An annual compliance monitoring checklist was used. You completed the FCCH and Centers in Residence Statements of Responsibility and the FCCH Verification Form. Your signature on these forms will serve as verification that the information is accurate and complete. Due to receiving an administrative action (summary suspension) which placed you on inactive status effective 1/29/26 your annual compliance visit was not completed by 2/17/26. You were placed back on active status as of 3/11/26 therefore your annual compliance visit is being completed as of today. This program currently operates with a four-star license. During the visit, the inside and outside areas of your home were monitored. During the visit there were two preschool children present, observed playing with toys. The childcare room and the bathroom the children use was observed. Smoke detector, fire extinguisher, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, steps, toys, and materials were all monitored for compliance. The permit, emergency numbers, schedule and were posted. Qualifying letters, CPR / First Aid, health questionnaire, emergency procedures, and children's records were also monitored for compliance. You stated you had been working with the children on colors, and alphabet recognition. The lunch met meal pattern requirements. Your last fire drill and monthly playground inspection was completed 3/16/26. The last EPR drill was a shelter-in-place completed 3/16/26. I reminded you this must be completed every three months and documentation kept on file for review. I recommend that you place your due dates on a calendar or a spreadsheet to help in maintaining compliance. The following violation was documented during the visit. Violation Number Comment Rule 1301 Operator did not complete the required number of on-going training hours as specified in rule. The operator had a total of 6.5 hours of in-service training hours completed rather than the ten hours required annually. GS 110-91(11); 10A NCAC 09 .1705(b)(5) The violation must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By April 6, 2026, please send me a letter addressing the violation, how the violation was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and for the item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov. If you are unable to correct the violation with the specified time frame, please submit a request for an extension. This request should include a specific plan to correct the violation and any timeframe needed. The request will need approval from management. Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. During today’s visit I reviewed you are required to complete ten hours annually of in-service training hours. I explained the education standards, CQI requirements, and the family and community engagement. The Notice was received by Zannette Jones, owner of Zan’s Play-N-Learn facility. I reviewed the Final Notice Cover Letter, Notice of Administrative Action, and Procedures to Appeal with you. Please be reminded that you must close by the end of the day. I explained that subsidy will also end today. If you operate a child care facility without being licensed, you will be in violation of North Carolina General Statute 110-98 and subject to penalties of North Carolina General Statute 110-103. This means the violation is a felony punishable by a fine, imprisonment, or both. The following violation was documented Violation Number Comment Rule 1842 A child care provider or uncompensated provider found to be disqualified and not eligible for employment in child care was employed or living in the home. A household member received a disqualification regarding the criminal background check. G.S. 110-90.2 &.2703(i) The violation must be corrected immediately. Today I reviewed your email address with you, and you stated nothing has changed. This program currently operates with a four-star license, issued February 25, 2016. During the visit, the inside and outside of your home was monitored. Upon arrival there were two children present observed playing with toys. The childcare room and the bathroom the children use was observed during today’s visit. Smoke detector, fire extinguisher, first aid kit, refrigerator thermometer, storage of medication and hazardous products, outlets, garbage storage, steps, toys, and materials were all monitored for compliance. The permit, emergency numbers, schedule and NC Summary of the Child Care Law were posted. CPR / First Aid, health questionnaire, and emergency procedures were also monitored for compliance. Your last fire drill and monthly outdoor playground inspection were dated 7/8/25. Your last EPR drill was a lockdown completed 6/8/25. I reminded you this must be completed every three months and documentation kept on file for review. Please be reminded to complete your EPR drill on a different day than your monthly fire drills. Your last fire inspection was completed 1/14/25. The following violations were documented and corrected during today’s visit. Violation Number Comment Rule 706 Corrosive agents, pesticides, bleaches, detergents, cleansers, polishes, and products under pressure in an aerosol dispenser and any substance that may be hazardous to a child if ingested, inhaled, or handled were not kept in locked storage when children were in care. A bottle of bathroom cleaner was located in an unlocked cabinet under the sink in the bathroom used by the child care children in care. .1719 (a)(7) 1804 Safe sleep policy and poster about safe sleep practices was not posted in a prominent place in the infant sleeping room or area where parents and caregivers are able to view daily.The safe sleep policy was not posted. .1724(b) Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Technical assistance regarding the following items was reviewed with you today. Please be reminded to keep all hazards cleaners in locked storage and inaccessible to children in care. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. The compliance of this rule will be monitored during your next visit. Please note, the hard copy of the Change of Information form will no longer be needed or accepted. Clean Classrooms for Carolina Kids is a program designed to identify and eliminate exposure to lead and asbestos hazards in buildings. By May 31, 2025 all facilities must have enrolled for lead in paint, and water, and asbestos at www.cleanwaterforuskids.org/carolina. At the time of today's visit, our area has been experiencing extreme heat with temperatures in excess of 100 degrees. We discussed several concerns presented by these high temperatures and ways to address them: Outdoor time must be limited to no more than fifteen minutes and must occur in the early hours of the day. Please refer to your weather watch chart for further instructions for your youngest children in care, as well as those with special health concerns that may be made worse when exposed to high temperatures. While outside, please pay special attention to all children and staff members to watch for signs of heat exhaustion including heavy sweating, pale or clammy skin, headaches, dizziness, weakness, vomiting and/or a fast, weak pulse. Heat strokes are considered a medical emergency and require immediate medical attention. Symptoms of heat stroke include a dangerously high body temperature (104 or higher), hot and dry skin, confusion, rapid pulse and potential loss of consciousness. If you suspect a child or staff member is experiencing either of these conditions, please seek medical attention. Drinking water must be freely available to all children and should be provided during outdoor play, as well as immediately upon re-entry into the facility following outdoor play. Your building/home must remain at 85 degrees or less based on Environmental Health requirements. Please refer to your Safe Sleep Policy regarding the temperature requirement for classrooms where infants are present.
