A checklist was used to note the childcare requirements monitored, and a copy was left with you today. The last annual compliance visit was conducted 3/25/24. Currently this program operates with a five (5) Star License, issued 6/11/24. The facility earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point for education options met. Teacher Maria Soto assisted me today. I conducted an unassisted walk-through of the center. The last sanitation inspection was completed 1/29/25 with a ‘Superior’ classification and 0 demerits. The last fire inspection was conducted on 10/7/24. Children were observed playing in activity areas, large group activities, playing outside, eating lunch, and routine care activities. Transportation is provided by children’s parents. The facility has two NCPK classrooms with twenty-nine children enrolled. The NCPK requirements were monitored during today’s visit. The program operates for 6.5 hours a day. A monthly breakfast and lunch menu was posted in each classroom. Four NC Pre-K children's files were monitored and each contained a health assessment within 30 days, a DIAL screening within 90 days, and evidence of each child's on-going progress. The teachers use Tools of the Mind and activity plans were current and posted. Staff/child ratio of 1:9 or less is being maintained. The parents receive a monthly newsletter and through DOJO for daily interactions. They are able to volunteer throughout the school year. The parents are also invited to attend family engagement nights. The teachers also have parent meetings and conferences three times a year. The current ECERS-R score was received 5/21/24. The facility received a score of 5.31 A full assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. Five outlets were uncovered in EL1. 10A NCAC 09 .0604(c) 841 Medications including prescription and non-prescription items were not stored in a locked cabinet or other locked container. Two prescriptions for one staff member were not in locked storage. 15A NCAC 18A .2820(d) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The letter for M. Soto was due by 9/26/24 and was received 10/10/24. G.S. 110-90.2(b) & .2703(n)&(o) You were provided an opportunity to ask questions regarding interpretation and implementation of all childcare requirements. Each violation cited was discussed with Ms. Boggs. The violation(s) documented above must be corrected immediately. A signed and dated letter of compliance must be received on or 3/10/25. The letter must include the following: *Facility Name and ID number *List of each violation number *How each violation was corrected All items must be included, or the corrective action letter will be returned for corrections. All requested documentation must accompany the Corrective Action Letter. The letter can be emailed to lori.rierson@dhhs.nc.gov. CONSULTATION: The CPR/FA for M. Soto will expire 3/30/25. The facility earned seven (7) points in staff education, seven (7) points in program standards, and one (1) quality point for education options met. Teachers Tyesha Boggs and Nikkie McCormick assisted me today. I conducted an unassisted walk-through of the center. The last sanitation inspection was completed 9/17/24 with a ‘Superior’ classification and 6 demerits. The last fire inspection was conducted on 12/20/23. Children were observed in large group activities, playing outside, resting, and routine care activities. Transportation is provided by children’s parents. The facility has two NCPK classrooms with twenty-five children enrolled. The NCPK requirements were not monitored during today’s visit. The files for three new staff member were reviewed today. A partial assessment of childcare requirements was conducted today. The following violation(s) were documented and technical assistance regarding maintaining compliance was offered. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). The qualification letter for M. Soto expired 9/26/24. CONSULTATION: We discussed items that I will need to see in the staff files for the annual compliance visit. Items are current health questionnaire, emergency information form, criminal background letter, CPR/First aid certificate, documentation of review of shaken baby syndrome, copies of ongoing training with the training log completed, and the health and safety training log. Ms. McCormick stated she will be completing playground safety during the month of October. Only staff who have completed the training can complete the monthly playground inspections. If Mc. McCormick does not complete the training, a staff member from another school with the training will need to complete the inspections. If Ms. James substitutes in the classroom, she must have current CPR/First Aid, criminal background letter, health questionnaire, emergency information form, and review of shaken baby policy on file. If she substitutes more than ten days in one classroom, she will be required to keep her health and safety trainings current.
