The provider Mrs. Shareka Kimber was present and assisted me during today's visit. I completed a general walk through of the daycare space, bathroom and outdoor play area. I observed four children resting on their cots upon arrival. The temporary license was issued to this family child care home on 8/22/25 and expires on 2/22/25. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit. The fire drill and the outdoor play inspection were completed on 10/2/25. The provider completed EPR training on 10/9/25 and is still working on getting everything in the Risk Management Portal system. A shelter-in-place was conducted on 9/15/25. The provider does not want to transport children at this time. If anything changes, I informed Mrs. Kimber to let me know so that the vehicle can be looked at for approval. The license was posted along with the NC Summary of the Law. A first aid reference sheet was available, and the daily schedule was posted. Attendance records have been maintained since opening. Keep this information on file for at least one year. The provider and children records were reviewed in detail during today's visit. The following violation was cited during today's visit: Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. This was not available for review in two children's files during today's visit. The letter must address each violation cited. Please send to my email address Marcia.Humphrey@dhhs.nc.gov and be sure to include the following: *Facility Name *Facility ID number *Each Item numbers QRIS INFORMATION: I discussed in detail with Mrs. Kimber the new QRIS system-Pathways to the Stars. I explained to her the three different pathways that she could choose from. It appears that Mrs. Kimber would be interested in Pathway #2, which deals with curriculum. I also informed her that it is very important that she gets all necessary paperwork (original transcript unopened) sent to the WORKS department. Thank you for your assistance during today's visit.
