Ms. A. Gattis was present and assisted me during the visit. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. Children were observed participating in the following activities: free play, outside time, toileting, washing hands. The center currently operates with a Five (5) Star License that was issued September 21st, 2017. The last fire drill was conducted 9/30/25 The last fire inspection 10/4/ 24. The last sanitation inspection was 9/23/25. The last playground inspection was 9/30/25. The last documented shelter and lock down drills were conducted on 9/30/25. The following items were to be observed for compliance during today’s visit: license posted, and restrictions followed, First Aid and CPR training up to date, qualifying letters current, daily schedule posted, current activity plans, menus posted, allergies posted, EMC posted, emergency phone numbers, staff: child ratio posted, safe pick-up and delivery posted and supervision. Seven violations were cited during today’s visit; One violation was corrected during today’s visit. The providers had a chance to ask questions to ensure staying in compliance. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The operator has scheduled a fire inspection, however it wasn't within 12 months of the previous inspection. Safe sleep policy A copy of the center's safe sleep policy shall be given and explained to the parents of an infant aged 12 months or younger on or before the first day the infant attends the center. The parent shall sign a statement acknowledging the receipt and explanation of the policy. Shaken baby policy A copy of the policy shall be given and explained to the parents of newly enrolled children up to five years of age on or before the first day the child receives care at the center. The center shall obtain the parent's signature on the statement acknowledging the receipt and explanation of the policy. The acknowledgement shall contain the following and be maintained in the child's file for review by the Division. Infant feeding plan The parent or health care provider of each child under 15 months of age shall provide the center an individual written feeding plan for the child. This plan shall be followed at the center. This plan shall include the child's name, be signed by the parent or health care provider, and be dated when received by the center. Each infant's plan shall be modified in consultation with the child's parent or health care provider to reflect changes in the child's needs as he or she develops. Fire inspection Each operator shall schedule and obtain a fire inspection within 12 months of the center's previous fire inspection. The operator shall notify the local fire inspector when it is time for the center's annual fire inspection. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. Records 10A NCAC 09 .2318 CHILD CARE CENTER RECORD RETENTION All records required in this Chapter shall be maintained for review by representatives of the Division as specified in G.S. 110-91(9), Rule .0304(g) of this Chapter, and as follows: (1) The records shall be available at the center during the hours of operation listed on the child care license. (2) Records may be maintained in a paper format or an electronic format, provided that all required signatures are preserved in a paper format, PDF or other used graphic format. Records. – Each child care facility shall keep accurate records on each child receiving care in the child care facility and on each staff member or other person delegated responsibility for the care of children in accordance with a form furnished or approved by the Commission, and shall submit records as required by the Department. All records of any child care facility, except financial records, shall be available for review by the Secretary or by duly authorized representatives of the Department or a cooperating agency who shall be designated by the Secretary and shall be submitted as required by the Department. Education evaluations can take some time to process so beginning the process early will help to ensure we have the accurate information needed when it is time to complete the rated license assessment. Ms. A. Gattis was present and assisted me during the visit. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted. Children were observed participating in the following activities: free play, outside time, toileting, washing hands. The center currently operates with a Five (5) Star License that was issued September 21st, 2017. The last fire drill was conducted 9/30/25 The last fire inspection 10/4/ 24. The last sanitation inspection was 9/23/25. The last playground inspection was 9/30/25. The last documented shelter and lock down drills were conducted on 9/30/25. The following items were to be observed for compliance during today’s visit: license posted, and restrictions followed, First Aid and CPR training up to date, qualifying letters current, daily schedule posted, current activity plans, menus posted, allergies posted, EMC posted, emergency phone numbers, staff: child ratio posted, safe pick-up and delivery posted and supervision. Seven violations were cited during today’s visit; One violation was corrected during today’s visit. The providers had a chance to ask questions to ensure staying in compliance. Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The operator has scheduled a fire inspection, however it wasn't within 12 months of the previous inspection. The parent shall sign a statement acknowledging the receipt and explanation of the policy. Shaken baby policy A copy of the policy shall be given and explained to the parents of newly enrolled children up to five years of age on or before the first day the child receives care at the center. The center shall obtain the parent's signature on the statement acknowledging the receipt and explanation of the policy. The acknowledgement shall contain the following and be maintained in the child's file for review by the Division. Infant feeding plan The parent or health care provider of each child under 15 months of age shall provide the center an individual written feeding plan for the child. This plan shall be followed at the center. This plan shall include the child's name, be signed by the parent or health care provider, and be dated when received by the center. Each infant's plan shall be modified in consultation with the child's parent or health care provider to reflect changes in the child's needs as he or she develops. Fire inspection Each operator shall schedule and obtain a fire inspection within 12 months of the center's previous fire inspection. The operator shall notify the local fire inspector when it is time for the center's annual fire inspection. The operator shall submit the original of the approved annual fire inspection report to the Division within one week of the inspection visit on the form provided by the Division. The last Annual Compliance visit was conducted on March 27, 2024. The facility currently operates with a Five-Star Child Care License that was issued on September 21, 2017. The last Sanitation Inspection was completed on December 3, 2024, with a "Superior" classification. The last Fire Inspection was conducted on October 4, 2024, and the facility was approved for daytime care only. Upon arrival, I was greeted by the program director, Mattie Wardsworth. She assisted me on a walkthrough of the facility where attendance was documented, and observations made. The infants and toddlers were participating in floor play and routines. Preschoolers were observed engaging in teacher directed and free play activities. Children were observed transitioning to the playground for outdoor play. A checklist was used to note the requirements I monitored today. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. A current lesson plan was not posted or available in Space 4, This violation was corrected during the visit. GS 110-91(12); .0508(a) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. .1102(g) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by March 20, 2025. You can email the letter to me at caroline.sykes@dhhs.nc.gov The letter should include: -Facility name -Facility ID number -Each item number If a compliance letter is not received to my office by 5:00 PM March 20, 2025, a return visit may be conducted to ensure the violations cited have been corrected or addressed. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. Technical Assistance: A violation was cited regarding the Recognizing and Responding to Suspicions of Maltreatment Training. We discussed the importance of all staff members being required to take the training within the first 90 days of employment, to ensure that they are informed of best practices and procedures for recognizing and reporting child maltreatment suspicions. A violation was cited regarding current activity plans not being available or displayed for all classrooms. We discussed the importance of having teachers completed and post weekly lesson plans prior to the week beginning or on the first day of the new week. This violation was corrected during the visit. You and your staff were provided with an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your program. It is your responsibility to always maintain compliance with all the applicable laws and rules. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.
