Upon my arrival I was greeted by the Director, Bridgette Cannon, she assisted me during today’s visit. I completed a walk-through of the classroom spaces, enrollment and attendance were documented, and observations were made. The children were observed participating in free play and participating in small group time activities. One group of children were observed eating breakfast consisting of cereal, milk and fruit juice. The school age children were observed playing outside on the playground. The last annual compliance visit was conducted on September 11,2024. The center is operating with a 5-star license that was issued October 27, 2022. The last sanitation inspection was conducted March 12, 2025, and the center received a Superior rating. Fire drills and playground inspections were monitored. The last fire inspection was conducted on June 13, 2025. There are no new staff since the last visit. Your compliance history is 99%, this may change following today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violations during today’s visit. Violation Number Comment Rule 115 A summary of the NC Child Care Law was not posted in a prominent place in the center. The summary of the law was not posted. G.S. 110-102 415 A current schedule was not posted for each group of children for reference. In two classrooms, a current schedule was not posted. GS 110-91(12);.0508(a) 428 A current activity plan was not posted for each group of children for reference. In three classrooms, a current activity plan was not posted. GS 110-91(12); .0508(a) The above violations must be corrected immediately, and a letter must be sent to me no later than August 29, 2025 stating how each item was corrected and a plan to maintain compliance with the requirement. The following information must be included in your signed and dated compliance letter: • the name of your center • facility ID number • date • title of the person who signs the letter • each violation number(s) • describes accurately and in detail how and when you corrected each violation and your plan to keep each item in compliance. Consultation During today’s visit your staff and training worksheets were not completed, I have emailed a blank copy of the form to you during today’s visit. Keep a blank copy of this form on file. Send the completed forms to me by 8/22/25. Remember to sign the forms before sending them. Technical Assistance In three classrooms, a current activity/lesson plan was not posted, and no schedule was posted. Although this is the last day of your summer program activity plans should remain posted until the end of the day. It is also fine if the last day is a free play day, however this must be noted on the activity plan. The facility has two buses that are used for transportation, the vans were not on-site during today’s visit. During the school year, the NCPREK will have two field trips, and during the summer the school age children have a variety of scheduled field trips. A Director’s meeting will be held to review the new QRIS rated license system. The meeting will be held, September 29, 2025, from 1pm to 3pm at the Impact Alamance located at 133 E Davis St, Burlington, NC 27215 Preparing for North Carolina's New QRIS System The Quality Rating Improvement System (QRIS) is being modernized to provide more flexible and equitable ways for programs to demonstrate quality. Starting later this year, providers will be able to choose from three star-rating pathways—Program Assessment, Classroom & Instructional Quality, or Accreditation—allowing you to align your strengths with the rating method that best fits your program. Now is the time to evaluate your current practices, staff credentials, curriculum use, and classroom interactions to determine which pathway is the best fit. Programs are encouraged to begin preparing for updates to staff qualification standards and assessment tools. While existing star ratings will remain valid for now, all new star ratings will be subject to the new system. Your program’s quality should reflect not just compliance—but a commitment to best practices, child development, and continuous improvement. A checklist was used to note the requirements I monitored today, and a copy was left with you. The last annual compliance visit was conducted 9/28/22. Currently this program operates with a five (5) Star License, issued 10/27/22. The facility earned six (6) points in staff education, seven (7) points in program standards, and one (1) quality point form programmatic options met. Center director Bridgette Cannon assisted me today. I conducted an assisted walk-through of the center. The last sanitation inspection was completed 7/14/23 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 6/2/23. A reinspection was scheduled for 7/2/23. Ms. Cannon emailed the required paperwork for reinspection on 6/26/23. D. Sharpe, fire inspector was contacted to complete the reinspection but has been out of the office. I messaged him today regarding completion of the inspection. The facility uses two passenger vans for transportation (AJU 5104 and FLH 4442). Both were on site and monitored today. Children were observed playing in activity areas, large group activities, and routine care activities. This classroom has one NCPK classroom with nine (9) NCPK children enrolled. NCPK requirements were not monitored today. A full assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. The last health questionnaire for A. Knight was dated 12/2/21. .0701(a) 1035 Child care providers, including the director, uncompensated providers, substitute providers, and volunteers did not have the required Emergency Information Form on file on or before the first day of work, which included all the required information and/or the information on the form was not updated as changes occur and at least annually. The last emergency information form for A. Knight was dated 12/2/21. .0701(a) 1110 Vehicles used to transport children enrolled in the childcare center did not comply with all applicable State and federal laws and regulations. The registration on van FLH 4442 expired February 2023. .1002(b) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. The last staff evaluation and staff development plans for B. Cannon was 11/2021 and for T. Pointer 12/2/21. 10A NCAC 09 .0514(f) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Cannon. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 10/3/23. All requested documentation must accompany the Corrective Action Letter. The staff development plans and staff evaluations for S. Bell, A. Pinnix, M. Carpenter, and Y. Garner are due in November 2023. The remainder of the health and safety trainings for V. Lee are due by 8/7/24 and for K. Wiley by 8/30/24. All staff must be registered in WORKS for education for count for rated license assessments. Information should be updated as degrees are completed. As we resume rated license assessments, your facility is in Cohort 3. You will have a preparation year July 1, 2025-June 30, 2026 and an assessment year from July 1, 2026-June 30, 2027. We discussed the latest rule changes from environmental health. DCDEE is hosting two virtual trainings on September 20th and 26th. Emails were sent with the links for these trainings.
