This program located in a public school currently operates with a Five Star Child Care License that was issued on May 4, 2018. The last annual compliance visit was conducted on April 23, 2025. Upon arrival, I signed in at the school’s main office and then proceeded to the preschool classrooms where I was greeted by the staff. The children were observed engaging in indoor teacher-directed activities and preparing for and taking naps. The last sanitation inspection was completed on September 22, 2025, with “Superior” classification. The annual fire inspection was completed on January 5, 2026. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. The children's files reviewed during the visit did not contain acknowledgement by the parent of receipt of the NC Child Care Law. GS 110-102 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Three staff members did not have an annual health questionnaire on file. This violation was corrected during the visit. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. None of the staff members had the required Emergency Information Form on file. This violation was corrected during the visit. .0701(a) 1203 Operational policies were not discussed with parents on or before the child's first day and/or they were not notified in writing of all changes. The children's files reviewed during today's visit did not contain acknowledgement that operational policies were shared or discussed with parents. 10A NCAC 09 .0514(b) 1314 Emergency information did not name childs health care professional. The children's records reviewed did not contain information regarding the child's health care professional. .0802(c)(2) 1324 Signed and dated statement by parent that discipline policy received and explained at enrollment was not in child's file. The children's records reviewed during the visit did not contained signed and dated statement by the parent that the discipline policy was received and explained at enrollment. .1804(c) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. The children's files reviewed today did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy. .0608(b)(1-6) The above violations must be corrected immediately. A Corrective Action Letter must be drafted and sent to me by April 7, 2026. Additional violation may be documented if each violation has not been properly corrected or otherwise addressed. Please be aware that any information received during this monitoring visit and any violations cited may be included in any future administrative actions. Violations of child care requirements, particularly repeated violations, could result in a civil penalty and/or administrative action that could jeopardize the status of the license for the child care facility. TECHNICAL ASSISTANCE: Violations were cited regarding children’s files. The following documents must be included in the children’s files- A signed and dated statement by parents acknowledging receipt of Summary of the Child Care Laws and Rules Acknowledgement by parents of receipt and discussion of operational policies Signed and dated acknowledgement by parents of receipt of the discipline policy Signed and dated acknowledgment by parents of Prevention of Shaken Baby Syndrome and Abusive Head Trauma Policy. These acknowledgement of receipt of these documents should be in each child’s file. A violation was cited regarding missing emergency medical care information including the responsible party’s choice of health care professional. It is important that this information be included in the child’s file in the event an emergency arises. Violations were also cited regarding the completion of Emergency Information and Health Questionnaire forms by all staff annually. It is important that teachers complete these forms each year to ensure that all emergency and health information is up to date in the event that an emergency should take place. This violation was corrected during the visit. RATED LICENSE INFORMATION: We discussed the QRIS Pathways to the Stars. The program will apply for Pathway 3 using their Head Start certification. I have received the application, staff and child ratio form and Request for Accreditation and Head Start Pathway form. I will submit the rated license packet for review. Once approved, your new license should arrive in the mail. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care laws and rules. The center’s last annual compliance visit was 5/9/24. Alexandra Romero, teacher, assisted me during today’s visit. The center currently operates with a five (5) star license issued 5/14/18. The facility earned seven (7) points in staff education, seven (7) points in program standards and one (1) quality point for programmatic options met. The ECERS reassessment report for the NCPK classroom was reviewed today. Ms. Romero was given an opportunity to ask questions and the item numbers scoring 4 points or less were reviewed. Children were observed resting, preparing for departure, and routine care activities. The following violations were observed today: Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. The letter for new staff member A. Galindez was dated 5/28/24. She began employment 5/15/24. G.S. 110-90.2(b) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Romero. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 6/12/24. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION: North Carolina General Statute 110-90(4)(d) requires all childcare centers to maintain a compliance history of at least seventy-five percent (75%) for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the Department’s issuance of a provisional license or other administrative action against the center’s license. Your compliance history as of 5/29/24 is 98%. Rated License Information: Program Standards: The center is meeting enhanced space and enhanced ratios minus one. The ECERS-R were completed 4/30/24 and scored 5.41. The center qualifies for seven (7) points in program standards. The program earned one (1) quality point for staff benefits package and infrastructure of parent involvement. Education Standards: All staff are required to register in WORKS. This is the online education evaluation system for DCDEE. The steps are as follows: 1. DCDEE website – ncchildcare.ncdhhs.gov, click the ‘Provider’ tab, then ‘DCDEE WORKS’ tab 2. On the right-hand side, you will see a menu box. FIRST- all staff must have and INDIVIDUAL NCID. Click the tab ‘Creating you individual NCID’ and follow the instructions. 3. Next, staff will register in WORKS. In the menu box, click DCDEE WORKS registration I and II. Follow ALL steps as this is a multistep process. 4. Once you receive confirmation of registration, you will login to your WORKS account. On the left side, click ‘Apply/Upload’ and click on all positions you would like your education to be evaluated for. 5. Last step is to have your official transcript sent to WORKS. Go back to the DCDEE WORKS tab, and in the menu box on the right, click ‘Electronic Transcript Submission’ and follow the instructions to have your transcripts sent in. I have requested your principal, Ms. Halsey, complete a Preservice Requirements for Administrators form. Once all staff have had their education evaluated and I have the Preservice form, I will evaluate Teacher education points and finalize the rated license packet for the new license. Your assessment scores received are valid until 6/30/25. ADDITIONAL TECHNICAL ASSISTANCE: New staff member A. Galindez must complete Recognizing and Responding to Suspicions of Child Maltreatment at www.preventchildabusenc.org. This is a two-hour training. She must print the certificate at the end and place in her file. She will need to register to complete the course EDU 119 to obtain her NCECC credential since she does not have any experience in licensed childcare.
