Upon arrival I was greeted by L. Montgomery Lead teacher in the preschool classroom. The Administrator/Owner, Ms. Stokes arrived within fifteen minutes at the facility and assisted with today’s visit. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law were posted. I monitored the program requirements, furnishings and equipment, outdoor area, and medications. The outdoor area was not monitored due to inclement weather at the time of the visit. The infants and toddlers observed were playing with toys on the cushion area of the classroom. The toddlers and preschool children were observed playing in activity areas in their classrooms. The theme in the lesson plans were on arctic animals and Christmas activities. The lunch menu met the meal pattern requirements. This program currently operates with a four -star license issued January 16, 2020. Your last fire drill was completed 11/25/25. The last monthly outdoor playground inspection was completed 11/24/25. Your last EPR drill was a lockdown completed 10/27/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. The following violation was documented and corrected during the visit. Violation Number Comment Rule 428 A current activity plan was not posted for each group of children for reference. The activity plan posted in the infant classroom was dated October 6-10,2025. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items was reviewed with you today. During the visit I suggested prior to leaving each Friday to make sure all activity plans are up to date and posted for the next week in each classroom. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. LICENSING FEES: Annual Licensing Fee: Pursuant to GS 110-90(1a) the Division establishes an annual fee for licensed child care facilities. The invoices for the fees are assessed based on your licensed capacity and emailed after the 1st of November every year. You will only receive one invoice, and payment is due in FULL within 30 days. Please closely monitor your email and be on the lookout for your invoice. Please don’t delay making your payment and be aware that Payment Plans are NOT available. In addition, please be aware that failure to pay a license fee within 30 days of receipt will result in late charges. If payments are made late and late charges are applied, the fee as well as the late charge must be resolved. Please be reminded that failure to pay a license fee could also result in the facility's child care license being revoked. A checklist was used to note the requirements I monitored today. Today Ms. L. Montgomery accompanied me on the walk through and assisted with today’s visit. You stated the Administrator was not on site today. This program currently operates with a four-star license issued January 16.2020. Please be reminded to have all the staff’s education sent to WORKS to be evaluated. The hold harmless has been extended until further notice at this time. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, and summary of NC Child Care Law, were posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. A random selection of staff and children’s records were also reviewed today. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. There was one infant present observed being held by the staff member and the toddlers were observed playing with toys. The preschool children were observed during morning group time and later playing outside. The children’s record worksheet was completed. The lunch menu consisted of ravioli, wheat bread, corn, pineapple, and milk. The last fire drill was completed 4/30/25 and your monthly playground inspection was dated 5/19/25. Your last EPR drill was a lockdown completed 4/30/25. I reminded you an EPR drill must be completed every three months and to keep all documentation on file for review. Your last fire inspection was dated 12/4/24. Your last sanitation inspection was completed 12/12/24 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. Violation Number Comment Rule 892 The center's safe sleep policy was not posted in a prominent place in the infant room where parents and caregivers were able to view daily. The center's adopted safe sleep policy was not posted in the classroom designated for infants as required by the rule. .0606(b) 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. Two staff members did not complete the training within the first 90 days of employment. The training has been completed after 90 days of employment. .1102(g) Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. All flexibilities expired, except for the flexibilities related to the star rated license. During today’s visit I reviewed the DCDEE website provider documents and forms. Please be reminded all new staff members must complete the Recognizing and Responding to Suspicions of Child Maltreatment training within the first ninety (90) days of employment. You stated you understood and did not have any questions at this time. I reminded you that the health and safety training courses have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded that the health and safety training courses are required to be completed every five years. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. A checklist was used to note the requirements I monitored today. During the visit the Owner/ Administrator Ms. Stokes assisted with the visit. Today I reviewed your email and address with you, and you stated nothing has changed. This program currently operates with a four -star license issued January 16, 2020. The hold harmless has been extended to June 2024 and if you have any questions, please contact me. Please be reminded your facility is in Cohort 2 and your prep year is 7/1/24-6/30/25. Your rated license year is 7/1/25-6/30/26. Please use your prep time to make sure all staff’s education has been submitted to WORKS for evaluation. Remember you may apply for a rated license at any time. Upon arrival there were a total of thirty children present once I completed the enrollment worksheet. In space 2 the preschool and school aged children share there were 20 children present, and the space can only hold 19 children. I reminded you were licensed for a capacity of 29 children for all three shifts. You stated there was a mix up with communication with a parent who dropped her four children off at 10:09am two preschools and 2 school aged. I arrived at the facility at 10:30am and you stated two of the four children were school aged who were to be picked up for summer school. You contacted the parent and let her know of the mix up with the summer school because it doesn’t begin until June 11, 2024. The parent came back and picked up all four children while I was present at the facility. A violation was documented for over capacity during my arrival at the facility and you came into compliance with your capacity while I was present. You also provided me with your correction compliance letter during the time of today's visit. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of hazardous products was also monitored for compliance. The staff and children records were also reviewed today. You stated you do not have any new staff members. The files were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, the toddlers and two- year- old children were playing with toys in their classroom. The preschool children were observed playing outside on the playground. The school aged children were observed drawing and then went outside later in the visit. The theme in the lesson plans was insects. The lunch menu for today consisted of fish sticks, bread, macaroni and cheese, pineapple, and milk. Your last fire drill was completed 5/26/24 and your last monthly outdoor playground inspection was dated 5/31/24. Your last EPR drill was a lockdown drill dated 5/26/24. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last fire inspection was dated 11/30/23. Your last sanitation inspection was completed 10/30/23 with a superior rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed.
