Restrictions include Daytime care onl; Meets reduced ratios; Meets enhanced space. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 97% prior to today’s visit. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children playing in their outdoor learning environment and free play. I observed materials and activities that were available to the children in the classrooms. Your program was also monitored for compliance with implementing an approved curriculum as required for all four- and five-star licensed facilities where four-year-old children are enrolled. Your facility uses Creative Curriculum. The last annual compliance visit was conducted on October 2, 2025. A sanitation inspection was completed on October 30, 2025, with a Superior classification. The last fire inspection was conducted on September 10, 2025. Program records and required postings were monitored. A fire drill was conducted on March 24, 2026. A shelter in place drill and lock down drill was documented on March 25, 2026. An outdoor inspection was documented on March 3, 2026. Staff files and children’s records were monitored per DCDEE procedures. Nutrition requirements were monitored per DCDEE procedures. Storage of hazardous items was monitored today. Storage and administration of medication were monitored. Medication authorization was monitored. The following violations were cited during today’s visit: Violation Number Comment Rule 1048 All staff did not successfully complete certification in First Aid appropriate to the age of children in care. Verification of staff completion of First Aid training from an approved training organization was not in the staff file. Two new staff members did not complete and approved first aid course. .1102(c) 1049 All staff did not successfully complete certification in CPR training appropriate to the age of the children in care. It streamlines tracking for 5-year recheck requirements, tracks employee status, and prevents unqualified individuals from working with children. • Completing CPR and first aid training is essential in childcare to provide immediate, life-saving care during emergencies like choking, cardiac arrest, or accidents. These skills allow caregivers to manage emergencies until professional help arrives, increasing survival chances. The annual compliance visit was conducted on October 2, 2025. Upon arrival at the facility, I stated the purpose of my follow-up visit. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Erika Nunnally, Director. A partial monitoring was conducted today. I monitored a sample of staff and children’s files. Program files and program requirements, were monitored during the previous annual compliance visit. The following violations were cited during today’s visit: Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff member started on 9/2/2025 and did not receive a medical report until 9/10/2025. 10A NCAC 09 .0701(a) 1033 On or before the first day of work, all staff, including the director and individuals who volunteer more than once per week did not provide results indicating that they were free of active TB and/or TB test or screening was older than 12 months. A staff member started on 9/2/2025 and their tb test was dated 2/16/2024. .0701(a) 1890 Each staff member did not have the required medical report, proof of tuberculosis test or screening and/or completed health questionnaire in a medical file, maintained separately from the staff member's individual personnel file. Upon my arrival I was greeted by the Lead Teacher, Latoya Tinnin, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, attendance was documented, and observations were made. Children were observed participating in free play outside on their playground and participating in free play in their classrooms. Some children were observed eating lunch which consisted of chicken, tortillas, bananas, cucumbers and milk. The last annual compliance visit was conducted November 17, 2023. The center is operating five star license that was issued July 18, 2024. The license was earned with 6 points in program standards, 7 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted April 12, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. All the required documents were posted. The Director was out of the office during today’s visit, staff files will be monitored at a later visit. Ten children’s files were monitored. Your compliance history is 98%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Out of the ten children's files monitored, one child did not have receipt of the summary of the law. GS 110-102 428 A current activity plan was not posted for each group of children for reference. In space 2 a current activity plan was not posted. GS 110-91(12); .0508(a) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Notification was not given to four of the ten children whose files were monitored. .0604(j) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information.Out of the ten children's files monitored, one child did not have receipt of the shaken baby policy. .0608(b)(1-6) The above violations must be corrected immediately, and a letter must be sent to me no later than November 15, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. Upon my arrival I was greeted by the Lead Teacher, Latoya Tinnin, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, attendance was documented, and observations were made. Children were observed participating in free play outside on their playground and participating in free play in their classrooms. Some children were observed eating lunch which consisted of chicken, tortillas, bananas, cucumbers and milk. The last annual compliance visit was conducted November 17, 2023. The center is operating five star license that was issued July 18, 2024. The license was earned with 6 points in program standards, 7 points in education and 1 quality point. Permit restrictions were in compliance. The last sanitation inspection was conducted April 12, 2024 the center received a Superior rating. Fire drills and playground inspections were monitored. All the required documents were posted. The Director was out of the office during today’s visit, staff files will be monitored at a later visit. Ten children’s files were monitored. Your compliance history is 98%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. The following violations were observed during today’s visit. Violation Number Comment Rule 114 A summary of the NC Child Care Law was not given to a parent of every child enrolled in the center. Out of the ten children's files monitored, one child did not have receipt of the summary of the law. GS 110-102 428 A current activity plan was not posted for each group of children for reference. In space 2 a current activity plan was not posted. GS 110-91(12); .0508(a) 1851 The operator did not notify the parent of each child enrolled in writing of the smoking and tobacco restriction. Notification was not given to four of the ten children whose files were monitored. .0604(j) 1908 A child's file did not have a statement with parent signature acknowledging receipt and explanation of the Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy and/or the acknowledgement did not have all the required information.Out of the ten children's files monitored, one child did not have receipt of the shaken baby policy. .0608(b)(1-6) The above violations must be corrected immediately, and a letter must be sent to me no later than November 15, 2024 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. If I do not receive your letter by November 15, 2024 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules.
