The last annual compliance visit was conducted 11/12/25. Currently this program operates with a five (5) Star License, issued 6/3/24. The facility earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point for education options met. Courtney Solis, lead teacher, assisted me today. I conducted an unassisted walk-through of the classroom. The last sanitation inspection was completed 4/16/26 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 9/4/25 and 4/13/26. Children were observed playing in centers, eating lunch, large group activities, and routine care activities. This facility has one NCPK classroom with twelve children enrolled. NC Pre-K requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Two NC Pre-K children's files were monitored and all contained a health assessment within 30 days, an ASQ3 screening within 90 days, and evidence of each child's on-going progress. The teachers use Creative Curriculum and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a monthly newsletter, daily notifications via class DOJO, and are able to volunteer throughout the school year. The teachers also have parent meetings and conferences three times a year. The current ECERS-R scores were received 5/23/24. The facility received a score of 5.59. Transportation is not provided. Today a partial assessment of childcare requirements was conducted. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. Substitute teacher T. Brown did not have a qualification letter. G.S. 110-90.2(b) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Solis. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or before 6/10/26. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. CONSULTATION: Any teacher that serves as a substitute must have a valid criminal background letter through DCDEE. A copy of the letter must be on file for review. Ms. Brown cannot return as a substitute in an NCPK classroom until she has obtained a valid qualification letter. If a staff member substitutes in a classroom for more than ten days, the staff member must complete the required health and safety trainings including CPR/First Aid. The days do not have to be consecutive. A substitute cannot be left alone with children in a classroom with children unless they have CPR/First Aid for childcare. I reviewed the process for obtaining and NCID and criminal background letter with Ms. Brown today. During the fall monitoring visit, we will discuss the requirements for the ERS assessment. This assessment will be completed in the Spring 2027. It is recommended when you set up your room in the fall, assess the activity areas to determine any items that you will need to purchase. You will need a copy of the ECERS 3 book. This visit was conducted by Turkessha Casterlow, Childcare Consultant with Courtney Solis, Lead Teacher. Your program currently operates with a Five Star license effective June 3, 2024. The program’s compliance history was 94% prior to today’s visit. Today, the following items were monitored: • Supervision • Staff / Child Ratio • CPR / First Aid • Special Training • CBC Qualification • ITS – SIDS • Emergency Medical Care Plan • Administration of Medication • Storage of Hazardous Products • Storage of Medication • General Safety • Discipline • Adequate / Approved Space • Program Records • License Posted • Permit Restrictions The license was observed, and the restrictions were found in compliance. A walk-through of the facility was completed today. Indoor and outdoor areas were observed. Staff/child ratio and space capacity were monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children in free choice activities. One new staff member was reported at this program. Files for the new staff were reviewed. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, Playground Safety, ITS-SIDS, Special Training including Health and Safety Training and Criminal Background Checks. Storage and administration of medication were monitored. Medication authorization was monitored. Storage hazardous items were monitored today. A playground inspection was recorded May 14. 2025. A fire drill was recorded May 19, 2025. A lock down drill for the facility was recorded April 28, 2025. The most recent fire inspection was on March 12, 2025. I observed a sanitation inspection that was last conducted on January 27, 2025, earning a Superior classification. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 716 All stationary equipment, more than 18 inches high, was not installed over protective surfacing. The playground mulch measured at 4 inches in the fall zone. .0605(j) Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. You must correct the violations cited during today's visit immediately and send Anita documentation verifying compliance on or before June 16, 2025. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation You may submit the letter to anita.c.graham@dhhs.nc.gov. Additional monitoring visits may be conducted during the year to monitor compliance with applicable childcare requirements. Technical assistance discussed during today’s visit: • Having an adequate surface in a fall zone is crucial in childcare settings to protect children from injuries during falls from play equipment. Fall zones are areas surrounding equipment like swings, slides, and climbing structures where protective surfacing is required to cushion falls. Proper surfacing reduces the risk of severe injuries, especially head injuries, that can occur from falls. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • The Health and Safety Trainings are offered on the DCDEE Moodle format. On the Division’s website look under the “SERVICES” tab for “DCDEE Moodle”. These training courses are free, and participants will receive training certificates when completed. E. Kearns must complete this training by April 15, 2026. • E. Kearns must complete Recognizing and Responding to the Suspicions of Child Maltreatment and CPR/First Aid by July 15, 2025.
