Children were observed playing freely outside. A report was received regarding the unsafe handling of a child on 5/15/2026. I reviewed video footage from the Toddler II classroom from that date. Rest time ends at 2:30 p.m. Near the end of rest time, around 2:20 p.m., a child was observed getting off his cot several times between 2:20 p.m. and 2:31 p.m. A teacher was seen picking the child up and returning him to the cot multiple times. Although the teacher lifted the child under his arms, the video showed her placing him back onto the cot with increasing force. During the final incident, the teacher dropped the child's blanket onto his head. The teacher was immediately removed from the classroom once the administrator became aware of the situation. The administrator spoke with the teacher about appropriate handling of children and reiterated that children’s heads must never be covered while resting. The administrator reassigned the teacher to another classroom with a more experienced staff member. The teacher completed safe handling training on 5/26/2026 and subsequently resigned on 5/28/2026. Findings The allegation regarding rough handling substantiated. The allegation regarding developmentally appropriate care is substantiated. A partial assessment of child care requirements was completed during the visit. Two violations of child care requirements were cited during today’s visit. Violation Number Comment Rule 902 Each child was not attended to in a nurturing and appropriate manner, or in keeping with the child's developmental needs. A toddler was made to stay on a cot after waking up from nap and was not given a quiet activity to do. G.S. 110-91(10) 904 A child was handled in a rough way, including shaking, pushing, shoving, pinching, slapping, biting, kicking, or spanking. A toddler was forcefully put back on his cot several times towards the end of rest time. .1803(a)(1) The above violation must be corrected immediately, and a letter must be sent to me no later than June 16, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. The facility currently holds a Five-Star License. The most recent Annual Compliance Visit was completed on February 13, 2026. The allegations involved concerns regarding outdoor safety, supervision and health and sanitation. Upon arrival at the facility, I was greeted by the Interim Director, Omira Thompson. She remained present and cooperative throughout the visit. The purpose of the visit was explained. The children were observed eating lunch. A complaint was received alleging a three-year-old child ingested feces while in care, was left on the playground, and was found playing in what appeared to be dog feces. It was further alleged the child was changed into clothing not belonging to her and parent sought medical evaluation following the incident. Information obtained from the parent, facility self-report, and field staff indicated that on April 17, 2026, the two-year-old classroom transitioned to the playground at approximately 9:44 a.m. While outside, two children located an unknown substance later identified by the facility as animal feces. Facility documentation indicated the material was believed to be from a small wild animal. The facility reported staff recognized the substance and immediately removed children from the area, bringing the class indoors. One child’s clothing was changed due to contamination. Handwashing and hygiene care were completed after reentry indoors. The Director reported she was not notified of the incident at the time it occurred and became aware later during parent pick-up. One parent was informed at pick-up and another parent later by telephone. The parent later clarified she did not believe the child had been left unsupervised and stated the child denied ingesting the material, though parent remained concerned possible licking/contact may have occurred. An additional anonymous complaint was later received alleging improper diaper changing procedures in the toddler classroom, specifically that a staff member changed children on her lap while seated on a stool rather than utilizing the designated diaper changing surface. Findings Animal feces was present on the playground and accessible to children during outdoor play. This indicates the outdoor play area was not adequately inspected for hazards prior to use. The allegation regarding safe outdoor environment is substantiated. Available information did not support that the child was left alone or without staff supervision. Staff were reported present and responded once the hazard was recognized. The allegation regarding supervision is unsubstantiated. The Teacher in the OG toddler classroom was interviewed, and she stated she does change the children standing up and was not aware that she had to use the diaper changing table. I explained to her children in diapers must be changed on a changing table or mat on the floor. The allegation regarding health and sanitation regarding diaper changing is substantiated. A partial assessment of child care requirements was completed during the visit. Two violations of child care requirements were cited during today’s visit. Violation Number Comment Rule 402 Each child's diapers were not changed at areas designated exclusively for diapering, on a surface that was smooth, nonabsorbent, easily cleanable and of tight construction. A teacher was changing diapers of toddlers while they were standing up. 15A NCAC 18A .2819(a) & (b) 856 The indoor and/or outdoor premises was not checked once a day, prior to initial use, ensuring debris, and broken equipment was removed and disposed of. Two children picked up feces of a small animal on the playground. 10A NCAC 09 .0604(p) The above violation must be corrected immediately, and a letter must be sent to me no later than May 12, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. Upon my arrival, I was greeted by the Director, Davina Woods. She assisted me throughout the visit. I conducted a walkthrough of all classroom space documented enrollment, attendance records, and completed observations. The children were observed participating in free play and circle time. The teachers were observed interacting with the children. The last annual compliance visit was conducted on March 6, 2025. The center is currently operating with a 5-star license issued on April 9, 2021. The most recent sanitation inspection was conducted on July 9, 2025, with the center receiving a Superior rating. The last fire inspection was documented, December 17, 2025. Fire drills and playground inspections were monitored. Twelve staff files including seven new staff, were monitored along with ten children’s files. Your compliance history is currently 93% this percentage may change based on the results of today’s visit. During today’s visit, a full assessment of applicable Child Care Requirements was conducted. I observed the following violation during today’s visit. Violation Number Comment Rule 415 A current schedule was not posted for each group of children for reference. In one of the classrooms for toddlers, there was no schedule posted. GS 110-91(12);.0508(a) The above violations must be corrected immediately, and a letter must be sent to me no later than February 27, 2026 stating how each item was corrected and a plan to maintain compliance with the requirement. Kaye Adkins, Licensing Supervisor accompanied me on today’s visit. Prior to this visit we conducted an “other” visit to measure space. Upon arrival we were greeted by the Assistant Director, Cindy Lamm. The Director, Davina Woods meet with us after finishing her task. I stated the purpose of the visit and was assisted on a walkthrough of the facility. Attendance was taken and observations made. Children were observed participating in routines, nap, snack, and transitions. The last Fire Inspection was conducted on August 7, 2023, and was approved for daytime care only. The last Sanitation Inspection was conducted on April 11, 2024. The facility achieved a Superior rating. A partial assessment of applicable childcare requirements was conducted on this date. The facility license was posted, permit restrictions, supervision, staff/child ratio, and general safety. On this visit the following violations were cited: Violation Number Comment Rule 106 Operator has not scheduled and obtained a fire inspection within 12 months of the previous inspection. Operator did not submit the original approved report to DCDEE within one week of the inspection visit on a form provided by the Division. The last fire inspection was dated 8/7/23. 10A NCAC 09 .0304(a) The above violations must be corrected immediately, and a compliance letter sent to my office stating how you addressed/corrected the items of non-compliance. The facility operates with a five-star license, which was issued on April 9, 2021. The last annual compliance visit was conducted on March 15, 2023. Upon arrival to the facility, I was greeted by the assistant administrator, Ms. Lamm. I stated the purpose of my visit. Ms. Davina Woods was on site and accompanied me on a walkthrough of the facility. Attendance was recorded and observation made. I observed the children participating in outdoor free play, indoor free play, teacher directed activities, having lunch, and resting. The last Fire Inspection was conducted on August 7, 2023 and was approved for day time and night time care. The last Sanitation Inspection was conducted on April 6, 2023. The facility achieved a Superior rating. A full assessment of child care requirements was monitored this date. The facility permit was posted, supervision, staff/child ratios, adequate/approved space were in compliance. I monitored all staff files and training. I monitored a sampling of children’s records and monitored program records. The following violations were observed, cited and technical assistance was offered. Violation Number Comment Rule 721 All equipment and furnishings were not in good repair. I observed protruding attachments which raised about an inch on Equipment used for a sandbox on the toddler playground. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In a classroom with children who are two, an aerosol can of disinfectant spray was observed in an unlocked closet. This was corrected during the visit. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A prescription medication was observed in a classroom with infants without parent signed permission to administer. .0803(12) 871 Center staff did not comply with the safe sleep policy. In a classroom with infants the staff did not record all the required information for safe sleep checks. 10A NCAC 09 .0606(a) The above violations must be corrected immediately, and a compliance letter drafted stating how you corrected the items and when the violation was corrected. The letter should be submitted on your business letterhead. Please reference the type of visit, the visit date, and the item numbers cited for the violations. Please sign and submit the letter. The facility operates with a five-star license, which was issued on April 9, 2021. The last annual compliance visit was conducted on March 15, 2023. Upon arrival to the facility, I was greeted by the assistant administrator, Ms. Lamm. I stated the purpose of my visit. Ms. Davina Woods was on site and accompanied me on a walkthrough of the facility. Attendance was recorded and observation made. I observed the children participating in outdoor free play, indoor free play, teacher directed activities, having lunch, and resting. The last Fire Inspection was conducted on August 7, 2023 and was approved for day time and night time care. The last Sanitation Inspection was conducted on April 6, 2023. The facility achieved a Superior rating. A full assessment of child care requirements was monitored this date. The facility permit was posted, supervision, staff/child ratios, adequate/approved space were in compliance. I monitored all staff files and training. I monitored a sampling of children’s records and monitored program records. The following violations were observed, cited and technical assistance was offered. I observed protruding attachments which raised about an inch on Equipment used for a sandbox on the toddler playground. G.S. 110-91(6); .0601(b) 840 All corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, any product which is under pressure in an aerosol dispenser, and any substance which may be hazardous to a child if ingested, inhaled, or handled were not stored in a locked room or cabinet. In a classroom with children who are two, an aerosol can of disinfectant spray was observed in an unlocked closet. This was corrected during the visit. .2820(b) 849 Leftover medicines were not returned to the parent after the course of treatment was completed, after authorization was withdrawn or after authorization had expired and/or medication was not discarded within 72 hours of completion of treatment or withdrawal of authorization. A prescription medication was observed in a classroom with infants without parent signed permission to administer. .0803(12) 871 Center staff did not comply with the safe sleep policy. In a classroom with infants the staff did not record all the required information for safe sleep checks. 10A NCAC 09 .0606(a) The above violations must be corrected immediately, and a compliance letter drafted stating how you corrected the items and when the violation was corrected. The letter should be submitted on your business letterhead. Please reference the type of visit, the visit date, and the item numbers cited for the violations. Please sign and submit the letter.
