A full assessment of childcare requirements was conducted today. Your program currently operates with a Four-star license. You received four stars on October 16, 2024. You are licensed for 45 children on first and second shift. You are approved to care for children 1-6 years of age. Your facility meets enhanced ratios and enhanced space. When I entered the building, I observed your childcare license posted in a visible location. Today’s visit was conducted with you, Ms. Alfreya Munroe, Center Director. You accompanied me throughout my walk-through of the facility today. I, Kimberly Girouard, Child Care Licensing Consultant, monitored four classrooms, the kitchen, one (1) playground, staff/child ratios, supervision, capacity, group size, staff interaction and discipline, required trainings, criminal background checks, emergency medical care plan, administration of medications, storage of substances and medications, health and safety requirements, adequate and approved space, program records, and permit restrictions. At the time of my visit there were a total of 18 children present between the ages of 1yrs. – 5yrs. years of age. The children were observed engaged in indoor center activities and outdoor play. While inside I observed the children participating in circle time, engaged in art activities, engaged in center play, eating lunch, and going down for naps. While conducting my walk-through of the facility today, I observed the staff engaged in the children’s activities while providing appropriate supervision. The playground offers a pleasant age-appropriate outdoor environment. Alfreya’s Playground uses Pre-K On My Way for their curriculum and child assessments. VERIFICATION OF RECORDS: *Last fire inspection – 2/16/26 *Sanitation inspection –6/9/25 (9 demerits and a superior rating) *I reviewed the NC Secretary of State’s website and observed the owner of this facility, Alfreya’s Play Ground LLC to be listed as current and active. * EPR plan was not completed as required, violation was cited * Fire drill – 2/9/26 * Lockdown drill – 12/4/25 * Playground inspections were found to be in compliance. Four violations were cited during today’s visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Electrical outlets were observed not covered in classroom 2 today. 10A NCAC 09 .0604(c) 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. Plastic bags were observed in an unlocked drawer in the 1 year old classroom that was accessible to the children. .0604(q) 1812 The center did not complete an EPR Plan within four months of completing the EPR in Child Care training and/or the Plan was not completed on a template provided by the Division. The director of the facility completed the EPR training on April 22, 2025 but did not complete the plan within the the required four months from completing the class. .0607(c) 1867 The depth of the loose surfacing was not based on critical height of the equipment. The pink climbing structure is over three feet tall and was not placed over the required loose surfacing. .0605(k)(1-4) RATED LICENSE: Alfreya’s Playground will be due to renew their rated license in October 2027. DCDEE has transitioned to a new Rated License process referred to as Pathways to the Stars and is in effect now. During today’s visit Ms. Alfreya Munroe and I discussed Pathway 1 and Pathway 2. Ms. Munroe is interested in following Pathway 2. I shared with Ms. Munroe where she can find information regarding our new QRIS Pathway to the Stars on the DCDEE website. TECHNICAL ASSISTANCE Four violations were cited today; plastic bags were accessible to children under the age of three, an EPR plan was not created in the required time frame, outlets were observed not covered and the required ground cover and fall zone was not provided under one of the play structures. Suggestions shared today: 1) I suggested Ms. Munroe confirm all of her staff are currently in WORKS and all of their education is up to date in the system. 2) Surfacing - The number one cause of injury on playgrounds is falls to the surface. Maintaining the correct depth of protective surfacing material is crucial for safety. The outdoor learning environment offers a sense of freedom for children. Children can play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment. 3) Approximately 2,400 children are injured annually by inserting objects into the slots of electrical outlets. For this reason, all electrical outlets, including those on power strips, not in use that are in areas accessible to children must be protected with a safety plug or should be classified as a tamper-resistant electrical outlet. Consider adding a note near the outlets to serve as a reminder to re-cover the outlets. Review this with teachers during orientation as part of the observation of daily routines. Hold a staff meeting to review the different ways you all can work together to maintain compliance with childcare requirements regarding outlet covers. 4) Plastic bags are a suffocation hazard for young children and should be made inaccessible to children less than three years of age. Any plastic bags must be stored out of reach of the children, which is five feet or higher from the finished floor. Keep trash bags tucked so large sections are not accessible to the children. 5) The EPR plan is a safety plan that will help keep the children/staff safe. Following completion of the EPR in Child Care training, the trained staff member of the center, or the operator(s) of the family child care home, shall develop the EPR Plan within (4) four months. The EPR Plan addresses how a child care center, or a family child care home will respond to both natural and man-made disasters; to ensure the safety and protection of the children and staff. Natural and man-made disasters include but are not limited to the following: fire, tornado, flood, power failures, chemical spills, bomb threats, earthquakes, blizzards, nuclear disasters, or a dangerous person(s) in the vicinity. Trained staff are required to review the EPR Plan with staff during orientation and annually. Substitutes and volunteers in ratio and additional caregivers who are present must be informed of the EPR Plan and its location. For questions related to the EPR Plan content, contact your EPR in Child Care trainer or Child Care Licensing Consultant. You may also contact the Division of Child Development and Early Education (DCDEE) at 1-800-859-0829 (out of state) or 919-814-6365 (in state) to request assistance from the Training and Program Development Consultant. In collaboration with NC Childcare Health and Safety Resource Center, and DCDEE, the NC Emergency Management developed a standard EPR template for use (statewide) by licensed childcare programs. The content for this template aligns with the EPR in Childcare training and the EPR rules. The Childcare Rules state that programs must use the EPR template provided by the Division which is available from the NC Risk Management Portal. which houses emergency planning tools for a variety of hazards. 6) I reminded the staff today of the importance of hand washing before and after changing diapers to maintain a safe, healthy environment. 7) I discussed with Ms. Munroe the importance of creating a system to confirm her children, staff and program files and required documentation remain current and up to date at all times. I suggested Ms. Munroe set aside time daily to complete these tasks so they do not become overwhelming and fall out of compliance. CORRECTIVE ACTION LETTER Please submit a corrective action letter for the above violation. While the Division allows you time to explain how you have corrected violations when they are documented, it is expected that you will correct all violations immediately. Your corrective action letter must include the following: Name of your program, Id number of your program, Date of Letter, Violation(s) number(s), and How and When each violation was corrected. The last annual compliance visit was conducted on 3/21/2025. You, Afreya Munroe, Center Administrator, assisted me today. Your program currently operates with a Four-Star license. You accompanied me as I walked through the facility today. Older children were participating in a group time activity. Two groups of older children were outside playing. Children were freely moving around the outdoor space, interacting with each other and staff, and using portable materials. Younger children later transitioned indoors and completed handwashing and started eating lunch. I observed staff completing handwashing after a child was diapered. The staff member was also diapered. One group of younger children listened to a book being read about animal sounds. Each child sat while holding different books in their hands. They also sang different songs and danced while singing. Another group participated in an activity where their height was measured on a large paper that was attached to the wall. Each child walked to the teacher, and she marked the child’s height. Throughout the classrooms and activities conducted today showed that staff were implementing the lesson plan. Supervision, group size, permit capacity, staff/child ratios and space capacity complied. Two new staff’s files were monitored. Other staff’s qualifying letters and special training complied. Children’s records were not monitored. Program records were monitored, and one violation was cited. Fire and sanitation inspections were current. Emergency medical care plan was accessible. ADDITIONAL REQUIREMENTS: General health requirements were monitored. One violation regarding storage of medication was cited. The facility continues to meet requirements for adequate and approved space. The NC Secretary of State website was reviewed, and the Owner of this facility is listed as current- active. Violation Number Comment Rule 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Diaper cream was stored in drawer under the diaper changing table. 15A NCAC 18A .2820(d) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A shelter-in-place or lockdown drill was last conducted on January 12, 2025 and not every three months. .0604(u);.0302(d)(8) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Practicing shelter-in-place and lockdown drills ensures staff and children know what to do in case of a true emergency. You explained you thought the drills were to be conducted every six months. We reviewed the form and observed it must be completed every three months. You explained you have made the notation in your records as a reminder. This facility was issued a temporary license effective April 15, 2024 through October 15, 2024 to care for 29 children ages 1-6 years old. The facility is approved to operate on first and second shifts. I verified that the temporary license, NC Summary of the law, safe arrival and departure procedures, emergency medical care plan, and emergency telephone numbers were posted. The menu was posted. Some of the children bring their lunches and some children eat lunch at the center. Two classrooms were open during today’s visit. Both groups of children were outside playing when I arrived. The children were observed washing their hands after returning inside from outdoor play. They ate lunch, washed hands and had rest time. The activity plan, daily schedule, staff/child ratios, allergies were posted in each classroom. No concerns with staff/child ratios, group sizes, or capacity during the visit. However, due to lack of staff, there could be issues if the one support staff that is present now, isn’t present later or leaves. Coverage will be needed for restroom breaks and lunch breaks in order to maintain supervision and staff/child ratios. There was one child that was one year old with children that were two and three years old. There was one three year old in this group. The director stated that the child was not potty trained. Since all the children were three or younger, this is acceptable. One teacher was not present during today’s visit, so the owner/director was the teacher for the preschool age children and another teacher was with the younger children. The staff bathroom had a can of Febreeze on the paper towel dispenser. The door was not locked. If the door is locked when not in use, it is acceptable to leave the aerosol can of Febreeze where it was. If the door is unlocked, the aerosol can of febreeze must be locked. The Febreeze was moved to the office area during the visit. A violation was not cited for this due to the room not being used, the classroom door was closed and the bathroom door was closed. Place any type of aerosol can, and all cleaning products in locked storage. The cabinet in the kitchen was used to store cleaning products and was locked during the visit. Be sure that it stays locked when no one is in the kitchen. Keep the kitchen doors locked when no adult is in the kitchen. You stated that you did not complete a fire drill during the first month of operation. The last fire drill was conducted in March 2024. Documentation of fire drills and emergency drills under the previous ownership had “voice” written under method of notification on the emergency drill form. It is not acceptable to use a voice command for the fire drill. The alarm should be pulled so the children know what to do when they hear the sound of the alarm. The fire alarm system should be monitored and connected to the fire department so they will come when the alarm is activated. You did not complete an outdoor play area inspection during the first month of operation. I reviewed the outdoor play area monthly inspection form with you during today’s visit. Daily checks of the outdoor play area should be conducted prior to children going outside to play. Empty water out of equipment and toys and verify that there are no safety concerns before the children come out to play. Since there is a wooded area around the outdoor play area, there could be snakes present. I monitored the outdoor play area today and emptied water out of the green see saw and the dramatic play piece of equipment. Surfacing was found to meet requirements. Four staff files were reviewed during today’s visit. One teacher did not have a criminal background check qualifying letter on file. She will be leaving the center at 1:00pm. One teacher did not have a medical or TB test on file. The medical and TB test is required prior to the first day of work. The medical, TB test and health questionnaire must be stored separately from other information in the staff file. I reviewed three children’s records during today’s visit. A copy of the NC Summary of the Law was in each file but a statement that the parent received it was not. A violation was not cited since it was included in the parent handbook and the parent handbook had a sign off page that it was received. I emailed you a copy of a parent sign off page that included a statement of receipt for the NC Summary of the Law. The permission to be outside the fenced area was not on file. The statement giving permission to be outside the fenced in area was included on the parent sign off page. In case there is a fire out back, the children will need to go out front, which is not fenced in. You may have a community service day with visiting fire trucks, this will allow children to participate. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill was not conducted during the first month of operation. .0604(t); .0302(d)(5) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. An outdoor play area inspection was not completed during the first month of operation. .0605(q) 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One teacher did not have a medical on file on or before the first day of work. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Keep cabinets and doors locked. A hazardous items list was posted in the kitchen for reference. This is also reviewed during orientation. Reminder: Building, fire and sanitation inspections are due one at least one month for the temporary license expires. Do not wait to request the inspections. You stated that the fire inspector filled out the form. The violations cited during today’s visit must be corrected immediately. Email or mail me a corrective action letter stating the violation, when the violation was corrected, how the violation was corrected and how you plan to prevent repeat violations in the future. G.S. 110-90.2(b) Technical assistance: Outdoor Inspections- The outdoor learning environment offers a sense of freedom for children. Children are able to play freely with peers, expand their imagination beyond restraints of indoor activities, release energy and explore their sense of touch, smell, taste and their sense of motion. Since children are exploring it is very important to provide a safe environment, free of debris. A designated staff member should verify daily that the area is safe for children to use. Each month, the outdoor play areas should be inspected and documented on the form provided by the Division of Child Development. Storage of Hazardous Items- Intentional planning of the environment ensures a safe environment has been created which prevents and reduces injuries to young children. Keep cabinets and doors locked. A hazardous items list was posted in the kitchen for reference. This is also reviewed during orientation. Reminder: Building, fire and sanitation inspections are due one at least one month for the temporary license expires. Do not wait to request the inspections. You stated that the fire inspector filled out the form. The violations cited during today’s visit must be corrected immediately.
