fiscal year. SECRETARY OF STATE: Prior to today's visit I reviewed the NC Secretary of State's website and observed that the owner of this facility, Carolina Babies & Tots, LLC, is listed as current/active. During this visit, a partial monitoring of applicable child care requirements was conducted, monitoring Supervision, Staff Child Ratio, CPR/First Aid, Special Training, Storage of Hazardous Substances, Storage of Medication, Adequate Approved/Space, Provider Records, Program Records, License Posted, and Permit Restrictions. Child Care Consultant also monitored children's records and found one child to be missing the required medical examination report. INDOOR ENVIRONMENT: The home is a two-level dwelling, which consists of a first and second floor. The provider cares for children on the main floor. In the children’s spaces there are a variety of materials for the children to explore throughout the day. The bathroom is accessible to the children that are potty trained, a changing table is available for those in diapers. During today’s visit there were three (3) children present between the ages of 0 yrs. – 2 yrs. old. The outdoor play area is in the backyard of the home and is fenced in. The provider informed me of the following: •She does not provide transportation. •She has had no incidents to warrant an incident/accident report. •She will apply diaper cream when requested. One diaper cream was monitored today and all required documentation was in compliance. One violation was cited during today's visit. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. One child file was found to be missing the required medical report. GS 110-91(1); 10A NCAC 09.1721(a)(1) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4)(d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Prior to today’s visit, the program’s compliance history score was 100%. Please submit a corrective action letter for the one violation cited during today’s visit. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When the violation was corrected.
