Upon my arrival, I was greeted by the Teachers, the Director arrived during the visit, she assisted me throughout the visit. I conducted a walkthrough of all classroom spaces, documented enrollment and attendance records, and completed observations. Children were observed participating in free play outside on their designated playgrounds. I observed the infants playing freely and interacting with the Teacher. I also observed the children eating lunch consisting of chicken and rice, peas, pears, and milk. The last annual compliance visit was conducted on October 1, 2024. The center is currently operating with a 3-star license issued on December 3, 2019. The most recent sanitation inspection was conducted on May 22, 2025, with the center receiving a Superior rating. Fire drills and playground inspections were reviewed, and the most recent fire inspection was conducted on December 17, 2024. All staff files were monitored along with three children’s files. Your compliance history is currently 95 % this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan was not current the preschool classroom. GS 110-91(12); .0508(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. None of the staff had updated emergency information on file. The last update was done in August 2024. .0701(a) 1052 Staff required to receive on-going training had not completed the required number of hours according to their education and experience. Three staff did not have documentation of completed on going training on file. .1103(a) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. One child did not have a medical report on file see childrens records form GS 110-91(1);.0302(d)(2); .0304(g) 1325 Parent’s statement includes the child’s name and date of enrollment and the date the parent signed the statement. Three of the children's files that were monitored did not have an enrollment date on the discipline policy, see children's records form. .1804(b) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Medical information was not in a separate file. .0701(d) The above violations must be corrected immediately, and a letter must be sent to me no later than October 3, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. If I do not receive your letter by October 3, 2025 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the violations have not been corrected or addressed. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/ , on the home page under the ‘Services’ tab. Technical Assistance Go through all staff files and remove any medial information and put the medical into a new file for each staff person. Keep Insurance documentation in the center or in the vehicle. You should always be prepared for your annual compliance visit at a month before your last one was completed. Staff and training worksheets should be updated as changes occur. An email was sent to you as a reminder of your upcoming visit on July 29, 2025. Send your staff and training worksheets to me by September 24, 2025. If not received a follow-up visit will be conducted. Preparing for North Carolina's New QRIS System The Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Starting later this year, providers will be able to choose from three star-rating pathways—Program Assessment, Classroom & Instructional Quality, or Accreditation—allowing you to align your strengths with the rating method that best fits your program. Now is the time to evaluate your current practices, staff credentials, curriculum use, and classroom interactions to determine which pathway is the best fit. Programs are encouraged to begin preparing for updates to staff qualification standards and assessment tools. While existing star ratings will remain valid for now, all new star ratings will be subject to the new system. Your program’s quality should reflect not just compliance—but a commitment to best practices, child development, and continuous improvement. Stay informed, stay proactive, and take advantage of technical assistance and training opportunities as they become available. A provider meeting will be held September 29, 2025, 1pm-3pm, at Impact Alamance in downtown Burlington, please plan to attend this meeting to get further information about the new QRIS system. The last annual compliance visit was conducted 10/1/24. Currently this program operates with a three (3) Star License, issued 12/3/19. The facility earned five (5) points in staff education, two (2) points in program standards, and one (1) quality point for education options met. The center director, Latelia Fountain, arrived after my arrival and assisted me. I conducted and unassisted walkthrough of the center. The last sanitation inspection was completed 10/28/24 with a ‘Superior’ classification and 3 demerits. The last fire inspection was conducted on 12/1/23. Children were observed playing in activity areas and routine care activities. A partial assessment of childcare rules was conducted today. The following violations were observed today: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The current inspection was received 12/17/24. Currently this program operates with a three (3) Star License, issued 12/3/19. The facility earned five (5) points in staff education, two (2) points in program standards, and one (1) quality point for education options met. The center director, Latelia Fountain, arrived after my arrival and assisted me. I conducted and unassisted walkthrough of the center. The last sanitation inspection was completed 2/8/24 with a ‘Superior’ classification and 3 demerits. The last fire inspection was conducted on 12/1/23. Children were observed playing in activity areas and routine care activities. Transportation is used for school-age children and was monitored today. The center used a Ford passenger van (PEZ 1671) to transport children. A full assessment of childcare rule was conducted today. The following violations were observed today: Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Documentation of review was not on file for one child. GS 110-102 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. A drill was not recorded for July 2024, September 2024. .0604(u);.0302(d)(8) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information. Documentation of review was not on file for one child. .0608(b)(1-6) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements.
