    Name of Operation: CHILDREN'S HOUSE MONTESSORI Facility ID: 01000044 Consultant: MONA NDIAYE Operation Type: Center Case Number: Visit Date: 1/8/2026 Number Present: 53 Completed Date: 1/8/2026 Age: From 0 To 5 Total Minutes: 135 Time In: 12:00 PM Time Out: 02:15 PM Time In: Time Out: List to Use: Center Type Of Visit: Routine Unannounced Announced/Unannounced: Unannounced The purpose of today’s visit was to monitor compliance with applicable childcare requirements. Upon my arrival I was greeted by the Director. The facility currently operates under a Three Star License that was issued on July 1, 2019. The last annual compliance visit was conducted on June 17, 2025. The last sanitation inspection was completed July 15, 2025 with a “Superior” classification. The last fire inspection was conducted on April 30, 2025. I completed a walk-through of the classrooms, attendance was documented, and observations were made. The children were resting. During today’s visit, a partial assessment of applicable Child Care Requirements was conducted which included, General Licensing Requirements, Program Requirements, New Staff Records, Staff Child Ratio, Supervision, Medications and General Safety. There was three new staff. Today I observed the following violation(s) regarding the Child Care Requirements Violation Number Comment Rule 1032 Child care providers and uncompensated providers who are not substitute providers or volunteers, including the director did not have a medical report on file prior to employment that was signed by a health care professional and/ or the medical report was older than 12 months. One staff hired in September had a medical report that was older than 12 months. 10A NCAC 09 .0701(a) 1045 New staff, who had contact with children, did not receive at least 16 hrs. orientation within first 6 weeks. One staff hired in September 2025, did not have a completed orientation. .1101(a) The above violations must be corrected immediately, and a letter must be sent to me no later than January 22, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. If I do not receive your letter January 22, 2026 and/or if you do not address all violations, a return visit to your program will be made to monitor for compliance of violations and to obtain your compliance letter. Additional violations may be documented if the situation has not been corrected or addressed. The violation was corrected during today’s visit. You were provided an opportunity to ask questions regarding interpretation and implementation of all child care requirements. Child care licensing rules and regulations are established to ensure a safe and healthy environment for children in care. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Preparing for North Carolina's New QRIS System We talked about the QRIS process, I advised the Director to start working on her family and community engagement and CQI for the center and the staff, using the latest Environment Rating Scales. This facility operates with a Three (3) Star License, issued on July 1, 2019. Upon arrival to the facility, I was greeted by the administrator, Grace Harris. I explained the purpose of my visit. Grace assisted me on a walkthrough of the facility. Attendance was taken and observations made. The children were observed resting, participating in indoor and outdoor free play, teacher directed activities and routines. The last fire inspection was conducted on April 30, 2025, and was approved for day time care only. The last sanitation inspection was conducted on October 14, 2024. The program received a Superior rating. Today, a full assessment of child care requirements was conducted. All new staff files were reviewed and a sample of previous staff including training. I monitored program records and a sampling of children’s records. The following violation was observed, cited, and technical assistance was provided during today’s visit. Violation Number Comment Rule 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). **One staff member has and expired CBC from February 2025.** G.S. 110-90.2(b) & .2703(n)&(o) The above violation must be corrected immediately, and a letter sent to my office describing when and how it was corrected. Please submit a corrective action letter to my office by July 1, 2025. You may send the letter by email to anita.c.graham@dhhs.nc.gov or you may mail it to my office at: Anita C. You may resubmit paperwork for the CBC renewal six (6) months in advance. The staff member may return to the facility once a qualifying letter is received. The last annual compliance visit was conducted on June 15, 2023. This facility operates with a (3) Three Star License, issued on July 1, 2019. A check of the Secretary of State website indicates the corporate status is current and active. Upon arrival to the facility, I was greeted by the administrator, Grace Harris-Ailsworth. I stated the purpose of my visit. Grace assisted me on a walkthrough of the facility. Attendance was taken and observations made. The children were observed participating in circle time outdoor free play and preparing for lunch. The last fire inspection was conducted on March 27, 2024 and approved for daytime care. The last sanitation inspection was conducted on February 26, 2024. The program received a Superior classification. A full assessment of child care requirements was conducted. Including permit restrictions, license posted, supervision and staff/child ratio. I monitored program records, children’s records, and two new staff files, staff trainings and criminal records. I monitored the outdoor learning space. The following violations were observed, cited and technical assistance, and resource links were provided. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. The March 2024 fire was not documented. .0604(t); .0302(d)(5) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. bleaches and detergents were observed in the kitchen, which was unlocked. .2820(b) 859 Monthly playground inspections were not completed and/or they were not completed by an individual trained in playground safety requirements. Current monthly outdoor inspections were not available for review. .0605(q) 1034 All staff, including the director, did not have an annual health questionnaire on file following the initial medical statement. Updated annual health questionnaires following the initial medical statements were not available for Division Representative review. .0701(a) 1232 Each employee's personnel file did not contain an annual staff evaluation and a staff development plan. The file for one staff member did not contain updated information for the staff development plan and evaluation. 10A NCAC 09 .0514(f) 1314 Emergency information did not name child's health care professional. One child’s file did not contain information regarding the health care professional or hospital preference in case of emergency. .0802(c)(2) 1811 Shelter-in-place or lockdown drills were not practiced every three months and/or drill record was incomplete. The last documented shelter-in-place drill was for April. .0604(u);.0302(d)(8) 1845 More than one child age 2 or older, was placed an age level above his/her chronological age when determined to be developmentally appropriate, per group. In the first classroom, there are 19 children present ranging in ages from two to five(2-5) years old. There are three children in this group who are two(2) years old. There are five(5) children who are three(3)years old, eleven children who are four(4) years old and a child who is five(5)years old. .0713(a)(4) 1864 Outdoor play equipment was not age and developmentally appropriate. A manufactured “Airplane Ace Flyer” which was designed for children who are three(3) and up was observed installed on a playground used by children who are one(1) and two(2) years old. .0605(a) You were provided an opportunity to ask questions regarding child care requirements. We discussed each of the areas of non-compliance and technical assistance was provided. The violation must be corrected immediately, and a compliance letter drafted stating how you corrected each violation cite. This letter must be received by my office by the end of the business day on June 25, 2024. If your letter of compliance is not received by my office by June 25, 2024 or if violations are not addressed in the letter, a return visit may be necessary to ensure that all violations have been corrected or addressed.
