A full assessment of childcare requirements was conducted today. Your program currently operates with a Five-star license. Your most recent license was issued on August 7, 2018, for 225 children on first shift and you are approved to care for children 0-5 years of age. Your facility meets enhanced ratios and enhanced space. Today’s visit was conducted with you, Ms. Melissa Brown, Center Director. You accompanied me throughout my walkthrough of the facility today. I, Kimberly Girouard, Child Care Licensing Consultant, monitored thirteen (13) classrooms, the gym, the kitchen, three (3) playgrounds, staff/child ratios, supervision, capacity, group size and staff interaction and discipline. I also monitored the facility for health and safety requirements. At the time of my visit there were a total of 158 children present between the ages of 0yrs. – 5yrs. years of age. The children were observed engaged in indoor and outdoor play, participating in circle time, eating lunch, and going down for nap. Three infant rooms were monitored today. The infants were observed engaged in floor activities and going down for nap. During my walkthrough of the facility today, I observed the staff engaged in the children’s activities while providing appropriate supervision. I monitored three playgrounds today. Each offered age-appropriate equipment that was observed to be in good repair and offered a pleasant and age-appropriate outdoor environment. This facility does not provide transportation. VERIFICATION OF RECORDS: *Last fire inspection= 7/17/2025 *Sanitation inspection=7/31/2025- 5 demerits and a superior rating. *Today, November 4, 2025, I reviewed the NC Secretary of State’s website and observed All Starz Cary INC, LLC listed as current and active. * Compliance history as of today’s visit 99% * EPR plan revised 12/2024 * Fire drill- 10/28/2025 @ 9:43 am and took2 minutes and 7 seconds * Lockdown drill- 10/9/2025 @ 3:24pm and took 1 minute and 48 seconds * Playground inspection- 10/10/2025 Two violations were cited during today’s visit. Violation Number Comment Rule 808 The child care center premises, including the outdoor learning environment, was not clean, drained to minimize standing water, free of litter and hazards, and/or maintained in a manner which does not create conditions that attract or harbor pests. The cover to the water table had pooling water on the top and the plastic ben that housed the water play toys also had pooling water on top. The teachers dumped the water when asked to do so by their facility Director. 15A NCAC 18A .2832(a) 871 Center staff did not comply with the safe sleep policy. Some of the safe sleep checks monitored today were observed to have "T" for tummy as the first position infants were placed in when going down for nap and there was no "Sleep Waiver" on file. 10A NCAC 09 .0606(a) Technical assistance *Today we discussed the importance of always documenting that infants are placed on their back when being placed in their crib for nap time, unless there is a Written Waiver on file, signed by the child’s parent/legal guardian. This includes if the infant rolls onto their tummy or side when being placed down for nap. By documenting tummy or back on the infant’s safe sleep check, the care giver is stating they purposefully placed the infant on their tummy or side when placing them in the crib for nap time. Sleep checks should tell a clear story of how the infant was initially placed in the crib when going down for nap and the time the infant went down for nap, each position the infant was observed in during each sleep check conducted by the infant room staff, and when the infant woke up. I recommend that if an infant does not nap on any given day, that the infant room staff document on their safe sleep chart, NO NAP, on that specific day. *During the walk through of the outdoor playgrounds today, I observed the sand table to have standing water on the cover and standing water on the top of a large plastic storage bin that housed toys for the water table. I recommend the administrative team conduct playground checks early in the morning before children participate in outdoor play and throughout the day to confirm no standing water is accessible to the children during child care hours. DCDEE has transitioned to a new Rated License process referred to as Pathways to the Stars. Wake County childcare consultants offered informational sessions on September 30, 2025, for all Wake County center and FCCH providers. I suggested Ms. Brown begin to become familiar with the new QRIS system to decide which Pathway they will want to follow. This new rated license/ star rating process is in affect now. Today Ms. Brown felt they would follow Pathway 2. I also suggested Ms. Brown complete the rule rollout training in Moodle to acquire a good understanding of the new rules and rated license information process. Based on two violations of childcare requirements cited today, a letter of correction is required by November 18, 2025. Please send via email to the address listed below. Katie Schmidt, Curriculum Director, accompanied me on a walk-through of the center. The Director, Melissa Brown, is on leave until Monday, September 15, 2025. Limited monitoring of child care requirements occurred during today’s visit. Staff/child ratios, space capacity and safe environment were monitored and observed to be in compliance. The allegation is that a child was not attended to in nurturing or appropriate manner. I discussed with Ms. Schmidt that in June/July 2025 timeframe, there were concerns that a staff member in the two-year-old classroom inappropriately touched their child. Ms. Schmidt stated that Administration were made aware of the allegation by the child’s parents and they interviewed the staff member and discussed the allegations and that the staff member denied any actions of inappropriate behavior towards a child. Ms. Schmidt stated that she was not aware if additional investigation was conducted by Administration such as viewing video footage. The center uses a “Watch Me Grow” live video footage system but Ms. Schmidt stated that she was unable to gain access for me to view dates for June/July 2025 due to the fact that the footage erases after a certain period of time. During my interview with Ms. Schmidt, I requested files for any children who are in any of the two-year-old classrooms who had been terminated for the timeframe June 2025 to September 2025. Ms. Schmidt initially stated that there were no children from the two-year-old classrooms which had been terminated but there had been some who withdrew from the center. I reviewed the children’s files and there were five children who withdrew from the center. Documentation of the paperwork in each child’s file indicated children were withdrawn for various reasons. One of the children’s files indicated the child was withdrawn for “curriculum, safety and communication” concerns. One child did not have any documentation in the file indicating the reason for withdrawal. It was also observed that this same child attended the center for over 30 days but did not have a health assessment on file. Ms. Schmidt and I discussed if there was any documentation on file for either withdrawal or termination for this child. Ms. Schmidt was unable to locate any paperwork documenting this. Ms. Schmidt further clarified that the child was no longer attending the center due to “agreement to mutually separate from the center.” Additionally, Ms. Schmidt submitted to me typed documentation on file dated 7-7-25 which discussed that a child’s parent spoke to Melissa Brown, Director indicating concerns with their child’s teacher. Documentation indicated that the child’s parents accused the staff member of inappropriate actions and the child’s parents requested that their child be moved to another classroom. On Wednesday, July 9, 2025, it is documented that Ms. Brown called the child’s father to explain that after investigation of the allegations, the decision that Administration came to as a result of the allegations is that they would “part ways due to aggressive accusations made against the staff member.” Ms. Brown documented that the child’s father “was fine with the solution and that he wanted to know the final decision.” Based on page 3 of the Policies and Procedures Parent Guide under the Dismissal Policy: On rare occasions, All Starz may find that our program is not meeting the needs of a particular child. In those instances, All Starz reserves the right to ask parents to make other arrangements for childcare. It is observed that the center disenrolling the child was within the procedures as stated in the parent handbook. During today’s visit, I also interviewed the staff member who was accused of the allegations as well as the Floater who was working in the classroom currently due to the fact the one of the previous staff members is no longer employed with the center. Staff member #1 was interviewed and discussed that the child had difficulty with transitions. Staff stated that the child cried a lot and would occasionally throw themself down on floor during times of distress. Staff #1 stated that nap times appeared to be the hardest times of the day for the child. Staff would hold the child to comfort the child. I discussed with staff #1 what strategies were being used to assist the child with the difficult times of the day and staff #1 stated that they allowed the child to hold a stuffed animal bunny during the day as well as staff would hold the child often during the day. Staff #1 stated they attempted to comfort the child by holding the child during difficult times and would pat the child’s back on top of the child’s shirt during nap time. Staff #1 stated they did not view video footage as part of the investigation with Administration. Staff member #2 indicated during the interview that they have not observed any episodes of inappropriate behavior between staff and children. Staff #2 did additionally indicate that they feel that staff #1 does get emotionally attached to the children. Staff #2 stated that staff #1 does get “loud” in terms of verbal tone at times during the day but they do not feel that an inappropriate tone is being used and that they believe sometimes people have a louder voice than others. Based on observations and interviews of staff, and that no video footage was available for review I am unable to confirm the allegations. Therefore, the allegation is unsubstantiated. We discussed that based on GS 110-91 (1) the center did not report a suspected case of child abuse or neglect as required by mandatory duty of reporting as prescribed in GS 7B-301. I verified with Kim Girouard, Child Care Consultant for the center that she has not received any communication from staff employed at the center that a suspected case of child abuse or neglect based on communication with child’s parents. This is a violation of child care requirements. Reporting allegations of abuse, neglect or maltreatment is not optional. Allegations must be reported to your Child Care Consultant/Division of Child Development and Early Education so that proper investigations can occur. I encourage you to use the resources available to you when conducting your own investigations and/or when randomly monitoring classrooms. This facility has video cameras that can be viewed to monitor classroom activities. The timeframe to view saved footage is limited and I was unable to review footage from the time frame indicated in the report. Staff stated footage was not reviewed during the investigation conducted by the director. This was a missed opportunity that could have indicated an incident/s did or did not occur. Two violations of child care requirements were observed. A corrective action letter stating how each violation is corrected is due to me no later than September 25, 2025, and can be mailed or e-mailed to me at the address at the bottom of this report. Violation Number Comment Rule 1949 The center did not report a suspected case of child abuse or neglect as required by the mandatory duty prescribed in G.S. 7B-301. The center failed to report a suspected case of abuse and neglect when the parents of a two-year-old child enrolled at the center expressed their complaint to Center Administration in July 2025.
