The license was issued on August 16, 2018. The last annual compliance visit was conducted on April 11, 2024. Upon arrival at the facility, I checked in at the office. Once checked in, I proceeded to the first preschool classroom; I was greeted by Ms. Walters, the lead teacher, and her assistant, Ms. McGraw, who assisted me during today’s visit. Attendance was taken and observations made. The children were observed participating in indoor and outdoor free play, teacher’s directed activities. Routines, having lunch and preparing for rest. The last Fire Inspection was conducted on September 10, 2024, and was approved for day time use only. The last Sanitation Inspection was conducted on March 19, 2025, and the facility achieved a superior classification. A full assessment of child care requirements was conducted during today’s visit. I monitored program requirements, including postings, permit with restrictions maintained, and the summary of laws. I monitored supervision, staff/child ratio, and general safety. I monitored program records, a sampling of children’s records, and staff files. The following violations were observed cited and technical assistance was provided. Violation Number Comment Rule 1068 On-going training documentation did not include all applicable information: subject matter, topic area in G.S. 110-91(11), name of training provider, date training was provided, number of hours of training, and name of staff. .1106(a) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. **Documentation of regular quarterly lockdown drills was not available. I observed documentation of September lockdown drill in classroom B28.** .0604(u);.0302(d)(8) The above violations must be corrected immediately, and a compliance letter drafted stating how you corrected the item and when the violations were corrected. The letter should be submitted on your business letterhead. Please reference the type of visit, the visit date, and the item number cited for the violation. Please sign and submit the letter. You may submit to my email at anita.c.graham@dhhs.nc.gov. The letter should be sent to my office no later than April 9, 2025. If the letter is not received by April 9, 2025, a return visit may be conducted to ensure the violations have been corrected or addressed. You were provided with an opportunity to ask questions regarding child care requirements. We discussed the areas of non-compliance. You have stated that the CPR/FA training has been scheduled. Please ensure the training is completed by the due date. If more time is needed, please be sure to reach out to me. TECHNICAL ASSISTANCE: Please complete the in-service training logs each year to include all relevant trainings and also ensure the certificates attached. We will count all in-service training related to preschool topics, including health and safety trainings. Please ensure that all quarterly lockdown drills are documented whenever practiced. The last Annual Compliance Visit was conducted on April 18, 2023. The center currently operates with a Five Star Child Care License that was issued on August 16, 2018. The last Fire Inspection was conducted March 22, 2024 and the facility was approved for daytime care only. The last Sanitation Inspection was completed March 19, 2024 and achieved a “Superior” classification. Upon arrival, I signed in at the school’s main office and then proceeded to the preschool classrooms where I was greeted by Ms. Lashunna Yancey, lead teacher. There are two Pre-K classrooms at this location. Ms. Yvonne Walters is the lead teacher in the second classroom. During the visit attendance was taken and observations made. Children were observed participating in indoor free play, teacher directed activities, having lunch, resting and departing for the day. This facility has two classrooms which currently serve twenty-seven NCPK children. The NCPK requirements were monitored during today’s visit. Five NC Pre-K children's records were monitored, and each contained a health assessment and a DIAL-4 screening. A full assessment of child care requirements was conducted during this date. I monitored program requirements, including postings such as, permit with restrictions maintained, summary of laws, daily schedule, First-Aid posters and the staff/child ratio charts. I monitored program records, a sampling of children’s records, staff personnel files, medical files and on-going trainings. The following violation was observed cited and a technical assistance was offered. It is always your responsibility to maintain compliance with all of the applicable laws and rules . The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Both are available on the DCDEE website https://ncchildcare.ncdhhs.gov/, on the home page under the ‘Services’ tab. Please be aware that new rules came into effect in January of this year. (b) Each center shall have a record of countable technical assistance in which each staff member participates, including copies of documentation, observations and evaluations provided by the person who has been endorsed by the NC Institute for Child Development Professionals as a technical assistance provider. That record shall include the dates and times of technical assistance with staff, names of all participating staff members and the name of the technical assistance provider. This documentation shall be on file and current.
