A checklist was used to note the requirements I monitored today. During the visit the Administrator Ms. K. Hamm accompanied me on the walk- through of the facility and assisted with the visit. This program currently operates with a four-star license issued March 25, 2019. I monitored the indoor and outdoor environments for compliance with child care requirements. Staff/child ratios, supervision, use of space and capacity were monitored for compliance. The permit, sanitation, evacuation, safe arrival, and departure procedures, first aid chart, menus, activity plans, daily schedules, emergency care plan, emergency telephone numbers, staff records, summary of NC Child Care Law, were all posted at the time of the visit. The storage of medication and hazardous products was also monitored for compliance. The staff and children’s records were also reviewed today. Files for new staff were monitored with a random selection of returning staff members. Prior to the visit I received your staff and training worksheets. Your signature on these forms will serve as verification that the information provided is accurate. A random selection of children’s files was reviewed, and the children’s record worksheet was completed. During the visit, Today the lesson plans were on fall activities. The children were observed playing in activity areas. The lunch menu consisted of wheat bread, chicken nuggets, green beans, applesauce, and milk. Your last fire drill and your last monthly outdoor playground inspections were dated 8/28/25. Your last EPR drill was a shelter-in-place completed 6/26/25. I reminded you that an EPR drill must be completed every three months and keep all paperwork on file for review. Your last sanitation inspection was completed 9/10/25 with an approved rating. Please contact Environmental Health and your local fire inspector within 30 days of the due date to request an inspection to be completed. Once your inspections have been completed, mail or email them to me. I recommend that you place your due dates on a calendar or spreadsheet to help you maintain compliance. The following violation was documented during the visit. Violation Number Comment Rule 1897 The child care administrator and all staff did not complete the Recognizing and Responding to Suspicions of Child Maltreatment training within 90 days of employment. One staff member with a hire date of 5/22/25 had not completed the training within 90 days of employment. .1102(g) The violation must be corrected immediately. A compliance letter is due to me within two (2) weeks of today’s visit. By September 30, 2025, please send me a letter addressing the violation, how the violation was corrected, and how compliance will be maintained in the future. In your letter, be sure to include your facility name, facility ID number and each item number for the violation cited. You may submit the letter to me electronically at carla.hanchey-edwards@dhhs.nc.gov Childcare licensing requirements are established to ensure a safe and healthy childcare environment. Therefore, it is important for you to be knowledgeable of all of the licensing requirements that apply to your type of program. It is your responsibility to maintain compliance with all of the applicable laws and rules at all times. The best way for you to make sure that you are meeting all requirements is to periodically review the childcare law and rules. Technical assistance regarding the following items were reviewed with you today. I reminded you the health and safety trainings have been relocated from Pro-Solutions to Moodle on the DCDEE website and they are free of charge. I discussed various other training resources, including those online and in-person. Remember continuing professional development training provides you and your staff with the knowledge and skills needed to provide a quality child care program. Please be reminded the health and safety training courses are required to be completed every five years. Technical assistance regarding the following items were reviewed with you today. CBC Provider Portal Technical Assistance & Notification to the Division of New Hires or Residents Technical Assistance Guidance As stated in G.S. 110-90.2 & .2703(r) child care operators are to notify the Division of any new child care providers working who were hired or moved into the child care facility within five business days. The process of notifying the Division has changed and is now captured in ABCMS. This change has been in effect since February 2024. Effective immediately, you will need to obtain a Business NCID and complete Provider Portal training in Moodle at https://www.dcdee.moodle.nc.gov/course/view.php?id=119. No action is needed on your part if you have completed the reference training and are currently using the ABCMS Provider Portal to update information regarding new hires or residents. Once the training has been completed and access has been given, you must verify your facility roster to ensure current staff are noted on the roster. This information should be updated in ABCMS in an ongoing basis as staff members are hired and when their employment is terminated. This satisfies the requirement to notify the Division of new child care providers working who were hired or moved into the child care facility within five business days. Upon my arrival the facility was having the children’s pictures taken and you stated they were almost finished. During today’s visit I completed a walk-through of the facility, and the owner Ms. H. Martin signed the documentation at the end of the visit. The Administrator Ms. K. Hamm was working in the infant/toddler room during the time of the visit. The group size, supervision, ratio, capacity and approved space were in compliance. The restrictions, license and emergency care plan were posted. First Aid/CPR, storage of hazardous materials and the NC Child Care Summary of the Law was posted. I monitored the program requirements, furnishings and equipment, and the outdoor area. The theme of the lesson plans was weather. During the visit you stated you had hired new staff since my last visit. The staff/training worksheet was completed and in compliance. The new staff were enrolled for ITS-SIDS training for Monday May 19, 2025, through Duplin County Partnership for Children. The lunch menu for today consisted of chicken with rice, mixed vegetables, fruit, and milk. The program currently operates with a three-star license issued March 7. 2018. During the visit I reminded you to make sure all staff have sent their education to WORKS to be evaluated. The hold harmless has been extended until further notice at this time. Your last fire drill and your last monthly outdoor playground inspection were completed 4/29/25.Your last EPR drill was a lockdown completed 3/27/25. I reminded you that an EPR drill must be completed every three months. Please keep all documentation on file for review. A violation was documented and corrected during the visit. Violation Number Comment Rule 533 Human milk, formula and other bottled beverages including sippy cups, sent from child's home were not fully prepared, dated, and labeled for the appropriate child. Today eight bottles were labeled with the child's name and dated 5/13/25. I conducted a walk-through of the facility unaccompanied. During the visit, I discussed the allegations with Hollie Martin, Owner, Kristen Hamm, Administrator, and additional staff members. Limited monitoring of child care requirements occurred during today’s visit. The following violations were observed during today’s visit. Violation Number Comment Rule 301 Minimum staff/child ratios and group sizes for the number and ages of children in care were not met. On April 16, 2025, a DCDEE representative observed a total nine infants and toddlers in the classroom for children ages six weeks to twenty-four months old with one staff member. The staff/child ratio required two staff members to be present. GS 110-91(7);.0713(a-d) 303 Children were not adequately supervised at all times. On April 16, 2025, a DCDEE representative observed a total of nine infants and toddlers unattended for approximately thirty seconds in the classroom for children ages six weeks to twenty-four months old. .1801(a)(1-5) 317 The staff/child ratios for children, two years and older, during naptime were not maintained by having at least one person in each room, visually supervising all children and the total number of required staff on the premises within calling distance of each room occupied by children. On April 16, 2025, there was one staff member supervising two separate classrooms of children age two years old and ages three to five years old. .1801(b) 1065 Child care providers scheduled to work in the infant room, did not complete ITS-SIDS training within two months of employment or did not complete the training every three years. Child care administrators did not complete the ITS-SIDS training within 90 days of employment and every three years thereafter. On April 16, 2025, a staff member provided coverage in the classroom for children ages zero months to one year old and had not completed the ITS-SIDS training within the required timeframe. .1102(f) Violations must be corrected immediately.
