Your program currently operates with a three-star license, issued July 25, 2022, earning 5 points in the education component, 2 points in the program standards component and 1 quality point for all lead teachers having at least 5 years of EC work experience in no more than two different centers. The last annual compliance visit was conducted November 25, 2025. The sanitation inspection was completed November 12, 2025, with a “Superior” classification. The last fire inspection was conducted April 18, 2025, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of November 14, 2025. During today’s visit, I observed children in the indoor learning environments. Children were participating in general personal care routines and free play activities. The teacher was seen walking around the room engaging with children during play. I found supervision, ratios, and interactions to be in compliance. The following were also inspected during today’s visit: program requirements, equipment, furnishings, staff/child ratios, supervision, medication, medication administration, outdoor learning environments, and program records including staff records and children records. The following was cited during today’s inspection. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The director did not update their health questionnaire annually. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The owner did not update their emergency information annually. .0701(a) 9995 A violation was found for which there is no item number. 15A NCAC 18A .2825 (a) In child care centers, the walls and ceilings, including doors and windows, of all rooms and areas shall be kept clean, free of visible fungal growth, and in good repair. All walls and ceilings shall be free of peeling, flaking, chalking, or otherwise deteriorating paint. The center has multiple cobb webs and water marks on the ceiling leading out the back emergency exit. Please clean the walls and ceiling from cobb webs. Please ensure the water marks on the ceiling in the back hallway are sealed to help prevent mold growth. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violation(s) documented must be corrected immediately. On or before November 28, 2025, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. We discussed different ways to clean walls and how to seal the water marks on the ceiling. We also discussed the importance of completing annual forms prior to the due date. Using a calendar to set reminders may be one way to remember completing the required forms. As a reminder you must complete 5 more hours of training by November 25, 2025. At the completion of the visit, this visit summary was printed, reviewed, and a copy was left with you. Your program currently operates with a three-star license, issued July 25, 2023, earning 5 points in the education component, 2 points in the program standards component All Enhanced requirements and 1 quality point for all lead teachers and teachers having at least 5 years of EC work experience in no more than 2 different centers. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-eight percent as of June 18, 2025. The license was posted, and the restrictions were in compliance. A walk-through of the facility was completed today, all indoor and outdoor areas were monitored. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in group time, free play in activity areas, transitions, and personal care routines. The caregiver was interacting and meeting the developmental needs for each of the children. Rated license requirements were reviewed to ensure compliance was maintained with current program and education components as well as the quality point. The most recent fire inspection was April 18, 2025, and the most recent sanitation inspection was November 12, 2024. The following was cited during today’s visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The Fire Drill log was not up to date with all recorded dates. April and May drills were not recorded. The provider portal was not updated with all staff. The last annual compliance visit was conducted December 20, 2022. The sanitation inspection was completed September 20, 2023, with a “Superior” classification. The last fire inspection was conducted February 24, 2023, and your facility was approved for daytime care only. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-nine percent as of November 29, 2023. Children were participating in group time. The Administrator was reading the children Christmas stories. I found supervision, staff/child ratios and interaction to be in compliance. All provider documents were inspected including staff records, Children records, emergency drills, outdoor checklist, all child care rules. The following was cited during today’s inspection. Violation Number Comment Rule 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. Verification of staff completion of the CPR course from an approved training organization was not in the staff file.
