The last annual compliance visit was conducted on January 20, 2025. The center is currently operating with a 4-star license issued on July 21, 2022. Upon arrival, I was greeted by the Director and Assistant Director. At the time of the visit, children were observed finishing lunch and preparing for nap and rest. A partial assessment of child care requirements was completed during today’s visit, and one violation was cited. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. I the classroom for 2 year olds, there were three outlets that were uncovered around the classroom. 10A NCAC 09 .0604(c) The cited violation must be corrected immediately. A letter must be submitted to me no later than June 11, 2026, describing how each item was corrected and outlining the plan to maintain compliance with the requirement. Technical Assistance The Assistant Director inquired about the safe sleep policy on the DCDEE website, which the center is currently using. She asked about the “I can roll” statement. I explained that if this option is selected on the safe sleep policy, the practice must be implemented, as this is best practice. She also asked about incident reports. Anytime a child is injured, an incident report must be completed and documented on the incident log. Rated License Information The Director has chosen Pathway 2. The following topics were discussed: • Enhanced ratios, which the program is already meeting. • Family and community engagement – please send me your foundational practices. • Development of a facility-wide CQI plan using the ITERS-3/ECERS-3 to develop your goal. • Development of individual staff CQI plans using the ITERS-3/ECERS-3 to develop goals. • The Director will review the Early Learning Matters Curriculum and Desired Results for Children and Families formative assessment to determine whether they are appropriate for the program. • One assessment must be shared with families once per year. • Coaching/mentoring requirement – mentoring hours must be completed for the Administrator and Lead Teachers and must be documented. • Staff education will be reviewed for all staff. • Please send me a list of staff, their positions, and birth dates within 30 days. Upon my arrival, I was greeted by the Assistant Director, she assisted me throughout the visit. I conducted a walkthrough of all classroom spaces, documented enrollment, attendance records, and completed observations. The children were observed participating in free play in their classrooms. The children had fish sticks, corn, peaches, bread and milk for lunch. The children were also observed resting. The last annual compliance visit was conducted on February 5, 2025. The center is currently operating with a 4-star license issued on July 21, 2022 . The most recent sanitation inspection was conducted on July 10, 2025, with the center receiving an Superior rating. The last fire inspection was documented, February 10, 2025. Fire drills and playground inspections were monitored. All staff files, including 2 new staff files were monitored along with two children’s files. Your compliance history is currently 88% this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 319 Staff/child ratios applicable to a classroom, were not posted in each classroom. The ratio form was posted however the information was not correct and did not match the required ratios of the license. .0713(a)(10), (c) & (f)(3); .2818(e) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. One staff S. Briggs, did not have updated emergency information on file. .0701(a) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). One staff W. Snipes did not have a current criminal background check. Her last check expired in August 2025. G.S. 110-90.2(b) & .2703(n)&(o) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. One staff, S. Briggs did not have a current health questionnaire on file. .0701(d) The above violations must be corrected immediately, and a letter must be sent to me no later than February 3, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Technical Assistance On-going training is calculated using the employment date for the staff. For example, if the date of hire is 6/2024, only training taken between 6/2024 and 6/2025 will be counted during the next annual compliance visit and the log and certificates should be together in the file. Health and safety training, CPR/FA, SIDS, BSAC, Playground safety, should always be in the file. Your training documentation was organized during today’s visit. Ms. W. Snipes has an expired criminal background check, the background check should be completed before Ms. Snipes can care for children. The background checked expired in August 2025. The ratio forms were posted however the information was not correct. The center is meeting enhanced ratios. The ages need to be updated as well. I discussed this with the Assistant Director during today’s visit. QRIS The Director has requested a technical assistance visit to review the QRIS and discuss which pathway will be the best for the center. RESOURCES The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These trainings are free, and participants will receive training certificates when completed. Expired medication was observed in the infant classroom. This was corrected during the visit. .0803(12) You were provided an opportunity to ask questions regarding the interpretation and implementation of all child care requirements. We discussed each of the violations documented this date: Technical Assistance: Prescription medication should be labeled with the child’s name; date the prescription was filled; name and contact information of the prescribing health professional; expiration date; medical need; instructions for administration, storage, and disposal; and name and strength of the medication. Further, labeled medications, including (over-the-counter) brought to the facility by the parents or guardians must be maintained in the original container. The labels should include the child's name; dosage; relevant warnings as well as specific; and legible instructions for administration, storage; and disposal, etc. -Section 0900 NUTRITION REQUIREMENTS: 10A NCAC 09 .0901(b) When food is prepared by or provided by the center, menus for nutritious meals and snacks shall be planned at least one week in advance. This facility operates with a Four-Star License which was issued on July 21, 2022. A check of the Secretary of State website shows the business as “Active and Current”. Rasanda Mitchell, the owner was present and assisted me during the visit. A walkthrough of the facility was conducted. Attendance was taken and observations made. Children were observed participating in indoor free play and routines. The center's last Fire Inspection was conducted on January 24, 2024 and was approved for daytime and night care. The last Sanitation Inspection was conducted on June 28, 2023. The facility achieved a Superior classification. A full assessment of child care requirements was conducted this date. The permit was posted, restrictions maintained. I monitored supervision, staff/child ratio, program records, a sampling of children’s records. I monitored all staff files. The following violations were observed cited and technical assistance was offered. Expired medication was observed in the infant classroom. This was corrected during the visit. .0803(12) You were provided an opportunity to ask questions regarding the interpretation and implementation of all child care requirements. We discussed each of the violations documented this date: Technical Assistance: Prescription medication should be labeled with the child’s name; date the prescription was filled; name and contact information of the prescribing health professional; expiration date; medical need; instructions for administration, storage, and disposal; and name and strength of the medication. Further, labeled medications, including (over-the-counter) brought to the facility by the parents or guardians must be maintained in the original container.
