Upon arrival I was greeted by Tammy Brown, Director. The last annual compliance visit was conducted on 11/14/24. The 18-month compliance history is currently 97%. I completed a walkthrough of the indoor space used by children and monitored for the following: supervision, staff/child ratios, CPR and First Aid certification, storage of hazardous products and medication, adequate and approved space and program records. Since the last monitoring visit one (1) new staff has been hired. The file of the new staff was monitored during this visit. The staff and training worksheet from the last annual compliance visit was used to monitor for the current required information for the existing staff. All staff have a current criminal background check qualifying letter on file. All staff have current specialized training. A fire drill was last conducted on 4/10/25. This fire drill was not documented on the emergency drill log. This was corrected during the visit. A lockdown drill was completed on 2/10/25. A superior sanitation inspection was completed on 9/4/24 with 4 demerits. The last fire inspection was completed on 2/13/25. The playground inspections have been completed as required. The following items were posted for easy reference: license, NC Summary of Law, First Aid poster, evacuation routes, safe arrival and departure procedures, menu, staff/child ratio chart, emergency telephone numbers, emergency procedures form, tobacco free signage and emergency medical care plan. Transportation is not provided. During the visit there were eighteen (18) children present. I observed the children participating in free play in activity areas, completing art activities, and bathroom/handwashing routines. The teachers were observed interacting with the children assisting with art projects and providing positive guidance. In space #1 I observed the teachers sitting at the tables with the children as they worked on art activities/projects. The teachers and children were having conversations about the art activity and the weather outside. All interactions observed were positive. In space # 5 I observed the children participating in free play with manipulatives. The teachers were actively engaged with the children. Guidance and assistance were provided as needed. All interactions and language observed were positive. There were two (2) violations of childcare requirements observed during the visit. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The facility completed a fire drill on 4/10/25 and had documentation of the drill but it was not logged on the drill record. .0604(t); .0302(d)(5) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. Medication authorization was observed in space #6 that expired in March 2025. .0803(12) The violation documented during the visit must be corrected immediately. To verify correction of the violation you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. The letter should include the facility name and facility ID number. List each item using the item number located to the left of the violation description. When submitting an email for corrections, that email must be sent from the email address of the person designated to submit corrections (example: director) and must include this person’s name and title at the end of the email. I must receive the letter by email or at the address below on or before 5/26/25. You are required to maintain compliance with all applicable childcare rules and regulations at all times. NC GS 110-90(4)(d) requires all childcare facilities to maintain a compliance history score of at least 75% for the past 18 months or during the length of time the facility has operated, whichever is less. Failure to do so may result in the issuance of a provisional license or other administrative action. Violations cited during the visit may negatively affect this score. Failure to maintain compliance with our requirements may result in administrative action against your license, which could include civil penalties. Repeated violations may significantly affect your compliance history. I reviewed this information with Tammy Brown, Director, at the conclusion of the visit and you were given a chance to ask questions. Technical Assistance: *Today we discussed that there is an updated version of the NC Summary of the North Carolina Child Care Law and Rule brochure and poster on the DCDEE website. *I reminded Ms. Brown that a shelter-in-place or lockdown drill must be completed in May 2025 to remain in compliance with the requirement of emergency drills conducted every three months. I met with Tammy Brown, Director, and I conducted a walk-through of the facility. The facility has a 4-star license with two (2) points in program standards, seven (7) points in education and one (1) quality point. Prior to this visit the facility had a 96% compliance history. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used to note the requirements monitored today. Prior to the visit I sent an email requesting the updated staff and training worksheets to Tammy Brown, Director. The worksheets were received prior to the visit. Staff and children's records were reviewed during the visit. Three (3) new staff have been hired since the last monitoring visit was conducted. During the visit, I monitored the files of the new staff members and two (2) files of existing staff members. I selected seven (7) children's records for review. The children were observed during indoor free play in centers, large group teacher-directed activities, outdoor free play, and bathroom/handwashing routines. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, emergency medical care plan, emergency telephone numbers, daily schedules, current activity plans, First Aid posters, menu, applicable staff/child ratio charts, tobacco free signage, and evacuation routes. The last fire inspection was conducted on 1/12/24, the facility was found to be satisfactory and approved for daytime care only. The last sanitation inspection was conducted on 9/4/24. The facility received a Superior rating with 4 demerits. The centers Emergency Drill Log report form was reviewed during the visit. Emergency Drills were completed as required. The last fire drill was conducted on 10/10/24. A lockdown drill was conducted on 8/21/24. The monthly outdoor inspections have been completed as required. Per the site director transportation is not provided. I monitored the following items today: supervision, staff/child ratio, staff, health, safety, medication administration, program records and management. One (1) violation of childcare requirements was observed during this visit. Violation Number Comment Rule 1329 Application for enrollment did not include all required information. Applications of two (2) of seven (7) children's files monitored during the visit were missing information. .0801(a)(1-7) The violations documented during the visit must be corrected immediately. Providers are not required to go any further with a rated license assessment unless they want to. Upon arrival I was greeted by Ms. T. Brown and I shared the reason for the visit. She assisted me with today’s visit. Your program currently operates with a Four (4) Star License. The license was posted, with restrictions to first (1st) shift care, meets enhanced ratios, children under two and one half (2 ½) years old only in rooms with direct exits, and meets enhanced ratios reduced by one (1) in each age group. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-six (96%) percent as of today’s visit. Ownership: The facility owner is SCUS Head Start Programs, INC. DBA Save the Children by SSI # 1814588 which is current and active. If any changes to the ownership need to be made, you must notify your consultant at least thirty (30) days prior to any changes occurring. The license cannot be bought, sold, subleased, transferred to another person or location or inherited. The following items were monitored: Licensing Posted, Permit Restrictions, Staff/Child Ratio, Supervision, Specialized Trainings, Storage of Hazardous Substances and Storage of Medication, Program Records, and General Safety. The following items above were found in compliance. The following inspections are below. •Last Fire drill was conducted on 4/16/2024. •Last Shelter in Place/Lockdown Drill was conducted 4/16/2024. •Last Playground Inspections were conducted on 4/12/2024. •Incident reports were reported current and in compliance. •Last fire inspection occurred on 1/12/2024. •Last Sanitation Inspection occurred on 1/10/24 with a Superior classification. The children were observed participating in dental screenings and cleanings and overall were calm and responded positively to the screenings. The younger children in the Early Head Start classrooms were participating in teacher assisted self-care routines such as diaper changing and hand washing, they were also engaging in self-directed freed play. The children in the older Head Start classrooms were participating in self-directed free choice center activities and in the oldest Head Start classroom the children were practicing for their graduations to kindergarten and/or Pre-Kindergarten. Overall, the children were happy, and well cared for, they were interacted with their teachers, myself and the Administrator. In the room that has children ages threes (3s) and fours (4s) there was one latch that had become unattached from the cabinet which held personal supplies. This was corrected during the visit. The outdoor learning environment was in good repair and offered numerous options for interactive play and social activities. I did observed the grass outside the fences on several sides of the playground as overgrown and in need of maintenance. The Administrator and I discussed speaking with Grounds Keeping a more frequent schedule now that it was warmer and grass was growing more quickly in this weather to maintain compliance. I was also able to speak with the Maintenance team and he assured me he would be taking care of the grass before I left which was in compliance. Staff Records: The staff-training worksheet was completed before the visit. There was one (1) new staff files to monitor. The file was organized and easy to follow and was in compliance. Medication: Two (2) classrooms had children who used Emergency medications, in the first room the paperwork and medication was in compliance. In the second room there was one medication permission form that had expired but it was located with the correct dates, filled out before the previous permission slip expired and was added to the medication bag. This is in compliance. The Menu and allergy list were posted with breakfast consisting of low sugar cereal, milk and apple juice. Weapons: Today you reported that your facility was in compliance with childcare requirements regarding firearms. Transportation: Transportation is not provided. One (1) violations were observed today and it was corrected during today’s visit. Violation Number Comment Rule 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In room number four (#4) the latch under the sink was not catching and remaining locked. .2820(b) Child care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent (75%). Any violation (s) documented may impact the compliance history score. The center's compliance history was reviewed with the operator. The compliance history could be impacted after today’s visit. Based on Child Care Rule 10A NCAC 09 Section .2200, the Division of Child Development and Early Education may take administrative action against the license and/or impose civil penalties based on the failure of the operator to correct any documented violations within the established time period. If you state in your letter that corrections have been made when they have not, it will be considered falsification of information. Rated License •Reach out to your Child Care Consultant for the facility’s star rated cohort assessment and preparation year. Please make the Child Care Consultant aware of when and if you are ready to begin the Star Rated License process within the cohort timeline. Please be aware there is an Assessment Preparation Year and an Assessment year for ER Scales it is strongly encouraged to participate in the Assessment Preparation Year. Technical Assistance •All classrooms were open today and Ms. Brown is aware that the newest opened Early Head Start classroom is in need of more learning supplies. •Reach out to your local CCRI and Partnership for Children for assistance with the Rated License process. •Rule 15A NCAC 18A .2816 requires currently operating child care centers to test water initially and every three years. Family child care homes are required to test water for lead once. o DCDEE encourages you to utilize the Clean Classrooms for Children’s program. You can take the pre-enrollment webinars which is available at www.cleanwaterforUSkids.org/carolina then enroll – At www.cleanwaterforUSkids.org/carolina completing the enrollment surveys for three program sections: 1) lead in water, 2) lead-based paint, and 3) asbestos. Keep mindful that once you evaluate potential hazards and receive results if there are recommendations and/or water mitigation support is needed they will provide direction in how to mitigate and ways to fund said mitigation. We jointly reviewed the visit summary and documentation of my findings and violations with you, I encouraged you to ask any questions and you stated that you had none.
