A checklist was used to note the requirements I monitored today. This program currently operates with a Five Star License, issued August 9, 2019. Restrictions include: a capacity of 8, daytime and overnight care, max. of 5 preschool children at any time, and no more than 2 children under age one. The license was posted, and the restrictions were in compliance. The last annual compliance visit was conducted on February 27, 2025. As of February 13, 2026, your facility has an 18-month compliance history of 100%. Child Care programs are expected to always achieve and maintain compliance and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. Owner/Operator, Valerie Green, was present and available for consultation. Ms. Green has a room set up exclusively for child care; a bathroom is located in this area. All spaces used by the children were monitored. Daily schedules were available for children. Activity plans were observed posted in the classroom. A variety of age-appropriate learning materials were observed in the classroom. Ms. Green has purchased and implementing the approved curriculum – Funnydaffer. There are eleven children enrolled on first shift, second shift, and third shift. You cannot have more than eight children present at one time with a maximum of five preschool children at any one time, no exceptions. There is no shift overlap. There were five preschool children, aged 1 -4, present. Children were observed eating lunch. Lunch consisted of Ramen noodles, peas, applesauce, chicken nuggets, milk. After lunch, children were observed laying down on individual cost with individual linens as soft music played, and the lights were off. All children were adequately supervised. Caregivers and children interacted in a positive and appropriate manner. There Following violations were observed and documented today. All violations must be corrected immediately. Continued compliance with applicable child care requirements is always required. Review the applicable child care requirements often. It is your responsibility to be knowledgeable of the child care laws and rules. Take time daily to review the applicable child care requirements. Criminal Background Check Information – This is a requirement and if it is not completed, it will be documented as a violation. DCDEE launched the Provider Portal for ABCMS on February 1, 2024. This access will allow providers to see applicants that associate themselves with their facility and allow them to “hire” the applicant to their roster, see the real-time background check status of staff/household members, and run a printable report of the staff roster to assist with compliance visits. Ms. Green reported that her facility choose Pathway 2 - Classroom and Instructional Quality, and plan to apply for a rated license by May 2026. Pathway 2 – Classroom and Instructional Quality: You are eligible for this pathway, which allows your program to demonstrate quality through smaller groups of children, child-focused planning, curriculum implementation, child observations, training and coaching for staff, CQI plans, and family engagement. Family and Community Engagement: The facility must complete the Family and Community Engagement Standards Worksheet selecting the applicable items Educational Opportunity, Engagement & Leadership, and Communication. Items selected must be verifiable. Continuous Quality Improvement (CQI): A CQI plan must be completed for the facility and each staff member. The CQI Plan must include the following: Identification of goal(s), Necessary resources, supports and actions needed to accomplish the goal(s). Document the reason for the goal(s), Annual review of efforts toward completion of the goal(s). During today's visit, we reviewed your education information in WORKS. Evaluate Hazards - The program will ship your facility a water sample kit and may coordinate an on-site visit by a professional to assess for lead-based paint and asbestos hazards, if needed. 4. Receive Results – Receive your results, recommendations, and water mitigation support, if needed. 5. Request Reimbursement - If you choose to conduct lead-based paint or asbestos mitigation, you can request reimbursement from NC DHHS. Limited child care requirements pertinent to this visit type were monitored. This program currently operates with a Five Star License, issued August 9, 2019. Restrictions include: a capacity of 8, daytime and overnight care, max. of 5 preschool children at any time, and no more than 2 children under age one. The license was posted, and the restrictions were in compliance. The last annual compliance visit was conducted on March 21, 2023. As of October 5, 2023, your facility has an 18-month compliance history of 100%. Owner/Operator, Valerie Green, was present and available for consultation. All spaces used by the children were monitored. There are nine children, including a drop in, enrolled on first shift. There are no children enrolled on second or third shift. There were three preschool children, ages1-4, present today. All children were observed napping on covered cots with blankets that were appropriately spaced. Daily schedule and activity plan was posted and available for review. A variety of age-appropriate learning materials was observed in the classroom. During today's visit child care requirements monitored for compliance include supervision, staff/child ratio, CPR and First Aid coverage, ITS-SIDS, Criminal Record Checks, storage of hazardous substances and medications, approved space, licensing requirements, staff and program records, posting of the license, and other applicable requirements. There was one violation observed today. Violation must be corrected immediately. Continued compliance with applicable child care requirements is required. Violation Number Comment Rule 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment. A child, who enrolled at this facility on 09/04/2023, does not have a health assessment on file. GS 110-91(1); 10A NCAC 09.1721(a)(1) Child Care programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. In order for your facility to remain in compliance with NC Child Care Requirements, violations must be corrected immediately. On or before October 12, 2023, I must receive a written, dated, and signed compliance letter that describes accurately and in detail, how and when the violations were corrected as well as how compliance will be maintained in the future. The time frame is established to allow you time for submitting your compliance letter. This timeframe is not intended to be used as a guide for correcting violations, as they should be corrected immediately. Child care licensing requirements are established to ensure a safe and healthy child care environment. Therefore, it is important for you to be knowledgeable of all the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all the applicable laws and rules always whether they have been monitored or specifically discussed with you in the past. The best way for you to make sure that you are meeting all requirements is to periodically review the child care law and rules. Technical Assistance: Each child must have a physical/health assessment on file within 30 days of enrollment, no exceptions. One child was enrolled at your facility on September4, 2023 and this information is not on file for child. Other Information: On March 13, 2023, the Military Child Care in Your Neighborhood-PLUS (MCCYN-PLUS) initiative launched in North Carolina to make it easier for military families to access and afford quality child care from community providers. Licensed child care facilities are now able to participate in MCCYN-PLUS based on their star rating. Health and Safety Training- These training topics outlined in Child Care Rule 10A NCAC 09 .1703(b) must be completed within the first year of employment for new staff, with the exception of Recognizing and Responding to Suspicions of Child Maltreatment – it must be completed within 90 days of hire. After initial completion of Health and Safety Trainings, they must be completed every five years. Use the Health and Safety Training Record which is found on the DCDEE website under Provider Documents and Forms to track the completion of these trainings. You completed these trainings in July 2022, therefore, you will need to completed these trainings again by July 2027. Completing, refreshing, and updating the required Health and Safety Training increases your awareness and knowledge regarding child development and safety. Rated License Reassessment Information Your facility’s three-year rated license reassessment was due to be completed in August 2022. DCDEE has developed a 3-Cohort system. There are three cohorts, and each includes a preparation year and a reassessment year. You are in Cohort 2. We will be working together through this process, and I will provide guidance along the way. I will reach out to you prior to the beginning of the preparation year of your assigned cohort and will provide guidance to assist you as you begin the process. As discussed today, reassessments will not become mandatory again until July 1, 2024, and your facility has been placed into “Cohort 2.” Placement in this cohort means you will have from July 1, 2024 to June 30, 2025 to plan and prepare for your reassessment, with your reassessment being due between July 1, 2025 and June 30, 2026. In your planning year, you may review resources available at www.ncrlap.org and contact Smart Start of Pender County and Region 4 for assistance with completion of the self-study component. Documentation was completed electronically, signed, and emailed to you. Please remember it is your responsibility to comply with all child care rules and requirements at all times.
