On July 1, 2024, a partial monitoring was conducted. The facility operates with a Four-Star License, which was issued on November 24, 2021. Upon arrival to the facility, I was greeted by a staff member. I stated the purpose of our visit. The staff member, Ms. Shannon Tincher, assisted me on a walkthrough of the facility. Attendance was taken and observation made. Children were observed, participating in outdoor free play. A partial assessment was conducted today. I monitored staff/child ratio, supervision, program records, children’s records, and staff files. The following violation was observed, cited and technical assistance and resource links have been provided. Violation Number Comment Rule 318 Children between 12 and 24 months of age were grouped with children 3 years of age or older. A three-year old child was enrolled in a classroom with a child less than 24 months old. 10A NCAC 09 .0713(a)(6) The above violation must be corrected immediately, and a letter sent to my office stating how the violation was corrected. Please include with the letter all documentation to support the corrective actions taken. Your letter should be received by my office on or before August 2, 2024. A follow-up visit may be conducted to ensure compliance with child care requirements. You were provided an opportunity to ask questions regarding child care requirements. We discussed that in special circumstances children may be assigned to a “multi-age” group setting. For example, an onsite child's Individualized Education Program (IEP) or in some cases an Individualized Family Service provided by a child’s physician which provides the qualification for the assignment. TECHNICAL ASSISTANCE: 10A NCAC 09 .0713 STAFF/CHILD RATIOS FOR CENTERS (a)(6) children between the ages of 12 months and 24 months shall not be grouped with older children unless all children in the group are less than three years of age. The last annual compliance visit was conducted on July 10, 2023. The facility operates with a Four-Star License, which was issued on November 24, 2021. Upon arrival to the facility, I was greeted by a staff member. I stated the purpose of our visit. The staff member stated that neither of the administrators are present today because they are technically closed this week. However, there are three children present and in the care of two staff members. I took attendance was taken and I observed the one classroom. Children were observed, participating in indoor free play. Emergency information for these three children are available on the online app used by the school. The last sanitation inspection was conducted on June 18, 2024 with a Superior rating. A partial assessment was conducted today, due to the fact that, there are only three children and two staff members present. All other classrooms and the office are closed. The following violation was observed and cited. Violation Number Comment Rule 124 The center did not maintain records as required in rule, and/or were not made available to the Division for review. All records were not available for review. The office was locked and staff did not have access.
