The facility currently holds a Five-Star Rated License. The most recent Annual Compliance Visit was completed 12/23/2025. Summary of Investigation Upon arrival at the facility, I was greeted by the Assistant Director, who remained present and cooperative throughout the visit. The purpose of the visit was explained. The report alleged concerns regarding supervision, sanitation, and safe indoor environment. Supervision, Health Practices, and Facility Safety A walkthrough of the indoor areas was conducted. Staff were observed providing adequate supervision of children at the time of the visit, in accordance with 10A NCAC 09 .1801(a)(1-5), which requires children to be adequately supervised at all times. During the walkthrough, the Assistant Director reported that approximately one month prior, a child was left unattended on the playground. In response, the center implemented corrective actions. The involved teacher received additional training on active supervision, was issued a written disciplinary action, and now conducts head counts before and after transitioning to and from outdoor play. The Directors also implemented head counts before and after outdoor play for a period of 30 days to reinforce supervision practices. Hygiene and Diapering Procedures Several children were observed with visibly dirty noses. Additionally, a diaper change was observed in the toddler classroom where all required diapering steps were not followed as required. The teacher was immediately advised of the concern and referred to the posted diaper-changing procedure located at the diaper-changing table. The Assistant Director stated that several children were currently out due to influenza. I did a walkthrough of the facility and did not observe and safety hazards. Findings Based on observations, the allegation regarding supervision is substantiated. Based on observations, the allegation regarding sanitation is substantiated. Based on observations, the allegation regarding safe indoor environment is unsubstantiated. A partial assessment of child care requirements was completed during the visit. Three violations of child care requirements were cited during today’s visit. Violation Number Comment Rule 303 Children were not adequately supervised at all times. Approximately one month ago, a child was left unattended on the playground. .1801(a)(1-5) 1860 Staff did not provide assistance to each child to ensure good hygiene. Several children were observed with runny noses or dried snot on their faces. .0806(e) 9999 A violation was found for which there is no item number. All diaper changing steps were not followed. As required by 15A NCAC 18A .2819 DIAPERING AND DIAPER CHANGING FACILITIES The above violations must be corrected immediately, and a letter must be sent to me no later than January 9, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. If I do not receive your letter by December 30, 2026 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance: The center was reminded that supervision must be continuous and intentional, particularly during transitions. Recommended practices include: Conducting and documenting head counts during all transitions Assigning staff specific supervision zones on the playground Reinforcing supervision expectations during staff meetings and ongoing training The center was also reminded that: Children’s noses must be cleaned promptly to reduce the spread of germs and viruses All diaper-changing steps must be followed every time, including proper handwashing and surface sanitation Staff should closely monitor health practices, especially during periods of increased illness The center was encouraged to provide refresher training on hygiene and diapering procedures and to actively monitor staff compliance. The facility currently holds a Five Star license. The last Annual Compliance Visit was completed on February 4, 2025. Upon arrival, I was greeted by the Director and Co-Director, who assisted me throughout the visit. I explained the purpose of my visit. Concerns had been raised regarding staff/child ratios in the classroom for 3- and 4-year-olds. While observing children playing outside, I also observed the children in the classroom for 2- and 3-year-olds and noted the class was out of ratio. I verified enrollment and attendance records and confirmed the number of children with a classroom teacher. The Director explained that a child from another classroom of 2-year-old had joined the 3-year-old classroom without a count being taken prior to the transition. Findings: Based on observations, staff/child ratios were not in compliance, resulting in a violation of childcare requirements. A partial assessment of child care requirements was conducted. Supervision was in compliance during today’s visit. One violation was documented. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. In the classroom for 2 and 3 year olds, there was 2 teachers with 19 children. GS 110-91(7);.0713(a-d) The above violation must be corrected immediately, and a letter must be sent to me no later than September 16, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The last annual compliance visit was conducted on February 4, 2025. During the previous visit on February 4, 2025, I monitored for supervision staff chart ratio adequate approved space and the outdoor learning environment monitored, a sampling of children's records and all program records. The facility license was posted, and permit restrictions were maintained. During today's visit I monitored eleven (11) new staff files and a sampling of other staff files. I monitored all staff training and staff medical files. The following violations were observed cited and technical assistance was provided. Violation Number Comment Rule 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. **Four staff member's files did not contain documentation of completed the number of training hours required for according to their education and experience.** .1103(a) 1068 On-going training documentation did not include all applicable information: subject matter, topic area in G.S. 110-91(11), name of training provider, date training was provided, number of hours of training, and name of staff. **Staff on-going training logs were not completed for any of the staff members.** .1106(a) The above violations must be corrected immediately and a compliance letter submitted to my office by March 5, 2025, the letter should be sent to my office by March 5, 2025. The last annual compliance visit was conducted on February 14, 2023. The facility operates with a Five (5) Star License, which was issued on August 1, 2019. Claire Bose, administrator was present and assisted me during the visit. We conducted a walkthrough of the facility. Attendance was taken and observations made. Children were observed participating in routines, having lunch, outdoor free play, teacher directed story time and resting. The last Sanitation Inspection was conducted on July 28,2023. The facility achieved a Superior classification. A partial assessment of child care requirements was conducted this date. I monitored supervision, staff/child ratio, program records, a sampling of children’s records. I monitored staff CBC, emergency trainings, and Health and Safety trainings. I monitored all new staff files. The permit was posted and restrictions maintained. The following violation was observed cited and technical assistance was offered. Violation Number Comment Rule 858 Plastic bags, materials that could be torn apart and toy parts small enough to be swallowed were accessible to children under three years of age. A plastic "grocery" bag was used to store dirty diapers in unlocked storage. .0604(q) The above violation must be corrected immediately, and a letter shall be sent to my office stating how the violation was corrected.
