Upon my arrival I was greeted by the Director, Lisa Moore she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance were documented, and observations were made. The infants were observed participating in free play in their classroom, one infant was observed napping in the crib. The older children were observed participating in free play in their classrooms and outside. The children were observed eating lunch which consisted of peas, peaches, chicken nuggets, rice and milk. The last annual compliance visit was conducted on September 18,2024. The center is operating with a 4-star license that was issued December 17, 2018. The last sanitation inspection was conducted July 9, 2025, and the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted on December 12, 2024. New staff files were monitored along with six children’s files were monitored. Your compliance history is 94%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks.One staff orientation was not signed by the employee. .1101(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. Staff S.Poage did not have annual evaluation on file. 10A NCAC 09 .0514(f) 1320 Children's records that include an application for enrollment, medical and immunization records, and permission to seek emergency medical care was not on file for each child. Two children did not have immunization records on file and one child did not have a medical report on file. See children's records form. GS 110-91(1);.0302(d)(2); .0304(g) 1874 The Prevention of Shaken Baby Syndrome and Abusive Head Trauma policy was not reviewed with new staff prior to providing care with children and/or a signed acknowledgement with all the required information was not maintained in the staff person's file. One staff did not have a signed acknowledgement on file for Prevention of Shaken baby syndrome and Abusive trauma policy on file. .0608(d)(1-4) The above violations must be corrected immediately, and a letter must be sent to me no later than August 11, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. Technical Assistance The temperatures have been very hot, review the following information. 1.Safety Check & Maintenance Inspect equipment: Check for loose bolts, rust, splinters, or cracks on play structures, swings, and slides. Surface check: Ensure mulch, rubber mats, or sand are evenly spread and thick enough to cushion falls. Fence & gate: Inspect for damage or sharp edges and make sure gates latch securely. Pest control: Check for bees/wasp nests, ant hills, or signs of other pests. 2. Sun Protection Shade structures: Install or check canopies, sails, or umbrellas over play areas and seating. Natural shade: Trim trees for safety but keep them healthy to provide extra sun coverage. Sunscreen policy: Have a plan for applying sunscreen (with parent permission) and ensure staff follows it. 3. Hydration Station Water access: Ensure water is available with disposable cups. Encourage water breaks: Schedule regular hydration times during play. 4. Outdoor Activities & Toys Summer-appropriate toys: Add water tables, sand toys, sports equipment, or ride-ons. Rotate toys: Keep the environment fresh and engaging by rotating outdoor toys. Nature-based play: Include items like gardening tools, pots, or sensory bins with natural materials. 5. Cleanliness Sanitize outdoor toys regularly—especially anything children put in their mouths. Tidy up landscaping: Remove any debris, broken branches, or trash. 6. Safety from Heat Playtime schedule: Shift outdoor play to cooler times (morning or late afternoon). This program is operating with a Four(4)-Star License, which was issued on December 17, 2018. A check of the Secretary of State website shows The First Baptist Church of Burlington as Current-Active. Upon arrival to the facility, I was greeted by the director, Lisa Moore. I stated the purpose of my visit and we conducted a walk through of the facility. The children were observed participating in outdoor free play, routines, and resting. The last Fire Inspection was conducted on December 13, 2023. The last Sanitation Inspection was completed on June 24, 2023 with a superior rating. A full assessment of child care requirements was conducted this date. I monitored the indoor and outdoor learning environment. I observed the permit posted, restrictions maintained, supervision, staff/child ratio. I monitored program records, a sampling of children’s records, four new staff files and all staff trainings. The following violations were observed, cited and technical assistance resource links were provided. Violation Number Comment Rule 535 Any excess formula, thawed human milk and/or bottled beverage were not discarded or returned home at the end of each day. A bottle of baby formula, dated 9/6 was observed in the refrigerator in the infant room. 15A NCAC 18A .2804(d) 1021 Individual responsible for a group of children did not meet the preservice requirements for a lead teacher or teacher. A staff member listed as a teacher assistant, and caring for infants, does not have a High School Diploma or a GED. 10A NCAC 09 .0710 (a-b) The above violations must be corrected immediately. A corrective action letter must be drafted describing how and when the violations were corrected. Please include the name and identification number of your facility, as well as your electronic signature. The letter must be received by my office on or before October 1, 2024. You may email it to anita.c.graham@dhhs.nc.gov or it may be mailed to the following address: PO Box 665, Pleasant Garden, NC 27313. If I do not receive your letter by October 1, 2024, a return visit to your program may be necessary to ensure the violations have been adequately corrected. Additional violations may be documented. TECHNICAL ASSISTANCE: During today’s visit a staff member caring for children in the infant room, who has not obtained the high school diploma, nor the GED. This preservice requirement prohibits staff without one of these documents, shall not be left alone with children, nor can she be counted in staff/child ratio. Further, since the staff member is older than 17 years, they do not meet the requirement to serve as a teacher’s aide. You have stated that the staff member is enrolled in the GED online program through Alamance Community College. You have stated that you will allow the staff member to complete that online coursework today during the work day, and request testing for the GED. We discussed that the staff member may serve in other non-caregiving capacities, i.e. as the cook, office aid, or cleaning until the class is complete and documentation is attained. Prepared powdered formula that has not been given/served to an infant should be covered, labeled with time and current date of preparation and child’s full name, and may be stored in the refrigerator for up to twenty-four hours. Bottles not served in this time frame shall be discarded or returned to the parents at the end of the day. The date observed on a infant bottle today was 9/6/. It may be necessary to re-check the dates prior to bottles being placed in the classroom refrigerator by the parents. This may ensure the staff are able to intercept out of date labeling. Thereby reducing the risk of inaccuracy or feeding the infant unsanitary or incorrect formula. You were provided an opportunity to ask questions regarding the interpretation and the implementation of childcare requirements applicable to the facility. It is the childcare operator’s responsibility to be familiar and up to date on the most current child are rules. Child Care Rulebook as well as the Provider Forms and Documents can be accessed by going to the DCDEE Website, https://ncchildcare.ncdhhs.gov/. AGENCY NEWS: The Division of Child Development and Early Education in partnership with Child Care Services Association has released the latest statewide study of the early care and education workforce. Top findings from the report include: • The early childhood workforce often earns too little to meet their basic needs. • More teachers are leaving their jobs, and many, plan to leave the field altogether. • Child care stabilization grants likely reduced the loss of child care centers during this time period. However, there was a significant decrease in family home providers. • Many teachers are highly educated, but overall have fewer years of education than in 2019. • Most teachers are female and disproportionately Black/African American. Sandra Scott, Assistant Director, accompanied me during a walk-through of the facility. During the visit, I discussed the allegations with Ms. Scott and 2 additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed and/or confirmed during today’s visit. Violation Number Comment Rule 852 Incident reports were not completed each time a child was injured, it did not include all the information required in rule, it was not signed by the parent and/or it was not maintained in the child's file. Facility management was unable to provide an incident report from 5/25/2023 upon request, for a child who was still enrolled. .0802 (e) 853 Incident logs were not completed and maintained as required. An incident report for one child was not added to the facility's incident log in May 2023. .0802(g)(1-6) 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A staff member grabbed children by their arms to make them sit down, and to manage their behavior, on more than one occasion. G.S. 110-91(10) 1876 A child was yelled at, shamed, humiliated, frightened, threatened or bullied. A staff member yelled at children to manage their behavior on more than one occasion. .1803(a)(9) Violations must be corrected immediately.
