This NCPK located in a public elementary school, currently operates with a Five-Star License effective January 9, 2019. There are two classrooms on site. Upon my arrival, I was greeted by the school office staff and signed in. I introduced myself and stated the purpose of my visit. Ms. L. Jones and Ms. A. Porada are the lead teachers. Ms. Jones gave me the necessary paperwork I needed to complete today’s visit. Thirty-one (31) children were present during today’s visit. I observed the children participating in free choice and teachers directed activities, completing bathroom routines, eating lunch and transitioning to rest time. The classrooms are organized with many items for play and activities. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. Required documents were posted and /or accessible. Today five violations of child care requirements were cited. Violation Number Comment Rule 603 All floors and floor coverings were not constructed of nonabsorbent material and/or were not kept clean and in good repair. The area rug in Space 090 was observed to be significantly stained and may require cleaning or replacement. 15A NCAC 18A .2824(a)&(b) 1044 Prior to the expiration date of the qualification letter, the child care provider did not complete and submit required forms to complete a criminal background check (a qualification letter is valid for a maximum of five years for the date of issuance). A. Porada initiated the criminal background check on July 18, 2025, but the case was closed on September 15, 2025 without being completed. G.S. 110-90.2(b) & .2703(n)&(o) 1757 A valid qualification letter was not on file and available to review at the facility. The qualification letter for A. Porada expired on August 18, 2025. G.S. 110-90.2(b) & (d) & .2703(e) 1864 Outdoor play equipment was not age and developmentally appropriate. There are 2 green teeter- tooters on the playground. Teeter-totters should not be used by preschool-age children unless they are equipped with a spring centering device and have an appropriate shock-absorbing material underneath the seats. .0605(a) 1865 The operator did not maintain the manufacturer's instructions on file electronically or in paper format for any outdoor play structures purchased or installed on or after September 1, 2017. The manufacturer's instructions for the two green teether totters were not on file electronically or in paper format. .0605(b) Childcare programs are expected to achieve and maintain compliance at all times and are required by NC GS 110-90(4) (d) to achieve and maintain an eighteen-month compliance history score of at least seventy-five percent. Any violation(s) documented may impact the compliance history score. The violations documented must be corrected immediately. A signed and dated letter of compliance must be emailed to me at sharon.y.williams@dhhs.nc.gov on or before October 6, 2025. The letter of compliance should contain the following information: 1. Facility Name 2. Facility ID 3. Date of the letter 4. The violation number. Date the violation was corrected. How the violation was corrected. 5. Signature of the person preparing the letter. If the letter of compliance is not received on October 6, 2025, a follow-up visit will be conducted. Today in Space #090: The qualification letter for A. Porada expired on August 18, 2025. According to the ABCMS system, A. Porada was previously qualified to work in child care on August 18, 2020. Qualification letters are valid for five years from the date of issuance. In ABCMS, it shows that the criminal background check process for A. Porada was initiated on July 18, 2025, but the case was closed on September 15, 2025, due to the Name Check Request not being submitted within the authorized timeframe. A. Porada will need to resubmit her information to restart the criminal background check process. Pathways to the Stars Transition Update The Pathways to the Stars transition will begin soon. A rule rollout module is now available on the DCDEE e-learning Moodle platform for your review. On September 8, 2025, the new rules outlined in Section .3200: Standards for Two through Five Star Rated Licenses were discussed with the school’s central office. The central office will determine which pathway your facility will follow. Today, we reviewed the three Pathways to the Stars, and I demonstrated how to access the relevant information on the DCDEE website. Thank you for your cooperation during today's visit. I observed the children finishing lunch, listening to a story completing bathroom and transition routines. A partial assessment of child care requirements occurred during today's visit. I monitored supervision, staff/child ratio, adequate/approved space, license posted and permit restrictions. Required documents were posted and or accessible. There was one violation of child care requirements cited during today’s visit. The violation was corrected during visit. Violation Number Comment Rule 812 Electrical outlets and power strips, not in use, which were located in space used by children did not have safety outlets or were not covered with safety plugs unless located behind furniture or equipment that cannot be moved by a child. In Room 90. two electrical outlets accessible to children were uncovered. 10A NCAC 09 .0604(c) Rated License Assessment Process: The Governor signed Senate Bill 425 (now Session Law 2024-34), Health and Human Services Omnibus Bill, which includes the Quality Rating Improvement System (QRIS) Modifications recommended to the Legislature by the NC Child Care Commission to update QRIS and create additional pathways for licensed child care facilities to earn star rated licenses. Hold Harmless provisions have been extended and star rated license assessments for licensed child care providers will be conducted by provider request only, until the QRIS rulemaking process has been completed. Star rated license assessments are still required for new child care programs. TA: Today I provided technical assistance to Ms. Porada. The technical assistance provided to Ms. Porada was discussed with Ms. Ortado, Assistant Principal and Ms. L. Pettiford, GSC PreK Coordinator. Technical Assistance: 10A NCAC 09 .1801 SUPERVISION IN CHILD CARE CENTERS (a) Children shall be adequately supervised at all times in child care centers. Adequate supervision shall mean that: (1) staff must be positioned in the indoor and outdoor environment to maximize their ability to hear or see the children at all times and render assistance; (2) staff must interact with the children while moving about the indoor or outdoor area; (3) staff must know where each child is located and be aware of the children's activities at all times; (4) staff must provide supervision appropriate to the individual age, needs, and capabilities of each child; Appropriate supervision of children is important for their safety, injury prevention, and making sure of quality child care. Staff must be able to see and hear the children to be ready to help in an emergency. To facilitate effective supervision, staff should thoughtfully arrange the classroom environment. Activities should be grouped together, and all equipment and furniture should be positioned to ensure clear sightlines and audibility throughout the space. During the observation, it was noted that the shelves separating the block area were taller than those in other areas, creating blind spots. For instance, a child sitting on the floor in the block area may be obscured from view near the classroom entrance. To remedy this, the furniture and shelving should be rearranged to allow staff unobstructed sight of all children. We have discussed the need to reorganize the block area to improve visibility. Teacher intervention is crucial to protect children and promote more acceptable behavior. When a child displays aggressive actions that threaten the wellbeing of others, teachers must intervene swiftly. The discussion today covered documenting incidents of aggressive behavior and communicating with school administrators and the PreK coordinator. Thank you for your cooperation during today’s visit. Upon my arrival, I was greeted by Ms. L. Jones and Ms. A. Porada, the lead teachers. I informed them the purpose of today’s visit. Ms. Jones gave me the necessary paperwork I needed to complete today’s visit. I observed and documented the attendance. Twenty-six (26) children were present during today’s visit. They were observed reviewing letters, listening to a story, building in blocks, playing with in housekeeping, playing with table toys cleaning up , washing their hands in preparation for lunch and eating lunch. Limited licensing requirements were monitored. The followings items were observed in compliance during today's visit: supervision, staff/child ratio: space capacity: proper storage of medications; use of approved space, permit posted; permit restrictions; CPR and FA certification. There was one violation cited during today’s visit. Violation Number Comment Rule 1041 Prior to employment a Criminal Background Check was not completed. One substitute did not complete a criminal background prior to employment. G.S. 110-90.2(b) The above violation must be corrected immediately, and a letter must be sent to me no later than November 29,2023 stating how each item was corrected and a plan to maintain compliance with the requirement. Additional violations may be documented if the situation has not been corrected or addressed. Consultation: Today I observed a substitute in on the NC PreK classroom. This substitute did not have a qualifying criminal letter. I discussed with Ms. Jones, Ms. Porada and Dr. B. McCain, the principal, the process for the criminal record for substitutes. I recommended that Dr. McCain have some of the teacher’s assistants in the school to go through the criminal record process for DCDEE. This way you will have staff that you can move to NC PreK classroom in the event the NC Pre-k staff is absent. Rated License I also discussed with Ms. Jones and Ms. Porada the Rated license assessment process will begin July 1,2024 next year. There is an observation assessment that you can contact the rated assessment project and a preparation assessment you with me. Ms. Jones and Ms. Porada stated they would let me know. I informed Ms. Jones and Ms. Porada this facility is in Cohort 2. Your Prep year is July 1,2024-June 30,2025. Your Reassessment year July 1,2025-June June 30,2026. You must let me know if you ready to start the process before June 30,2024. Thank you for your time during today's visit.
