I met with Ms. Crystal Lemons, Administrator, we conducted a walk-through of the facility. The program operates with a three-star license issued February 22, 2019. The facility had a 96% compliance history prior to today's visit. The Secretary of State’s website was monitored, and Kountry Kids Learning Center, LLC was current and active. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used as a monitoring tool during today’s visit. There are one hundred seven (107) children currently enrolled and fifty-six (56) were present during the visit. The children were observed engaged in the following activities: free-play in activity areas, tummy time, outdoor free-play, bathroom/handwashing routines, lunch, and nap. Prior to the visit, an email containing the staff and training worksheets was received from Ms. Lemons, Administrator. Two (2) new staff members have been hired since the last monitoring visit was conducted. The files of the new staff and files of two (2) existing staff were monitored during today’s visit. Eleven (11) children's records were monitored during the visit. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, daily schedules, current activity plans, First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, allergies, safe sleep policy, and evacuation routes. The annual fire inspection was conducted 3/30/2026 and the facility was found to be satisfactory and approved for daytime care only. A sanitation inspection was conducted on 6/27/25 with a score of 6 demerits for a Superior classification. The last documented fire drill was conducted on 4/2/26. The last documented shelter-in-place drill was conducted on 4/1/26. The monthly playground inspections have been completed as required. All required components to meet nutritional requirements were served during lunch. I monitored one (1) van for compliance with transportation requirements. The following items were monitored today: supervision, staff/child ratio, staff, health, safety, program records, management, and transportation. Rated License: The facility has chosen pathway 2: classroom and instructional quality. During today’s visit the following were reviewed: A Continuous Quality Improvement Plan for each staff member, facility CQI plan, the family and community engagement form with supporting documents, the curriculum for each age group, formative assessment tool, training certificate for the administrator, C. Lemons, verifying completion of training on the use and implementation of a curriculum and formative assessment. The facility uses the Funny Daffer curriculum for all ages and the Teaching Strategies GOLD formative assessment. The staff information and education sheets were received during today’s visit. Staff education will be reviewed via WORKS. Upon completion of the staff education review the administrator, C. Lemons, will be contacted regarding the star level for which the staff education qualifies the facility to receive. Two (2) violations of childcare requirements were observed today. Violation Number Comment Rule 805 Fire drills were not practiced monthly and/or the drill record was incomplete. A fire drill was not recorded for the month of December 2025 on the fire drill record. .0604(t); .0302(d)(5) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Z. Baldwin hired 11/17/25, completed this required training on 2/20/26. .1102(g) You shall submit a signed letter to me explaining in detail how you have corrected the violations described above. During today’s visit the education status of several staff was checked via WORKS. The staff information was discussed with the Administrator, C. Lemons. A discussion regarding the requirement for gross motor activity to be a part of the activity plan in each classroom took place. The activity plans that will be used as part of the Funny Daffer curriculum were updated during this visit to include gross motor activities. The facility operates with a 3-star license earning 2 points in program standards, 4 points in education and 1 quality point. The facility has a current compliance history of 96%. There were 70 children present today. The children were observed during completing lunch, rest time, and bathroom/handwashing routines. The last annual compliance visit was conducted on 5/13/24. The rated license assessment was completed in February 2019. A superior sanitation inspection was completed on 6/25/24 with 0 demerits. A fire drill was last conducted on 11/1/24. A lockdown place drill was last conducted on 11/1/24. The playground inspections were completed as required. All indoor areas used by children were monitored today. The following items were posted for easy reference: license, NC Summary of Child Care Law, safe arrival and departure procedures, daily schedules, First Aid posters, applicable ratio charts, emergency medical care plan, emergency telephone numbers SIDS policy, evacuation routes, tobacco policy, cot lists, and menus. The following items were monitored today: supervision, staff/child ratios, storage of hazardous products and medications, adequate/approved space, new staff files, program files, posted license and license restrictions. Seven (7) new staff have been hired since the annual compliance visit was conducted. These files for the seven (7) new staff were monitored today. I monitored the current criminal record qualifications and specialized trainings for the staff using the staff and training worksheets that were completed for the last annual compliance visit. All staff have current CPR and First Aid certification. The staff scheduled to work in the infant classroom have current SIDS certification. All staff except for the new staff have completed the required health and safety training topic areas. I observed children in space #7 engaged in tummy time, and eating. The teachers present in the classroom were attentive to the needs of the children. One (1) teacher was sitting on a playmat engaged with the children as they experienced tummy time. The other teacher was attending to a child who was in a highchair eating. All interactions and language observed were positive. I observed children in space #3b engaged in free play in activity areas. One teacher was observed sitting on the carpet engaging with the children as they played. The other teacher was observed completing routine duties. The interactions and language were positive. I observed children in space #8 engaged in a large group art activity. The teachers were observed assisting the children as needed. All interactions were positive. There was one (1) violation of child care requirements observed during today’s visit. Upon arrival, I was greeted by Crystal Lemons, director, and I shared the purpose of the visit with you. During the visit, I observed supervision, staff/child ratio, adequate/approved space and permit restrictions. During this visit I reviewed attendance logs, activity plans, and observed lunch. I monitored each classroom during this visit and spoke with staff in regards to any knowledge they may have about children being treated in a less than nurturing manner. None of the staff interviewed were aware of any child or children being treated in a manner that would not be nurturing. Staff members interviewed were not aware of instances where child-staff ratios were not maintained. All staff-child ratios were in compliance during this visit. Children in each classroom were observed engaged in play and activities that were developmentally appropriate for their age. Lunch was observed being served to various groups of children during this visit. The portion sizes observed being served during this visit to different ages of children met the requirements as listed in the meal patterns for children in child care programs chart located at https://www.fns.usda.gov/cacfp/meals-and-snacks, that is referenced in 10A NCAC 09 .0901 GENERAL NUTRITION REQUIREMENTS. Violation Number Comment Rule 524 When children 3 years and older were in care, screen time was not used to stimulate a developmental domain; was not limited to 30 minutes a day and no more than a total of two and a half hours per week, per child; and/or was not documented on a cumulative log or the activity plan that is available for review. Screen time was used in space #1 and space #5 and screen time logs were not available. .0510(d)(2)(A-C) The violations documented during the visit must be corrected immediately. To verify correction of the violations you shall submit a signed letter to me explaining in detail how you have corrected the violations described above. I met with Ms. Crystal Lemons, Director, we conducted a walk-through of the facility. The program operates with a three-star license issued February 22, 2019. Restriction: Meets enhanced ratios. The facility had an 99% compliance history prior to today's visit. The “Annual Compliance Monitoring Checklist for Child Care Centers” was used as a monitoring tool during today’s visit. There are one hundred fifty-two (152) children currently enrolled and one hundred three (103) were present during the visit. The children were observed engaged in the following activities: free-play in activity areas, outdoor free-play, bathroom/handwashing routines, lunch, and nap. Prior to the visit, I sent an email requesting the staff and training worksheets to Crystal Lemons, Director. The worksheets were received prior to the visit. Five (5) new staff members have been hired since the last annual compliance visit was conducted on 5/15/23. I monitored the files of all new staff and two (2) staff files of existing staff for 10% of staff files. I monitored sixteen (16) children's records during the visit for 10% of children's records. The following items were posted for easy reference: license, Summary of NC Child Care Law and Rules, safe arrival and departure procedures, emergency medical care plan, daily schedules, current activity plans, First Aid poster, menu, applicable staff/child ratio charts, tobacco free signage, allergies and evacuation routes. The annual fire inspection was conducted 4/22/2024 and the facility was found to be satisfactory and approved for day time care only. The sanitation inspection was conducted 6/29/23 with a score of 4 demerits for a Superior classification. The last documented fire drill was conducted on 5/11/24. A lock-down drill was conducted on 4/9/24. The monthly playground inspections have been completed as required. All required components to meet nutritional requirements were served during lunch. I monitored one (1) van for compliance with transportation requirements. The following items were monitored today: supervision, staff/child ratio, staff, health, safety, program records, management and transportation. Three (3) violations of childcare requirements were observed today. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. During this visit a bottle observed in space #7 and a bottle in space #3a was not labeled. 15A NCAC 18A .2804(d) 1321 Medical exam or health assessment record was not on file before or within 30 days after admission. Two (2) children's files monitored during today's visit had medical exams that were submitted more than 30 days after admission.
