A full assessment of child care requirements was conducted today. Your program currently operates with a Five-star license. Your most recent license was issued on April 24, 2023, for 194 children on first shift and you are approved to care for children 0-12 years of age. Today’s visit was conducted with you, Mr. Leslie Holt, Center Director. You accompanied me throughout my walkthrough of the facility today. I, Kimberly Girouard, Child Care Licensing Consultant monitored eleven (6) six classrooms, the gym, three playgrounds and a splash pad for compliance with staff/child ratios, supervision, capacity, group size and staff interaction/discipline. The facility was monitored for health and safety requirements. I observed one (1) infant classrooms. Safe sleep documentation was being conducted as required and was found to be in compliance. One infant was awake and engaged in floor activities and one infant was napping. I observed the children throughout the rest of the building engaged in water play, center activities, participating in art projects, eating lunch and napping. I observed school age children leaving and returning from field trips throughput my visit today. I observed the staff following all proper protocols before leaving for the field and when they returned from the field trip. Today, I observed the staff moving about the classrooms, actively engaged in the children’s activities while meeting the needs of the children and providing appropriate supervision. I also observed the school age children coming and going on field trips through the day. I observed proper name to face attendance being taken before the children left the facility and when they returned to the facility today. Program requirements were monitored today. Bright Horizons Clubhouse At West Cary does provide transportation. Transportation requirements were monitored today and found to be in compliance. All inspections and fire drills were current. Children and staff records were monitored, and the information was documented on the children’s records form and staff and training worksheets. The following violations were cited during today’s visit. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff member did not complete the required 16 hours of orientation within the first 6 weeks of employment. .1101(a) 1805 A child care operator did not notify the Division of any new child care providers, as defined in G.S. 110-90.2(a)(2), who were hired or moved into the child care facility within five business days. The child care operator has not linked her staff in the Automated Background Check Management System. G.S. 110-90.2 & .2703(r) 1916 Written permission was not given by parent for child to participate in aquatic activity. Two children's files were missing the required parent sign off form prior to the children engaging in water play activities. .1403(i) TECHNICAL ASSISTANCE: *ABCMS- As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. You stated that you will complete the training and send me your certificate with the compliance letter. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS on an ongoing basis as staff members are hired and when their employment is terminated. Should you need assistance please contact the Criminal Background Check Unit at (919) 814-8401 and someone will assist you. I provided you with instructions for how to connect applications during the visit. * Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you updated of the status of out-of-state checks if a new employee has a Provisional Qualification. Instructions on how to link to the facility was provided today. In the future, new staff need to link within five days of hire or transfer to your center. Review Criminal History Record Check Requirements for Child Care Centers in the NC Child Care Requirements 10A NCAC 09 .2703. * Staff Orientation - Due within 1st Six Weeks – 1.Documentation of Staff Orientation (form available on DCDEE website; Child Care Center form and School-age Center Form) --16 hours of orientation required a. You are reviewing basic information on your center’s policies, procedures and handbooks in these topic areas d. I suggest creating an annual check off list for your staff and children files to confirm all required information is maintained and updated accordingly. *LEAD WATER TESTING -Centers are required to have their water tested every 3 years by the Clean Water for Carolina Kids™ program. Based on the documentation you shared with me today, your last water testing was completed on February 6, 2024, and the results came back in normal range. You are due to have your water tested before February 2027. RATED LICENSE *DCDEE is transitioning to a new Rated License process referred to as Pathways to the Stars. Bright Horizons at West Cary Clubhouse operates with a Five (5) Star Child Care License issued on April 24, 2023. Director, Mr. Leslie Holt and Assistant Director, Ms. Kezia Holder-Cozart were present at the facility today and accompanied me throughout my visit. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, fire and sanitation inspections and playground inspections. All reports were found to be in compliance. CORPORATION STATUS Your current corporation status is listed as “Current-Active” on the North Carolina Secretary of State Website. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. Staff qualifications were discussed and reviewed. OBSERVATIONS I completed a walk-through of the facility. At the time of the visit, there were a total of 78 children present between the ages of 0yrs-10yrs. The children were observed engaged in outdoor and indoor activities, having lunch, and going down for nap. The staff were observed engaged in the children’s activities while providing appropriate supervision. Three violations were cited during today’s visit. Violation Number Comment Rule 871 Center staff did not comply with the safe sleep policy. Staff were documenting that infants were placed on their side and tummy when going down for a nap and no Written Waiver was completed and on file for review. 10A NCAC 09 .0606(a) 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. One new staff member did not complete First Aide in the required 90 day time frame. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file. One new staff member did not complete CPR training in the required 90 day time frame. .1102(d) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. Prior to today’s visit, the compliance history score for the center was 95% During today's visit I had the opportunity to speak with the facility director about the struggles some facilities are facing in hiring educated childcare professionals. We discussed the importance of focusing on finding qualified staff which might require pausing facility enrollment to get new staff in and trained before introducing new children into the program. CONSULTATION & REMINDERS *During today’s visit I observed safe sleep documentation that was not being completed correctly. I discussed with the infant staff the importance of always documenting that infants are placed on their back when being placed in their crib for nap time, unless there is a Written Waiver on file, signed by the child’s parent/legal guardian. This includes if the infant rolls onto their tummy or side when being placed down for nap. By documenting tummy or side when the infant is being placed in the crib for the first time, the care giver is stating they purposefully placed the infant on their tummy or side when placing them in the crib for nap time. Sleep checks should tell a clear story of how the infant was initially placed in the crib when going down for nap and the time the infant went down for nap, each position the infant was observed in during each sleep check conducted by the infant room staff, and when the infant woke up. I suggested that if an infant does not nap on any given day, that the infant room staff document on their safe sleep chart, NO NAP, on that specific day or if an infant is absent on any given day that the staff record absent on the day. *Staff files: Record keeping is one of the most time-consuming tasks associated with administering a childcare program. It is an important part of licensure. Review staff files often to ensure the program maintains accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. We discussed making sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. Administrative staff must allow time to review all necessary documentation prior to employment of staff to ensure accuracy. As an administrator, it is your responsibility to review all employment documentation and verify that the required documents are received within the specified time frame. Using a checklist to check and date receipt of documents will help you track employment documents. A staff file checklist is available on the DCDEE website under the Provider tab and provider documents link. During today’s visit one new staff member was found to not have her required CPR and First Aide certification within the required 90-day time frame. Please refer to the Staff and Training Worksheets to continue to keep all staff members current with their upcoming due dates for CPR/First Aid training, child maltreatment training, and health and safety training requirements. *Medical Action Plans - The maintenance of these plans and the ability to implement such plans for the treatment of reactions are essential. It is important to remember that a medical action plan must be attached to the child’s application for enrollment, completed by the child's parent or a health care professional and must be updated on an annual basis. Permission to administer emergency medications must be updated every 6 months by the parent. I suggested to the director and assistant director that they review the names of all enrolled children who have emergency medications at every staff meeting to provide a constant reminder to everyone in the building. *This facility has a school age program and provides transportation. Motor vehicle crashes are the leading cause of death in children two to fourteen years of age in the United States. It is necessary for the safety of children to require that the caregiver/teacher comply with requirements governing the transportation of children in care, in the absence of the parent/guardian. As a reminder, each child transported must have emergency contact information on file along with a photograph of each child. This is helpful in identifying children in the event of an emergency/accident. RATED LICENSE UPDATE The Third editions of the ECERS, ITERS, and FCCERS are now being used for official assessments; these scales all have a spiral binding on the top. More information about these versions is posted on the NCRLAP and DCDEE websites. I recommend you begin becoming familiar with the new measures so you can properly prepare for future assessments. I recommended the administrative team reach out to Smart Start to prepare for the upcoming Rated License process. A flier with the challenging behaviors hotline information was provided today. *Background Checks – I shared trainings and information guides with Mr. Holt and Ms. Holder-Cozart regarding linking staff in the new Automated Background Check Management System (ABCMS) to Bright Horizons @ West Cary Clubhouse. Linking Criminal Background Checks – Staff need to log into the Criminal Background Check Portal on the DCDEE website and link to the facility license number. It is important to do this so staff at DCDEE can contact the center if a random criminal history check uncovers a staff member with a criminal charge or conviction working in a child care facility. It is also a way to for staff at the CBC Unit to keep you updated of the status of out-of-state checks if a new employee has a Provisional Qualification. Bright Horizons at West Cary Clubhouse operates with a Five (5) Star Child Care License issued on April 24, 2023. Director, Mr. Leslie Holt and Assistant Director, Ms. Kezia Holder-Cozart were present at the facility today and accompanied me throughout my visit. INSPECTIONS I reviewed required inspections including fire drills, emergency drills, fire and sanitation inspections and playground inspections. All reports were found to be in compliance. CORPORATION STATUS Your current corporation status is listed as “Current-Active” on the North Carolina Secretary of State Website. MONITORING During today’s visit, a partial assessment was conducted, monitoring all space occupied by the children. Staff qualifications were discussed and reviewed. Six new staff files were reviewed today. All spaces were monitored for supervision, staff/child ratios, staff/child interactions, materials, equipment and required postings. Upon arrival, I was greeted by the director, Ms. Angela Whitted. Ms. Whitted accompanied me on my walk-through of the facility today. At the time of my visit there were a total of 73 children present. The children were observed engaged in circle time and centers, playing outside, and eating lunch. I observed one infant room today with three infants present. Throughout the center the staff were observed engaged in the children’s activities and providing appropriate supervision. LICENSE STATUS Currently this center operates with a Five Star license. The restrictions on the license were updated on April 24, 2023. Restrictions are Daytime care only, Meets enhanced ratios and Meets enhanced space. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire, and outdoor inspections. The required annual fire inspection is currently past due. MONITORING During today’s visit, a full assessment was conducted monitoring all spaces occupied by the children. All staff files were reviewed. A selection of children’s files was reviewed. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements I monitored today and given to the director at the conclusion of today’s visit. The following violations were observed during today’s visit. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection at this facility was on 6/1/2022. 10A NCAC 09 .0304(a) 533 Breast milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Two bottles and two containers of food were observed not dated in the refrigerator in the infant room today. 15A NCAC 18A .2804(d) 1921 Staff working in part-time, full-day or track out school age care programs required to complete BSAC training, did not complete it within three months of employment. The director and assistant director have not completed the required BSAC training. There are two other staff that assist when needed in the school age room who have not completed the required BSAC training. .2510(j) CONSULTATION/NOTES To improve the quality of your program, we discussed the following items: • During my walk-through of the facility the director and I observed the children eating in some of the classrooms. We discussed the importance of the staff always being within arm’s reach of the children during snack time and mealtime. • During today’s visit I found the automated safe sleep check to not be recording the position the babies were placed in the crib when first being put down for a nap. The director instructed the teacher to immediately begin using the paper safe sleep charts until further notice. • There are a lot of wood boarders and structures constructed on the playgrounds. I reminded both the director and assistant director to keep an eye on the wood as some pieces are starting to weather. The play areas had good ground cover. I reminded the staff to continue to fluff the mulch each day, especially after it rains. The director stated the school age playground is currently closed due to concerns with the wood play structure. • The facility has three 15 passenger buses. The director stated the bus with license plate KX 7784 is not in use right now as they are waiting for the vehicle registration. • During my discussion with the director today, regarding transportation, I reminded her that all children who are under eight (8) years old and weigh less than 80lbs., must use an approved age and weight appropriate car seat at all times. Rated License/COHORT 2 Information Bright Horizons Clubhouse At West Cary is in COHORT 2. Prior to today’s visit I emailed resources you could think about using as you begin preparing for your Rated License and Environmental Rating Scale visit. As discussed today, it is very important that all your staff create a WORKS account as soon as possible, if they have not already done so, and begin submitting any education that is not already documented in their WORKS account and continue to do so as they complete additional semester credit hours or degrees. DCDEE in collaboration with other agencies have started a challenging behavior hotline. To Speak to a Behavior Support Advisor for advice and resources specific to the challenging behaviors in your classroom please call 1-888-600-1685 and choose Option 1.
