    Name of Operation: CARTHAGE ELEMENTARY PRESCHOOL Facility ID: 63000453 Consultant: DEBORAH DAVIS Operation Type: Center Case Number: Visit Date: 10/23/2025 Number Present: 16 Completed Date: 10/23/2025 Age: From 3 To 5 Total Minutes: 90 Time In: 09:30 AM Time Out: 11:00 AM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor your program for compliance with applicable child care requirements for a routine unannounced visit. Mrs. Kraft, Lead Teacher, assisted me with the visit. Janet Edwards, Licensing Supervisor, accompanied me on the visit. Your program currently operates with a five star license, effective date February 27, 2019. Restrictions are capacity of eighteen children, ages 3 to 5, daytime care only, meets enhanced space and ratios minus one. The center's compliance history was reviewed with the operator. The program’s compliance history was one hundred percent as of October 23, 2025. The five star license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. The children were observed having free play in centers, teacher directed art activities, group time, toileting and preparing for lunch. Lunch consisted of Cheesy penne, lima beans, broccoli, banana, raisins, texas toast and milk. Supervision and staff interaction were adequate. There were two violations documented today and discussed with Mrs. Kraft. Violation Number Comment Rule 807 A safe indoor and outdoor environment was not provided for the children. An opening in the side of the building beside the playground was accessible to children. 10A NCAC 09 .0601(a) 1755 Certified developmental day center staff/child ratios and group sizes were not met. Sixteen children were present during the visit. The lead teacher walked out of the classroom leaving two staff with sixteen children, ages three to five. Later a staff person left to get lunch from the cafeteria leaving two staff with sixteen children. Staff/child ratios for certified developmental day is one staff to six children. .2902 (f) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 6, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware any information submitted by you is legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. Due to the staff/child ratio violation, a follow up visit is required. Davidson, Moore County Schools and Partners for Children NC Pre-K representatives, on October 15, 2025, to discuss the new QRIS Pathways to the Stars. The Program Assessment Pathway was chosen for all facilities. We discussed the Pathway chosen, Application for Assessment, Family and Community Engagement/Foundational Practices, CQI Plan for Facilities/Individuals and staff education information. The QRIS conversation template was completed during the meeting. The following resources were suggested and discussed- Contact Moore County Partners for Children and Family or The NC Rated License Assessment Project (NCRLAP) to schedule mock ECERS-3 assessments for Fall/Winter 2025.
