Name of Operation: CREATIVE YOUNG MINDS Facility ID: 41002552 Consultant: MARY MCMILLAN Operation Type: Family CC Home Case Number: Visit Date: 6/4/2026 Number Present: 4 Completed Date: 6/4/2026 Age: From 2 To 3 Total Minutes: 105 Time In: 10:30 AM Time Out: 12:15 PM Time In: Time Out: List to Use: Family CC Home Type Of Visit: Temp Time Period Announced/Unannounced: Unannounced I conducted an unannounced visit today to monitor applicable child care requirements during a Temporary Time Period visit. Today I met with Brenda Pritchett, Owner/Operator. The license was issued on 4/7/26. Upon arrival the children were observed transitioning from outside play, having free play, and teacher directed story time. The children were being cared for in an age-appropriate manner. I conducted a walk-through of the inside and outside areas that the children use. Limited items were monitored during today’s visit: Supervision, capacity, approved space, hazardous item storage, 4 children’s files were observed, program records, permit posted and restrictions met. The fire drill was conducted today at 10:55am am with 2 minutes evacuation time, for May 2026. The following violations were cited during today’s visit. Violation Number Comment Rule 714 Monthly check for hazards on the outdoor play area was not completed using a form supplied by the Division. There were no monthly checks completed. 10A NCAC .1721(e)(5)(A-F) 910 Copy of each child's health assessment, including the operator's own preschool child(ren), was not on file within 30 days of enrollment, for all 4 children enrolled. GS 110-91(1); 10A NCAC 09.1721(a)(1) 911 Copy of each child's immunization record, including the operator's own preschool child(ren), was not on file for 3 enrolled children. GS 110-91(1); .1721(a)(2) 1705 Summary of the NC Child Care Law was not posted with the facility's license in a prominent place in the home. GS 110-102 1853 The operator did not conduct a monthly fire drill. No fire drills had been conducted. .1719(a)(15) & .1721( e)(2) 1897 Application did not have an a medical action plan attached for any child with health care needs such as allergies, asthma, or other chronic conditions that require specialized health services. There was no medical action plan for one child with asthma. .1721 (a )(4) The following items of technical assistance were discussed during today’s visit: -File requirements for children and having all required items on file, as stated on the children’s records checklist, and having parents only put 1 medication on each permission slip and the required information. -Conducting playground inspections monthly. -Making sure that all program records are posted, at all times, that are required. -Fire drills must be conducted monthly to help children learn and know what to do when they hear the sound of the smoke alarm and how to get out of the home. -Record keeping is one of the most time-consuming tasks associated with administering a child care program. It is an important part of licensure. Review staff and children's files often to ensure the program is maintaining accurate licensing records. Organization and completeness of records is important to ensure easy access to information in emergency situations. Make sure that you develop a record keeping system for staff and children’s records that promotes organization and efficiency. We discussed the following items of Consultation: -You were given an opportunity to ask any child care related questions: you had questions about what is required to care for your cousins school-aged children. We discussed that any children that you keep will count in your enrolled numbers and you must have items on file just like any other enrolled child (except school-aged children do not have to have a physical and shot record) and you must have age-appropriate school-age activities available, all rules still apply for this age group, so you must have a lesson plan for them as well as request an age range change to your license, before you can care for them. -We discussed the pathway to the stars information for pathways 1 and 2. You can find all information in child care rules .3200 on our DCDEE website at ncchildcare.ncdhhs.gov and under the What's New tab, under QRIS modernization and scroll down to the information sheets and webinars. If you wish to meet DSS subsidy eligibility, you must be at least a 3-star license at the end of your Temporary Time Period. You stated that you are not sure if you wish to be more than 1-star license at this time but will review the pathway information. Reminders: COMPLIANCE HISTORY must be above 75% to apply for a star rated license and to keep a license. When you receive your Child Care License from the State of NC, you agreed to comply at all times with the Law, Rules and Requirements pertaining to and governing Child Care in NC. It is your responsibility to read, learn, and maintain all Child Care Requirements that apply to your licensed facility. It is also your responsibility to ask Division staff questions for clarification of requirements you do not understand and to monitor your staff to ensure that all requirements are maintained at all times. Recognizing and Responding to Suspicions of Maltreatment: Child care staff are expected to be well informed about child abuse and neglect issues and adequately trained to prevent and recognize potential maltreatment. All staff are required to complete the training within 90 days of hire. Recognizing and Responding to Suspicions of Child Maltreatment training is available on Prevent Child Abuse NC website, www.preventchildabusenc.org, free of charge, and counts toward Health and Safety Training. You must correct all violations immediately and you must submit a compliance letter no later than 6/18/26, stating how each of the violations was corrected. You can email the letter to me at mary.mcmillan@dhhs.nc.gov A sample letter will be emailed to you. All information in this report has been reviewed with me today.I understand that it is my responsibility to maintaincompliance with applicable NC Child Care Requirements at all times
