The center director/owner, Mrs. D. Tomecek, was present and assisted me during today's visit. Currently this center operates with a Temporary License, issued on March 16th, 2026, to September 2026. The restrictions include daytime care only; Children under 2 1/2 years old in rooms with direct exits only; age range 0-6 years of age; with a capacity of 99 for first shift only. The NC Secretary of State website was viewed before the visit and, Montessori Academy of Carrboro LLC active/current as of July 7th, 2026, if any changes to the corporation need to be made or anyone from upper management decides to sell the business, then you must notify your consultant at least thirty days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. I used the Child Care Center Item Number Listing as a basic monitoring tool and checklist to assess compliance with all applicable childcare requirements pertinent to this facility. I observed children in the indoor learning environment and found supervision and staff/child ratios to be in compliance. Children throughout the facility were participating in morning snacks, outdoor time, transitions, and personal care routines. The caregivers were interacting and meeting the developmental needs for each of the children. Fire (5/11/26) inspection remains current. The last sanitation inspection was completed on 5/4/26. I reviewed all staff files during today’s visit. Existing staff files were also reviewed during the visit because a new Staff and Training Worksheet was started by the administrator prior to today’s visit. We reviewed and updated it for compliance before today’s visit. Four violations were cited during today’s visit. Three were corrected during today’s visit. Violation Number Comment Rule 540 An individual written feeding plan was not provided by child's parent or health care provider or was not followed and posted. (omit posting for centers located in a residence) A feeding plan was not posted. 10A NCAC 09 .0902(a) 541 The written feeding plan did not include the child's name, parent signature, and/or was not dated when received by the center. The written feeding plan was not signed by the teacher or the parent when received by the center. .0902(a) 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. .0606(b) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. Several staff members emergency information and health questionnaire were not dated at least annually. Most were corrected during today's visit. One staff member was on vacation during today's visit and will update her information upon her return. .0701(a) The violation(s) documented must be corrected immediately. On or before 7-21-26, I must receive a written, dated, and signed compliance letter that describes accurately and in detail how and when the violations were corrected. Also, please provide information regarding how the facility will maintain compliance in the future. Please be aware that any information submitted by you is considered legal documentation. Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. North Carolina General Statute 110-90(4)(d) requires all child care facilities to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action against the facility's license. Technical Assistance Infant Information The director and I discussed requirements for the infant classroom. The center shall post a copy of its safe sleep policy about infant safe sleep practices in a prominent place in the infant room where parents and caregivers are able to view daily along with the safe sleep poster and feeding schedule. Make sure the center's safe sleep policy is given to and explained to the parents of an infant aged 12 months or younger on or before the first day the infant attends the center. The parents shall sign a statement acknowledging the receipt and explanation of the policy. We discussed that the parent and teacher must sign the infant feeding schedule. Staff Records All staff documents must be reviewed annually and signed by staff and the administrator. Staff medical statements, proof of a tuberculosis test or screening, and completed health questionnaires shall be included in the staff member's medical file, which must be maintained separately from the staff member's individual personnel file in the center. Rated License Information Once I receive your correction letter, I will submit your rated license information to go back to a one star, to the Division of Child Development and Early Education, who will make the final decision. Please allow two weeks to receive your license. The license will be mailed to you from the Raleigh office. If and when you are ready to transition from a one star, please let me know, review the information that was provided on previous visits or visit our website.
