This visit was conducted by Turkessha Casterlow, Child Care Consultant, with Tashe Atwater, Additional Caregiver. Kaye Adkins, Licensing Supervisor, assisted me during today’s visit. Your program currently operates with a Three Star license. Restrictions include Daytime care only; Maximum of 5 preschool children at any time; Children in care on ground level only. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 94% prior to today’s visit. Today, the following items were monitored: • Supervision • Capacity • CPR / First Aid • ITS-SIDS • CBC Qualification • General Health Administering Medication • Storage of Hazardous Products • Storage of Medication • General Safety • General License Requirements • Program Records • License Posted • Fire Safety • Sanitation The license was observed, and the restrictions were found in compliance. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children during personal care routines and free play. Limited monitoring of staff files was conducted for the purpose of reviewing training regarding CPR/First Aid, EPR, ITS-SIDS, and Criminal Background Checks. No medications were observed nor reported. Storage hazardous items were monitored today. We were unable to monitor safety drills, fire drills, and playground inspections. There are two pets inside the home. The program does not provide transportation. The following violations were cited during today’s visit: Violation Number Comment Rule 920 Records were not made available for review. Safety drills and additional caregiver records were not available for review. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed the owner status of this facility. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on December 10, 2025. A shelter in place drill and lockdown drill was documented on December 10, 2025. It establishes a baseline for health and helps staff recognize changes in a child's well-being, ensuring they can provide appropriate care for physical, emotional, and developmental needs. Additional Information/Consultation: • You must notify your consultant 30 days prior to changing the ownership status of your facility. • The health and safety training is required to be completed within the first year of employment, except for Recognizing and Responding to Suspicions of Child Maltreatment, CPR and First Aid training. These three (3) courses must be completed within ninety (90) days of employment. Following the initial completion of the health and safety training must be completed every five years of employment following the initial completion. There is a training log available on the Division’s website to document the completion of the training. All topics must be covered. The Recognizing and Responding to Suspicions of Child Maltreatment training can be found at https://www.preventchildabusenc.org/. Your Health and Safety Trainings must be completed by January 16, 2026. These classes will fulfill the required 10 training hours that you need for the 2025-2026 training year. • During our visit we discussed organizing your operator and children’s file. I left checklist to assist you with organizing your files. Be sure to keep your medical report, tb, and health questionnaires are kept separate from your operator file. These forms can be found on the DCDEE website. • Preparing for North Carolina’s New QRIS System North Carolina’s Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Soon you will begin to choose from three star-rating pathways: Program Assessment, Classroom & Instructional Quality Accreditation. This new approach allows programs to align their strengths with the pathway that best fits their operations. Now is the time to prepare. Programs should begin evaluating staff qualifications including updating staff education in NC Works, curriculum, and classroom interactions to determine the most suitable pathway. Updates to staff qualification standards and assessment tools are expected, and while current star ratings remain valid for now, all new ratings will be issued under the revised QRIS system. Your program’s quality should reflect not only compliance, but also a strong commitment to best practices, child development, and continuous improvement. Providers are encouraged to take advantage of available technical assistance and training opportunities. Your program currently operates with a Three Star license. Restrictions include Daytime care only; Maximum of 5 preschool children at any time; Children in care on ground level only. The license was observed, and the restrictions were found in compliance. The program’s compliance history was 93% prior to today’s visit. A walk-through of the home was completed today. Indoor and outdoor areas were observed. We tested the smoke detector during today’s visit, and it worked. Capacity was monitored as indicated on the attached worksheet. Supervision was adequate during the visit. I observed staff/child interactions today. I observed children playing in their outside learning environment, group time, and free play. I observed materials and activities that were available to the children in the classrooms. This is the first annual compliance visit conducted at this facility. A Verification of Required Information for Operator and Additional Caregivers form was completed during the visit for the operator and additional caregiver. A FCCH and Centers Located in a Residence Operator’s Statement of Responsibility form was reviewed and signed during the visit. We discussed the owner status of this facility. Individuals living in the home were discussed and are recorded on the Verification of Required Information for Operator and Additional Caregiver form. Be reminded that everyone living in the home must have criminal record checks conducted every five (5) years. You may go to the Division of Child Development & Early Education's website at https://ncchildcare.ncdhhs.gov and complete this process six (6) months prior to the expiration date on your qualifying letter. Program records and required postings were monitored. A fire drill was conducted on December 10, 2025. A shelter in place drill and lockdown drill was documented on December 10, 2025. The provider Ms. Ismay James was present and assisted me during today's visit. I completed a general walk through of the daycare space, bathroom and outdoor play area. I observed seven children present in care. Of the seven children four children three and under watching an educational program on the television. Ms. Ismay was holding the infant child who was falling asleep. The temporary license was issued to this family child care home on 1/16/25 and expires on 7/16/25. The Verification of Required Information for Operator and Additional Caregiver form was completed during today's visit and reviewed with the provider). The fire drill and outdoor play inspection will be completed on 5/14/25. Ms. James has not completed the EPR training at this time but does plan on completing this before 1/16/26. I informed Ms. James that once completed she will have four months to place it in the Risk Management Portal. Once completed I informed Ms. James that I or her local consultant will need to see this training certificate showing the completion date. The provider has not been approved for transportation at this time but if decides to in the future I informed her that before taking the children in any vehicle, to please contact me or the local consultant to make sure the vehicle is approved with all inspections and all necessary paperwork for transporting is available in the vehicle. The license was posted along with the NC Summary of the Law. A first aid reference sheet was available, and the daily schedule was posted. Attendance records have been maintained since opening. Keep this information on file for at least one year. The provider and children's records were reviewed during today's visit. The following violations were cited: Violation Number Comment Rule 101 Number of children exceeded number allowed. Upon arrival there were seven children present in care ranging in age from 0-3 years of age. GS 110-91(7)(b) & 110-86(3)(b) 543 Screen time was not prohibited for children under the age of three. The operator did not offer alternative activities for children under the age of three years. Upon arrival four children under the age of three were watching an educational show on television upon arrival. .1718(c) In order to comply with NC Child Care Laws and Rules any violation cited must be corrected IMMEDIATELY. A Compliance letter must be sent by June 16, 2025. The letter must address each violation cited. The current license states that this family child care home operates first shift only with a capacity of eight children (five preschool ages 0-5 and three school age children grades kindergarten-fifth grade). One of the restrictions that was pointed out to Ms. James was where it states maximum of five preschool children at anytime. I told Ms. James that she would need to drop down to the correct capacity as soon as possible. Ms. James stated that she will talk to the parents and drop to the correct capacity. I also discussed screen time with Ms. James and informed her that any children under the age of three are not permitted to participate in screen time. For children three and up they are permitted to have screen time but it is limited to thirty minutes a day with no more than 2.5 hours per week. The provider corrected this violation while I was present. Also make sure to document this on the screen time from which is available on the DCDEE website. Lastly, I discussed with Ms. James as a reminder the importance of making sure all necessary trainings have been completed within the required timeframe. In addition, I discussed in detail with Ms. James the children records and the importance of making sure all paperwork has been returned according to NC Child Care Laws and Rules. RATED LICENSE INFORMATION I discussed with Ms. James in a prior conversation the rated license process. During today's visit Ms. James handed me her reflective self-study, I will review and if I have any additional questions or concerns I informed Ms. James I will let her know. TECHNICAL ASSISTANCE *Annual in-service training hours are due by 1/17/26. I would recommend that you complete your training three months prior to the above date.
