    Name of Operation: BAMBINO'S PLAYSCHOOL Facility ID: 92003437 Consultant: KIMBERLY GIROUARD Operation Type: Center Case Number: Visit Date: 11/20/2023 Number Present: 41 Completed Date: 11/20/2023 Age: From 0 To 5 Total Minutes: 315 Time In: 10:30 AM Time Out: 03:45 PM Time In: Time Out: List to Use: Center Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance during an annual compliance visit. Upon arrival, I was greeted by the director, Ms. Adria Boyd. Ms. Boyd accompanied me on my walk-through of the facility. At the time of my visit there were a total of 41 children present between the ages of 0-5 years old. The children were observed engaged in indoor and outdoor play, participating in circle time, eating lunch, and going down for nap. One infant room was monitored today. There were two (2) infants present today with two staff. Both infants were observed engaged in floor activities with the staff members. All the staff were observed engaged in the children’s activities while providing appropriate supervision. LICENSE STATUS Currently this center operates with a Five Star license issued May 4, 2017. INSPECTIONS I reviewed all required inspections including fire drills, emergency drills, sanitation, fire, and outdoor inspections. All inspections were found to be in compliance today. MONITORING During today’s visit, a full assessment was conducted, monitoring all indoor and outdoor space occupied by the children. Staff files were monitored today. A selection of children’s files was monitored. All spaces were monitored for supervision, staff/child ratios, materials, equipment and required postings. A checklist was used to note the requirements that were monitored today. The checklist will be provided at the conclusion of the visit. Three violations were cited during today’s visit. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. The protective barrier on the first step of the large play structure on the big playground has fallen off leaving a metal edge exposed. 10A NCAC 09 .0601(a) 828 Air conditioning units were accessible or did not have a guard to keep objects from being thrown into the unit. The AC unit on the big playground does have the protective barrier but one of the slats is missing from the wooden fence around the AC unit leaving a space a child could fit through. .0604 (m) 1867 The depth of the loose surfacing was not based on critical height of the equipment. Mulch did not measure the required 6 inches deep under the monkey bars and play structure on the 3-5 year old playground. .0605(k)(1-4) COMPLIANCE HISTORY According to NC General Statute all Child Care programs must maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Any violations documented during visits to your program may have an impact on the total compliance history score and cause your score to drop below the mandated level. **Prior to today’s visit, the compliance history score for the center was 99%. TECHNICAL ASSISTANCE 1) I suggested to the director that she provide one locked cabinet for staff to use if necessary. 2) I suggested staff store brooms and dustpans away and out of the reach of the children. 3) As staff conduct classroom safety checks, remind them to remember to keep an eye out for small crayon and chalk pieces in buckets that are accessible to the children. CONSULTATION RATED LICENSE Reminders/Suggestions/To Do List Bambino’s Playschool is in Cohort 1. I recommended to the director that she consider completing the following tasks and activities that will help prepare her staff for the upcoming rated license assessment, which will be sometime between July 1, 2024, and June 30, 2025. *Have all staff update WORKS (DCDEE-WORKS) accounts to reflect all currently completed coursework. I will begin to review your staff’s education that is in WORKS in early February 2024. We will only be able to use the education that is documented in the WORKS system so please make sure all your staff have their WORKS accounts updated by January 2024. *Request technical assistance with your childcare consultant and local partners (Wake County Smart Start - https://www.wakesmartstart.org and NCRLAP - www.NCRLAP.org) and visit the following website https://ncchildcare.ncdhhs.gov/Provider/Licensing/Star-Rated-License/Resuming-Star-Rated-License for additional resources and important timelines. *Consider requesting an ERS assessment (free of charge) during your preparation year. Remember the scores can be used in a variety of ways: ERS assessments can be requested during the preparation year. You can request an “Outreach” assessment, but you will not be able to use any scores from an Outreach assessment towards your Rated License. You can also request a “Prep Year” assessment which you can choose to use the scores towards your rated license. With both of these assessments, your scores can be used as guidance to enhance program quality. If desired, Prep Year assessment scores can be used during the preparation year if you decide you want to move forward with a rated license reassessment. Assessment scores from an assessment completed during your prep year can also be saved to use during the reassessment year. Facilities can request another ERS assessment (free of charge) in your reassessment year for any ERS assessment scoring less than a 5.0. I left additional detailed information with the director regarding the rated license process. *Participate in local CCR&R and NCPC quality related training/workshops (CCRR Information) *Review the NCRLAP website ERS resources (www.NCRLAP.org). To help review ERS requirements and think carefully about the current characteristics and practices in your program and classrooms, check out the Thinking More worksheets. A worksheet is available for each subscale of each rating scale. *I suggested the director create a rated license timeline for her program starting with the month and year the facility plans to complete their rated license and work backwards, listing deadlines with associated dates for each required “to do” action item. I asked Ms. Boyd to keep me updated on how she plans to move forward with the rated license assessment. If Ms. Boyd plans to work with Wake County Smart Start to prepare for the facilities ERS visit, Ms. Boyd should reach out to them as soon as possible to get on their calendar. I reminded the Ms. Boyd that she will only be able to use the education that is documented in each staff member’s personal WORKS account at the time of their rated license assessment. IT IS VERY IMPORTANT THAT EACH STAFF KEEP THEIR PERSONAL WORKS ACCOUNT UP TO DATE. Please submit a corrective action letter for the above violations. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. Your letter is due no later than December 4, 2023, and should be emailed to Kimberly Girouard at Kimberly.Girouard@dhhs.nc.gov .
