Name of Operation: WALKING BY FAITH DAYCARE Facility ID: 01000636 Consultant: LORI RIERSON Operation Type: Family CC Home Case Number: Visit Date: 5/20/2025 Number Present: 5 Completed Date: 5/20/2025 Age: From 1 To 3 Total Minutes: 140 Time In: 10:35 AM Time Out: 12:55 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Annual Comp Full Announced/Unannounced: Unannounced Today an unannounced annual compliance visit was conducted. A checklist was used today to note the childcare requirements monitored today, and a copy was left with you. The last annual compliance visit was conducted 5/30/24. This family childcare home currently operates with a three (3) star license issued 8/19/19. The owner/operator Brenda Harris assisted me during today’s visit. Eight preschool-age children and three school-age children are enrolled. Five preschool children were present today. The home is licensed for first, second, and third shifts. The children were observed eating lunch, resting, and routine care activities. The designated care areas are the living room, kitchen, den area, spare bedroom, and bathroom. The last fire inspection was completed 1/2/25. Transportation is not provided. The provider does not administer medications and no firearms are present in the home. A full assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 2023 Operator and/or staff who work with children, did not complete health and safety training as part of on-going training so that every five years, all the topic areas were covered. The five-year completion for B. Harris was 12/2024. Technical assistance was provided. .1703(d)(2) 2058 The family child care home operator did not collect and submit samples of water from each outlet used for drinking or food preparation for lead analysis to the local health department or a laboratory certified to analyze for lead in drinking water by the North Carolina State Laboratory of Public Health. The required testing was not complete. Technical assistance was provided. 10A NCAC 09 .1725(a)(2) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Harris. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 6/3/25. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected *How you will maintain compliance in the future All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION: All licensed childcare facilities are currently under the ‘Hold-Harmless’ state for their license from DCDEE. Once QRIS is implemented, you will receive information from DCDEE regarding the new pathways for licensure and your timeframe for completing your license renewal. The NCRLAP website, https://ncrlap.org, has trainings geared towards the new FCCERS3 assessment scales. These will introduce you to the updated assessment scale and provide tips for completing the new scale. Ms. Harris and I spent some time today visiting the clean water for US kids website, registering for the webinar, and discussing next steps for completing the water sample. Ms. Harris and I logged into the Moodle website and I assisted her in locating the 2025 CCDF Health and Safety trainings. I left a copy of the training log today. We discussed medication storage if a child requires a medical action plan. First, be sure to carefully review each child’s application to determine if an action plan is required. The parent must complete an action plan and permission to administer medication forms. If the parent signs the forms, they are valid for six months. If a physician signs, they are valid for twelve months. Emergency medications are not in locked storage and must be kept up at least five feet above children and inaccessible to children. If an additional over-the-counter medication is listed like Benadryl, it must be stored in a small locked box and kept with the unlocked emergency medication. A child requiring an action plan cannot attend without the plan and required medications being onsite. The medications must be in the original prescription packaging. Upcoming training opportunities can be found on the CCSA website https://www.childcareservices.org/programs/ccrr/professional-development/ Documents and forms for DCDEE licensure can be found at https://ncchildcare.ncdhhs.gov/Provider/Provider-Documents-and-Forms.
