K. Cooper, Family Child Care Home Operator, assisted me with the visit. Your program currently operates with a four-star license, issued February 15, 2025, earning 7 points in the education component, 2 points in the program standards component and 1 quality point for has a BS/BA or higher in Early Childhood/Child Development. The last annual compliance visit was conducted on June 9, 2025. The center's compliance history was reviewed with the operator. The program’s compliance history was ninety-four percent as of April 16, 2026. The family child care home is owned as a Sole Proprietorship. License and contact information were current. Contact me to request any changes to your license or contact information. I monitored the indoor spaces used for child care (or accessible to children) and outdoor play areas for applicable child care center requirements using the Child Care Center Requirements, effective 7/1/2025 and Item Numbers Listing, effective 4/2025. I monitored operator and household member/staff records, all children’s records and all program records. I observed each indoor and outdoor space used by children or accessible to children. The two infants and toddlers played with toys and materials on the floor. The infants spent about twenty minutes in baby walkers. The operator moved them to the floor. She read stories to the toddlers. The toddlers ate in high chairs. The operator held the infants when she fed them. After lunch, the infants rested in playpens. Documentation of safe sleep checks was available. The toddlers rested on sleeping mats. Lunch was observed and consisted of cheese pizza, carrots, mandarin oranges, and milk. The infants ate baby food and drank formula from bottles. I observed and documented the following violations during today’s visit. You corrected four of the six violations during the visit. A substitute required to re-certify First Aid by January 31, 2026 did not complete it until March 2, 2026. .1703(a)(2) 511 Operator did not successfully complete a CPR course as referenced in Rule.1702(b)(2) CPR training was not renewed on or before the expiration of the certification. A substitute required to re-certify CPR by January 31, 2026 did not complete it until March 2, 2026. .1703(a)(3) 906 Authorization to seek medical care in the parent's absence was not available. The Child's Application for a child enrolled on September 8, 2025 did not include a signed and dated statement providing authorization to see medical care. .1721(a)(3)(I) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Children's medical reports were received later than thirty days after enrollment for four children enrolled on August 12, 2025, September 8, 2025, February 5, 2026, and February 20, 2026. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file. Immunization records were received later than thirty days after enrollment for four children enrolled on August 12, 2025, September 8, 2025, November 19, 2025, and February 20, 2026. GS 110-91(1); .1721(a)(2) 2041 Application did not include the names and the phone numbers of the child's physician and the persons to be contacted in an emergency situation. The Child's Application for a child enrolled on September 8, 2025 did not included the name of the child's physician and phone number. 10A NCAC 09 .1721(a)(3)(G)(H) * Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violations documented may impact the compliance history score. You corrected four of the six violations during the visit. The remaining two violations documented must be corrected immediately. On or before May 4, 2026, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected. Please be aware that any information submitted by you is considered legal documentation. If it is determined the information provided in the letter is not true, this may be considered falsification of information. If sufficient information is not received by the due date, a follow-up visit will be conducted. It also provides a refresher so that you are confident to act in a medical emergency. First Aid and CPR expired for one of your substitutes (KE) on January 31, 2026, but she did not complete First Aid and CPR until March 2, 2026. You stated that KE was not able to take the class when you registered. Use your FCCH Verification Form to track due dates for items that expire such as Criminal Background Checks, CPR/First Aid, ITS/SIDS Training, etc. Blended classes with on-line instruction and face-to-face skills test are now allowed if provided by approved agency and trainer. This has been corrected. No further action or documentation in required except to maintain current First Aid and CPR in the future. Children’s Files - Maintaining accurate records on the children in your care helps to ensure their health and safety, informs families of rules you are required to follow and your center’s policies, and provides a certain transfer of knowledge so that you can provide the best care possible for their children. The following items were missing from your children’s records: #906 – The Child’s Application completed for PL (enrolled September 8, 2025) did not include a signed and dated signature authorizing the operator to seek emergency medical care in the parent’s absence. For your compliance letter, state the date when the parent has signed and dated the authorization to seek emergency medical care for PL. #910 – Children’s Medical Reports were received after the thirty-day deadline for the following children: RM (enrolled August 12, 2025, received February 3, 2026); PL (enrolled September 8, 2025, received November 24, 2025); TC (enrolled February 5, 2026, received April 20, 2026); YS (enrolled on February 20, 2026; received April 20, 2026). This violation has been corrected. #911 – Documentation of current immunizations were received after the thirty-day deadline for the following children: RM (enrolled August 12, 2025, received February 3, 2026); PL (enrolled September 8, 2025, received November 24, 2025); AS (enrolled November 19, 2025, received February 10, 2026); YS (enrolled on February 20, 2026; received April 20, 2026). This violation has been corrected. #2041 – The Child’s Application completed for PL (enrolled September 8, 2025) did not include the child’s preferred physician and phone number. For your compliance letter, state the date when the parent has added the child’s physician and phone number for PL. You stated that you had a difficult time receiving immunization and medical records for children within the thirty-day time frame due to parents or guardians not providing the forms in time. Even though you have thirty days for the parents/guardians to provide you with health assessments and immunizations, you can require them at enrollment to reduce the amount of chasing around you may have to do to receive the required paperwork. If you do provide the thirty days, give them weekly reminders but if the paperwork is not received within the thirty days, let them know that their child cannot attend until received. Today, you asked to remove approval for transportation. You stated that you do not take children in your vehicle and only use your daughters as substitutes occasionally, less than ten days per year. ABCMS Portal – The Criminal Background Check Unit sent out a list of facilities that either do not have a certificate that they have completed the ABCMS Provider Portal Training and/or have not actively used the ABCMS Provider Portal to obtain their pay codes (Provider Pay Code or Applicant Pay Code), assist staff and/or household members with connecting to their license using the pay code, and then hiring staff and/or household members to their roster. At a Routine Unannounced visit on 12/16/24, you showed me a certificate that you had completed the ABCMS Provider Portal Training; however, I am still not seeing that you and your household members have gone through the process to connect to your facility after which you can “hire” them to your roster. Why is this important? It assists the Division know who is working and/or residing in licensed child care facilities and helps to protect the children in care and staff who work there. By April 30, 2026, please email me a copy of your certificate that you have completed this training and let me know that you have completed your roster. I will forward the ABCMS Provider Portal Technical Assistance information (click by click instructions) to complete the steps to create your roster. To be successful at this (or if you need a new certificate or a refresher), you may need to consider completing the ABCMS Provider Portal Training in Moodle again. (I had to complete it three times!) Consider cutting the pizza into smaller bites to make it easier for them to pick up and handfeed themselves. You may want to split up feeding times for the two infants and the two toddlers so that you can assist each group with feeding. This violation has been corrected. #2041 – The Child’s Application completed for PL (enrolled September 8, 2025) did not include the child’s preferred physician and phone number. For your compliance letter, state the date when the parent has added the child’s physician and phone number for PL. You stated that you had a difficult time receiving immunization and medical records for children within the thirty-day time frame due to parents or guardians not providing the forms in time. Even though you have thirty days for the parents/guardians to provide you with health assessments and immunizations, you can require them at enrollment to reduce the amount of chasing around you may have to do to receive the required paperwork. If you do provide the thirty days, give them weekly reminders but if the paperwork is not received within the thirty days, let them know that their child cannot attend until received. At a Routine Unannounced visit on 12/16/24, you showed me a certificate that you had completed the ABCMS Provider Portal Training; however, I am still not seeing that you and your household members have gone through the process to connect to your facility after which you can “hire” them to your roster. Why is this important? It assists the Division know who is working and/or residing in licensed child care facilities and helps to protect the children in care and staff who work there. By April 30, 2026, please email me a copy of your certificate that you have completed this training and let me know that you have completed your roster. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 7/1/2025 and Item Number Listing, effective 4/2025. I also reviewed operator and household member records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation; however, the operator stated that she only occasionally takes children in her car because they are so young. The operator stated that her daughters occasionally substitute for her. The Written Plan of Care currently states that the operator occasionally performs routine tasks during operating hours by transporting children in her vehicle and/or by leaving them with a qualified substitute. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations I completed a walk-through of the child care spaces and monitored all indoor and outdoor spaces used for child care. Two infants, both four months of age, were present and participated in typical infant routines. They slept, ate, and had their diapers changed on their own individual schedules. The operator provided floor time for both infants. Permit capacity and supervision were in compliance. Infants were placed on their backs to sleep, and documentation of safe sleep checks was available. The operator interacted positively with the children, providing encouragement and guidance. They were fed bottles on demand. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 921 Operator did not maintain accurate daily attendance records including documentation of arrival and departure for all children in care, including the operator's own preschool children. Documentation of arrival was not recorded for the two children present. .1721(e)(6) 1821 Caregivers did not document compliance with visually checking on sleeping infants aged 12 months or younger and/or the documents were not maintained for a minimum of 30 days. Documentation of safe sleep checks were not available for the two infants who recently enrolled. .1724(a)(8)&(f) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, K. Cooper. We reviewed the visit summary and two violations documented during today’s visit. You corrected both violations during the visit, so no further action is needed except to continue documenting arrival and departure and documenting safe sleep check. Technical Assistance With Documented Violations: Attendance – Having accurate attendance is important to have as documentation if there is a question about children’s attendance related to payments, subsidy, or even parental disputes. More importantly it helps to accurately account for children in the event of an emergency where you might have to evacuate the home. Documentation of arrival for the two infants had not been completed today. You stated that you had just forgotten and added the infants’ arrival times and will keep your clipboard handy so that you remember. Documentation of attendance helps to ensure the safety of children and must be current throughout the operating day, including arrival and departure times. Your attendance records must reflect all children who are enrolled in and attend your program (full-time, part-time, drop-in, one-time) whether they are present or not. You can complete this information, but it is best practice to require parents to complete this daily at drop-off and pick-up and then sign or initial. Regardless of who does it, current, up-to-date attendance records must be available, and they must match what is submitted to other agencies for subsidy or food program requirements. Maintain attendance records for at least one year. This item has been corrected. Safe Sleep Charts – Checking on infants every fifteen minutes and documenting it on a safe sleep chart, helps to ensure that infants sleep in a safe environment. Documentation of safe sleep checks was not available for two infants recently enrolled. You stated that you had forgotten to print out charts and check on them continuously but had not documented their sleep position. Today you printed out charts for each baby and completed an initial safe sleep check when they went down for a nap. This item has been corrected. Continue to conduct safe sleep checks. Always place infants on their backs and document it on the safe sleep chart. Once infants can roll over on their own, you can document the other sleep positions. The Clean Classrooms for Carolina Kids website reflects the following: Water Testing –completed 9/19/24 Lead Paint Testing – enrollment started Asbestos Testing – enrollment started *Complete the enrollment process for your lead paint and asbestos testing. The Child Care Commission adopted the proposed QRIS Modernization rules, but they are not in effect yet. Note that the program assessment tool is now the Family Child Care Environment Rating Scale – 3 (FCCERS-3). The family child care home is owned as a Sole Proprietorship. K. Cooper reported no changes to owner information, facility contact information (phone, address, email) or license information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 11/1/2024 and Item Number Listing, effective 4/2025. I also reviewed all children’s records, operator and household member records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation; however, the operator stated that she only occasionally takes children in her car because they are so young. The operator stated that her daughters occasionally substitute for her. I monitored their staff files. The Written Plan of Care currently states that the operator occasionally performs routine tasks during operating hours by transporting children in her vehicle and/or by leaving them with a qualified substitute. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations The operator was present with one child, four years of age. A school-age child was expected to arrive later along with other children who attend on second and third shifts. The child worked on letters with the operator who praised his work and his efforts. He played with the developmentally appropriate materials. The operator read “How to Catch a Mermaid” and asked questions to check for understanding. Lunch was reported to be hot dogs on bun, applesauce, French fries, and milk. I observed and documented the following violations during today’s visit. Last Annual Compliance Visit – 8/6/24 18-month compliance history from 12/5/23 – 6/4/25 = 95%. The Clean Classrooms for Carolina Kids website reflects the following: Water Testing –completed 9/19/24 Lead Paint Testing – enrollment started Asbestos Testing – enrollment started *Complete the enrollment process for your lead paint and asbestos testing. Cooper, was present and assisted me with the visit. The family child care home currently operates with a Four-Star License issued 2/15/2025, earning points in the following components: Program (2 PTS) + Education (7 PTS) + Quality Point (1 PT) = 10 Points = 4 Stars Your three-year rated license assessment is due 2/15/2028. The Child Care Commission adopted the proposed QRIS Modernization rules, but they are not in effect yet. Information, training, resources, and updated forms will be shared with you as we transition to the new rules. See Additional Comments for more information. Note that the program assessment tool is now the Family Child Care Environment Rating Scale – 3 (FCCERS-3). Information and training on the new program assessment tool is available on the NC Rating License Assessment Project website at https://ncrlap.org/. The family child care home is owned as a Sole Proprietorship. K. Cooper reported no changes to owner information, facility contact information (phone, address, email) or license information. Please contact me prior to any information change regarding the ownership of this facility or contact information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 11/1/2024 and Item Number Listing, effective 4/2025. I also reviewed all children’s records, operator and household member records, and program records. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation; however, the operator stated that she only occasionally takes children in her car because they are so young. The operator stated that her daughters occasionally substitute for her. I monitored their staff files. The Written Plan of Care currently states that the operator occasionally performs routine tasks during operating hours by transporting children in her vehicle and/or by leaving them with a qualified substitute. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations The operator was present with one child, four years of age. A school-age child was expected to arrive later along with other children who attend on second and third shifts. The child worked on letters with the operator who praised his work and his efforts. He played with the developmentally appropriate materials. The operator read “How to Catch a Mermaid” and asked questions to check for understanding. Lunch was reported to be hot dogs on bun, applesauce, French fries, and milk. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. Operator did not receive the child's medical report or health assessment until March 20, 2025 for a child enrolled on November 22, 2024. GS 110-91(1); 10A NCAC 09.1721(a)(1) 1306 Operator has not completed ITS-SIDS training every three years from the completion date of the previous ITS-SIDS training. ITS/SIDS training for the operator and two substitutes expired on March 3, 2025. The operator and one substitute did not complete ITS/SIDS training until April 21, 2025. The second substitute has not completed ITS/SIDS training. .1703(a)(4) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, K. Cooper. We reviewed the visit summary and violation(s) documented during today’s visit. Correct these immediately. Send evidence of correction to me so that I receive it no later than 6/23/25. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Do not include names of children in the compliance letter. Use their initials. Also include the following documents/pictures to verify compliance. Technical Assistance with Documented Violations: Child’s Medical Report - Maintaining accurate records on the children in your care helps to ensure their health and safety, informs families of rules you are required to follow and your center’s policies, and provides a certain transfer of knowledge so that you can provide the best care possible for their children. A Children’s Medical Report was not received for KR, enrolled on 11/22/24, until 3/20/25. You stated that you had misplaced the one received at enrollment and the mother provided you with a more current child’s medical report. This has been corrected. Filing documents when received will help you maintain organized, current files. ITS/SIDS Training – ITS/SIDS Training completed before licensure and every three years ensures that caregivers of infants are aware of the latest research on safe sleep practices and SIDS reduction. ITS/SIDS training renewal was due for the operator and two substitutes by 3/3/2025 but was not renewed by the operator and one substitute until 4/21/2025. The second substitute, K. Evans, does not have current ITS/SIDS training. You stated that you tried to register for an earlier class, but space was not available. K. Evans is registered for ITS/SIDS training on 6/16/25. Knowing that classes offered by AACF fill up quickly, consider registering for training a few months before training expires to ensure that you complete them on time. For your compliance letter, state the date when KE completes ITS/SIDS training. Attach a copy of her training certificate with the compliance email. You need 3 hours of on-going training by 8/6/25 (anniversary of the last Annual Compliance Visit). I will check for completion at your next visit. Then you need 5 hours of on-going training by 6/9/26 (anniversary of this Annual Compliance visit). You stated that your substitutes work less than 10 days per year. Currently they are not required to complete on-going training. Five-Year H&S Training for you and your substitutes is due by 1/2028. ABCMS Portal - The process of notifying the Division when you have new staff (or household members for family child care homes and centers licensed in a residence) has changed and is now captured in ABCMS, the new Criminal Background Check portal. You have completed the ABCMS Provider Portal Training and are now authorized to log in the ABCMS Provider Portal https://ncabcms.nc.gov/DCDEE/ using your Business NCID. You have not been able to navigate the portal. I will send you the ABCMS Provider Portal Access Guide which was created to take you step-by-step through the process to connect you, staff, and household members to your “roster”. *Complete your own Connecting Application so that you can learn how to do it and assist staff/hhm. Emergency Preparedness and Response (EPR) Requirements and Ready-to-Go File – Your Emergency Preparedness and Response (EPR) Plan needs to be reviewed and updated annually. 1. Log into the Emergency Management Portal on the DCDEE website -> Provider tab -> Emergency Preparedness and Response 2. Scroll down to NC Risk Management Portal link under the heading “Emergency Preparedness and Response Plan Template. Log in at top right using NCID used to create or update EPR Plan. 3. Click Planning Tools at the top. 4. Click Child Care Emergency Preparedness and Response Plan 5. Click “Get Started”. Your plan should appear in a drop-down box. If you click on the down arrow, there may be other plans connected to the account. 6. Review EPR Plan (listed by name on cover of most recent plan) and update as needed. Remember that this is the plan that you will practice through fire and emergency drills with the intention of training your substitutes and children to be knowledgeable and confident in an emergency. EPR Plans have saved lives in NC child cares when there have been tornadoes, fires, and unwanted visitors. The more information you can provide (names, phone numbers, center floor plans, evacuation plans, stay-in-place plans, meet-up plans), the better prepared you and staff will be. These plans are also available to the county emergency management teams who can assist you. Don’t take this lightly. Be intentional with your EPR Plan, review it with staff, and practice. 7. Document update/review date on page 28. If changes are needed, print the plan up to page 29. If no changes are needed, print the new cover page and page 28 with the update/review date reflected. They gathered for group time and the operator read a story to them. Breakfast was sausage, biscuit, banana, milk, and juice. I observed and documented the following violation during today’s visit. Technical Assistance With Documented Violation: Storage of Hand Sanitizer - Improper storge of medications results in millions of accidental poisonings in children annually. Hand sanitizer was stored on a table within reach of the two children present. The operator stated she accidently left it out and gave it to a household member to store out of reach in the home. Topical ointments and lotions such as hand sanitizer, diaper cream, sunscreen, etc. do not have to be locked but must be stored out of reach of children, at least five feet or higher. Finally, group sensory play can teach kids how to cooperate and interact with others in terms of problem-solving." This is the introduction to the article, "6 creative sensory tubs to inspire kid explorers" on the Mashable.com website. The article notes: "With just a few items you can create your own sensory bin — and it's a lot better than sitting the kids in front of the television. To do so, fill a shallow container with items that naturally stimulate a child's senses — this can range from the scent from a cinnamon stick to feeling uncooked beans in a tub. Just be careful of choking hazards." ABCMS Portal - The process of notifying the Division when you have new household members (or staff if you choose to have a substitute or additional caregiver) has changed and is now captured in ABCMS, the new Criminal Background Check portal. You have completed the ABCMS Provider Portal Training and have received a facility code to access the ABCMS Provider Portal where you will verify your facility roster to ensure current household members are listed. If household members are not listed, you will provide them with the facility code so they can complete a Connecting Application (no fee) and be added to your roster. You can “remove” household members (or staff) listed on your roster who are no longer living or working at your home. Then you will update this information in ABCMS within five days as household members move in or turn 16 years of age or move out. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. • Complete your facility roster which will include household members who are 16 years of age and older and staff, if any. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation; however, the operator stated that she only occasionally takes children in her car because they are so young. The operator stated that her daughters occasionally substitute for her. I monitored their staff files. The Written Plan of Care currently reflects that the operator performs routine tasks during operating hours by transporting children in her vehicle and/or by leaving them with a qualified substitute. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations The operator was present with seven children, 22 months through 7 years of age. The parent of the two of the children who attend on the overnight shift was late picking up her children and arrived about ten minutes later. After they left, a child, 4 years of age, arrived, and the operator was present with five children, 22 months through 5 years of age. The operator’s school-age grandchild who lives in the home as in the home and not in the child care space. They played with the developmentally toys and materials available to them. When the operator’s alarm went off, they joined her for circle time where she read a story to them. The children washed their hands and ate morning snack which was graham crackers and milk. The younger toddlers played while the operator worked on letters with the older children using a coloring worksheet and then playing a game with them. Later they danced to a music and movement video. The operator stated that no children enrolled require medication; and no medications were observed. I observed and documented the following violation during today’s visit. The Cohort 2 Rated License Preparation Year is from 7/1/2024 – 6/30/2025 and will start this summer. You may complete your FCCERS-R Self-Assessment and rated license reassessment during this year or you may wait until your Assessment Year from 7/1/2025 – 6/30/2026. During both of these years, you may request and complete a FCCERS-R assessment to potentially earn more points in Program Standards and a Fifth Star. We will discuss this more at your next visit, but feel free to contact me with any questions about the Cohort Model. The family child care home’s 18-month compliance history from 8/2/22 – 2/1/24 = 100%. The family child care home is owned as a Sole Proprietorship. K. Cooper reported no changes to facility contact information (phone, address, email) or license information. Please contact me prior to any information change regarding the ownership of this facility or contact information. I monitored the indoor and outdoor spaces used for child care for applicable family child care home requirements using the NC Child Care Requirements, effective 1/1/2024 and Item Number Listing, effective 6/2022. I also reviewed program records but used the FCCH Verification Form to remind you of staff and household member due dates. I checked the car designated for transportation and the required documents; the family child care home continues to be approved for transportation; however, the operator stated that she only takes school-age children for off-premises activities in the summer. The operator stated that her daughters occasionally substitute for her. I monitored their staff files. The Written Plan of Care currently reflects that the operator performs routine tasks during operating hours by transporting children in her vehicle and/or by leaving them with a qualified substitute. If there are any changes to the Written Plan of Care, it must be revised and provided to parents. Parents must sign that they have received the updated Written Plan of Care. Observations The operator was present with five children, sixteen months through three years of age. Her school-age granddaughter who lives in the home was present in the home and not in school today. The older children were using a computer program to sing songs and fingerplays. The operator read a story about bears and hibernation. After they engaged in free play. Lunch was turkey and cheese with crackers, carrots, peaches, and milk. I observed and documented the following violations during today’s visit. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. Documentation of outdoor checks was not available since September 2023. 10A NCAC .1721(e)(5)(A-F) * Child care programs are expected to achieve and always maintain compliance and are required by NC GS 110-90(4) d(d) to achieve and maintain an 18-month compliance history score of at least 75%. Any violations documented may impact the compliance history score. I completed an electronic copy of the visit summary and provided a printed copy of it to you, K. Cooper. We reviewed the visit summary and violation documented during today’s visit. Correct this immediately. Send evidence of correction to me so that I receive it no later than 2/19/2024. Your compliance verification letter needs to state your facility name and license number and list each violation by Item # and describe in detail when and how each violation has been corrected. Send compliance verification letter in an email from your fcch’s official email address, kaycooper66@hotmail.com, to my email address, Marjorie.White@dhhs.nc.gov. You may include the compliance verification as an attachment or in the body of the email. I will email a sample compliance verification letter to you. If sufficient information is not received by the due date, a follow-up visit may be conducted. Please be aware that any information submitted by you is legal documentation. Technical Assistance with Documented Violation: Outdoor Inspections – Regular outdoor inspections are critical to prevent deterioration of equipment and accumulation of hazardous materials within the play site, and to ensure that appropriate repairs are made as soon as possible A monthly safety check of all the equipment within the facility as a focused task provides an opportunity to notice wear and tear that requires maintenance or replacement. Documentation of outdoor inspections was not available since September 2023. You stated that you check the playground daily but had misplaced your outdoor inspection sheets during the renovation and then just forgot about them. Begin completing them again for February 2024. General Visit Information: As a licensed family child care home operator, it is your responsibility to be knowledgeable of the child care laws and rules. The most recent versions of laws and rules regarding child care facilities in North Carolina are available on the DCDEE website, www.ncchildcare.ncdhhs.gov. You can also review the “What’s New” section and download a copy of the Items Number Listing which you can use as a checklist for your program Reminders: On-going Training and Health & Safety Training - You need 2.5 hours of on-going training by 9/18/24 (anniversary of last Annual Compliance visit). You stated that your daughters substitute less than 10 hours annually and are not required to complete on-going training. Your Five-Year H&S Training for you and your substitutes is due by 1/2028. Emergency Preparedness and Response (EPR) Requirements – *Your EPR Plan was last reviewed in February 2023. Review in the Emergency Management Portal and update or document date of review by February 2024. Log into the Emergency Management Portal and review your EPR Plan annually and update, as needed. Here are the steps: *DCDEE website (https://ncchildcare.ncdhhs.gov/) -> Provider tab -> Training and Professional Development -> Emergency Preparedness and Response -> NC Risk Management Portal -> Log in using NCID used to create or update EPR Plan *Review information and make needed updates. If no updates are needed, document your date of review on Page 28 and only print a new cover page and page 28. Include it with your EPR Plan. If updates are needed, revise in the Emergency Management Portal and print a new copy with the revision date. Document revision date on Page 28 and print the entire plan through page 28. Eaton, Training and Program Development Consultant Amia.Eaton@dhhs.nc.gov Activity Plans - Complete weekly activity plans (dated) that include developmentally appropriate activities for enrolled children. Activity plans for all children must meet the following minimum licensing requirements: *Include activities daily to stimulate the following 5 developmental domains, as listed in your NC Foundations for Early Learning and Development book: (social &emotional development, health & physical development, approaches to play and learning, language development/communication, and cognitive development). *Include a daily gross motor activity which may occur indoors or outdoors. *Reflect at least 4 activities daily (one of which is gross motor), one of which is offered outdoors, weather permitting. The remaining 3 activities should occur in the following areas (art/creative play, children’s books, blocks and block building, manipulatives and family living/dramatic play). *Include both free choice and adult-directed activities, which allow children to choose to participate with the whole group, in a small group or independently. *In addition, activity plans must include each of these 3 enrichment activities on at least a weekly basis: sand or water play; science and nature; and music and rhythm. A sample lesson plan form containing all required components was provided. *Materials and equipment must be available both inside and outside to support the activities on the activity plan and the daily schedule. *When school-agers are enrolled, your activity plan must also list a balance of activities that are developmentally appropriate for the age, needs and interests of the school-age children. Screen Time – Screen time includes time using television, computer, monitor, phone, tablet, etc. Children typically receive at least 2 hours or more of screen time at home. Time that children spend in front of a screen reduces their time to interact with adults and other children to learn vocabulary, conversation, and social/emotional skills. It also affects their distance vision and tends to cause children to be sedentary instead of active and moving. Screen time rules include: 1. Not permitted for children younger than 3 years of age. If you have children younger than 3 years of age, you will need to offer a non-screen time activity with them. 2. Limited to 30 minutes per day. Document on a screen time log. 3. Must be educational and included on the activity plan. 4. If used daily, include on daily schedule. 5. TV, including news, should not be played in the background. Subjects that are not appropriate for young children are on display that could cause young children stress and anxiety. Materials – Add manipulatives (puzzles, linking toys and building toys, etc.) to your activity centers. Children need to be able to pick from a variety of materials in centers to keep them engaged and interested in play without fighting over popular items. Storage in Playpens – If storing items in playpens, be sure to place clean linen in the playpen before the infant or toddlers uses it for naptime.
